Book excerpt: PURPOSE: Delayed Onset Muscle Soreness (DOMS) is a common response to activities involving lengthening contractions. Muscle inflammation is associated with DOMS and may play an integral role in protecting a muscle from damage and soreness in response to subsequent bouts of lengthening contractions. Research in animals has shown that prior exposure to passive stretching and isometric contractions of a muscle resulting in muscle inflammation attenuates the muscle inflammatory response following subsequent bouts of lengthening contractions. The purpose of this study was to determine whether passive stretching and isometric contractions in humans would reduce DOMS following a typical bout of resistance exercise. METHODS: Thirty untrained male subjects were assigned to a control (C), stretching (S) or isometric (I) contraction group (n=10/group). In the week prior to the typical resistance training bout subjects in S and I were exposed to 3 separate sessions involving 5 minutes of passive stretching or maximal isometric contractions, respectively. Passive and active soreness, thigh girth, and relaxed knee angle were assessed prior to intervention and on days 1, 2, 4, and 8 following the bout of resistance exercise. RESULTS: Passive and active muscle soreness increased similarly in all groups. However, active soreness returned higher values than passive soreness at several time points following resistance exercise. Peak soreness occurred at 48h post exercise. Thigh girth and relaxed knee angle reached their highest values at 5 minutes following resistance exercise however there were no differences between the groups. CONCLUSIONS: The present study demonstrates that a typical bout of resistance exercise is sufficient to cause measurable levels of DOMS in untrained subjects and that subjects are more sensitive to active measures of DOMS compared to a passive assessment. Nevertheless the passive stretching and isometric contraction interventions did nothing to reduce DOMS in the current subjects.

Book excerpt: The Social Security Administration (SSA) administers two programs that provide disability benefits: the Social Security Disability Insurance (SSDI) program and the Supplemental Security Income (SSI) program. SSDI provides disability benefits to people (under the full retirement age) who are no longer able to work because of a disabling medical condition. SSI provides income assistance for disabled, blind, and aged people who have limited income and resources regardless of their prior participation in the labor force. Both programs share a common disability determination process administered by SSA and state agencies as well as a common definition of disability for adults: "the inability to engage in any substantial gainful activity by reason of any medically determinable physical or mental impairment which can be expected to result in death or which has lasted or can be expected to last for a continuous period of not less than 12 months." Disabled workers might receive either SSDI benefits or SSI payments, or both, depending on their recent work history and current income and assets. Disabled workers might also receive benefits from other public programs such as workers' compensation, which insures against work-related illness or injuries occurring on the job, but those other programs have their own definitions and eligibility criteria. Selected Health Conditions and Likelihood of Improvement with Treatment identifies and defines the professionally accepted, standard measurements of outcomes improvement for medical conditions. This report also identifies specific, long-lasting medical conditions for adults in the categories of mental health disorders, cancers, and musculoskeletal disorders. Specifically, these conditions are disabling for a length of time, but typically don't result in permanently disabling limitations; are responsive to treatment; and after a specific length of time of treatment, improve to the point at which the conditions are no longer disabling.

Book excerpt: Delayed Onset Muscle Soreness (DOMS) is a common phenomenon experienced by individuals who perform unaccustomed exercise that typically involves an eccentric component. Soreness peaks between 24 and 48 hours post-exercise with residual soreness usually remaining beyond that time frame. Negative implications of DOMS include minimal to severe soreness, the inability to continue safe and effective training or performance, biomechanical alterations predisposing individuals to injury, and decreases in strength and power. There have been many clinical and therapeutic interventions utilized in an attempt to minimize DOMS and the negative impact on athletic performance. Exercise, therapeutic massage, cryotherapy, ultrasound, and anti-inflammatory drugs have all been tested as methods of determining an effective intervention strategy. The present research attempted to minimize the negative impacts that DOMS has on neuromuscular function by utilizing a specific exercise program as a treatment intervention. Twenty females between the ages of 19 and 35 participated in the present research. (Abstract shortened by UMI.).

Book excerpt: Physiological responses after maximal and submaximal exercise are routinely monitored in a plethora of diseases (e.g. cardiovascular diseases, cancer, diabetes, asthma, neuromuscular disorders), and normal populations (e.g. athletes, youth, elderly), while slower or irregular post-exercise recovery usually indicates poor health and/or low fitness level. Abnormal post-exercise recovery (as assessed via blunted post-exercise heart rate dynamics) helps to predict the presence and severity of coronary artery disease, while differences in recovery outcomes in athletes might discriminate between fit and unfit individuals. Disturbances in post-exercise recovery might be due to acute or persistent changes in: (1) adaptive responses mediated by the autonomic nervous system and vasodilator substances, (2) cellular bioenergetics, and/or (3) muscular plasticity. Preliminary evidence suggests possible role of time-dependent modulation of nitric oxide synthase and adenosine receptors during post-exercise recovery, yet no molecular attributes of post-exercise recovery are revealed so far. Currently several markers of post-exercise recovery are used (e.g. heart rate measures, hormone profiles, biochemical and hematological indices); however none of them meets all criteria to make its use generally accepted as the gold standard. In addition, recent studies suggest that different pharmacological agents and dietary interventions, or manipulative actions (e.g. massage, cold-water immersion, compression garments, athletic training) administered before, during or immediately after exercise could positively affect post-exercise recovery. There is a growing interest to provide more evidence-based data concerning the effectiveness and safety of traditional and novel interventions to affect post-exercise recovery. The goals of this research topic are to critically evaluate the current advances on mechanisms and clinical implications of post-exercise recovery, and to summarize recent experimental data from interventional studies. This knowledge may help to identify the hierarchy of key mechanisms, and recognize methods to monitor and improve post-exercise recovery in both health and disease.
