This trend is also seen in the African American college population, despite easier access to mental health resources. This report examines the central barriers experienced by African Americans in seeking out mental health services, which are most often seen in the literature. Barriers explored include stigma, the foundations of counseling, cultural mistrust, and attitudes and beliefs about mental health services. The barriers are then used as a framework to discuss preventive, developmental, and community interventions. Further suggestions are made as to how to best utilize interventions to reduce the barriers experienced by African American students in seeking mental health services.

Book excerpt: Research has shown that the African American community underuses professional mental health services and has generally more negative attitudes toward seeking help for mental health challenges than their European American counterparts. Stigma surrounding mental illness and the desire to keep challenges secret from others are two factors that have received much attention in the literature examining this phenomenon (Mishra, Lucksted, Gioia, Barnet, & Baquet, 2009; Thompson, Bazile, & Akbar, 2004; Larson & Chastain, 1990; Cramer, 1999). Further, social support has also been linked to help-seeking attitudes in the African American community, (Pickard, Inoue, Chadiha, & Johnson, 2011; Constantine, Wilton, & Caldwell, 2003). The current study assessed help-seeking attitudes, mental health stigma, self concealment, stigmatizing attitudes towards mental health and mentoring experience in 123 African American/Black college students. Mental health stigma, self-concealment, mentoring experience, and help-seeking attitudes demonstrated statistically significant bivariate correlations in expected directions. Further, a linear multiple regression analysis revealed stigma and self-concealment to be mediators in the relationship between mentoring and help-seeking attitudes. Finally, differences were discovered between mentoring type and mentoring experience. Students with both informal and formal mentors had the best mentoring experience in comparison to students with neither or informal mentor. Results from the current study further support the idea that mentoring relationships play a role in reducing stigma and self-concealment, which contribute to better help-seeking attitudes for African American college students. The importance of mentoring relationships to variables related to better well-being are discussed.

Book excerpt: Cultural mistrust and fear of potential racism from people in the medical profession may be contributing to a noted disdain for mental health counseling among African American men. The purpose of this study was designed to gain insight into how attitudes toward religious help-seeking, acceptance of change, cultural mistrust, and mental health stigma impact therapy utilization among African American male college students. The review of literature explored the theoretical frameworks followed by the historical perspective, epigenetics, family structure, masculinity concept and help-seeking behaviors, noting barriers, fear, stigmas, spiritual influence, and economic hardships. Acceptance of change-evolving action is not currently endorsed by male African American college students. This appears to be due to attitudes, cultural competence, mistrust, risks, and misunderstanding of potential benefits to therapy utilization. The information examined in the literature review provided a foundational framework for understanding the impact of barriers on African American male college students regarding utilization of therapy. Although there have been awareness efforts, prompted by the federal government and the counseling community, the problem of underutilization continues. A quantitative correlational study was conducted to explore if religious help seeking, acceptance of change, cultural mistrust and mental health stigma have a connection to decreased therapy utilization. The study significance focuses on minimizing barriers to therapy utilization while increasing understanding concerning the benefits of its use. The significance of the study is designed to influence health care outcomes and disparities. The methods and procedures used in this research are presented in eight sections: (a) overview, (b) design, (c) research questions, (d) hypotheses, (e) participants, (f) instrument, (g) data collection, and (h) summary.

Book excerpt: This study explores where African American college students find mental health support and why those supports are chosen. Greater knowledge of the sources of mental health support sought by African American college students can assist higher education institutions in adapting current services to meet the needs of this specific student population. A qualitative phenomenological approach was utilized, and the study's sample included twelve participants, 6 female and 6 male, from a large public four-year university in Texas. These participants, undergraduate students with ages ranging from 18 to 24, were given a survey and completed two semi-structured interviews throughout one semester. Results indicated that study participants were more likely to utilize informal than formal support for their mental health and many had no source of support. Family stigma, peer attitudes, as well as internal and external pressures all influenced participant's choices to seek support. Based on findings from the study, recommendations for two distinct groups, counseling center directors and higher education administrators, are also discussed.

Book excerpt: African American/Black college students are less likely to utilize mental health services, when compared to other races or ethnicities in the United States. It must be noted that in general, Black college students have unique experiences that affect their mental health. However, there is paucity of published research and information in the literature on the utilization of mental health services by this population of college students. This study was conducted to gain a better understanding and appreciation of the shared barriers and facilitators that affect Black students in seeking and utilizing mental health services at the main campus of California State University San Marcos. Primary data collection involved two qualitative focus group discussions with a total of 20 self-identified Black/African American undergraduate students who are enrolled for classes during the Fall of 2018 semester. Semi-structured interviews were administered on separate days to the two focus groups. The participants were asked questions that assessed their mental health knowledge, attitudes and beliefs, and personal experiences in accessing and utilizing mental health services on campus. Results from the study identified a total of 4 themes as being barriers to accessing and utilizing mental health services: racism/discrimination on campus, lack of Black mental health counselors on campus, cultural perceptions of mental health, and stigma. There were no facilitators identified in utilizing mental health services on campus. The results of the study suggest a greater need to: (1) employ more providers who are Black mental health counselors/psychologists, (2) explore the necessity for additional alternative mental health treatments for Black students, such as group discussions, or group therapy, etc., (3) train faculty members and campus staff on cultural competencies to minimize potential biases and discrimination, and (4) promote inclusiveness through mental health outreach for Black students. It is hoped that these findings may play a significant role in increasing mental health service utilization and treatment for Black students at CSUSM.

Book excerpt: This study evaluated clinical and cultural barriers to help seeking to identify which factor best predicted African American undergraduate student initial attitudes towards seeking professional psychological help (ATSPPH-short form). Predictor variables included anticipated utility, emotional openness, cultural mistrust, racial identity development, and stigma. Moderator variables controlled for prior counseling, gender and site. This study built upon prior research by evaluating clinical and cultural factors together and utilizing an information processing model to conceptualize data (Vogel et al., 2006). The focus on African American undergraduate students was based on retention problems for this population (American Council on Education, 2006) and underutilization of counseling by this population despite evidence of links between counseling & college persistence (Alford, 2000; Hamilton, 2006; Kearney, Draper & Barron, 2005; Trippi & Cheatham, 1991; & Wilson, Mason & Ewing, 1997). Sixty seven students from two Northeastern US colleges were surveyed. Direct correlation analysis found nine significant relationships with ATSPPH. Controlled for prior counseling use, anticipated benefit (r = .423, p=.001), Emotional Openness (r = .330, p=.010), and an Assimilationist Ideology of Racial Identity (r = .296, p=.022) were correlated with ATSPPH. Book excerpt: In recent years, scholars have moved beyond attributing academic difficulties to cognitive and personal characteristics of African American students, and have begun to consider the effects of the predominantly White university (PWU) setting on the educational and psychological outcomes of Black collegians. Unfortunately, the literature paints a bleak picture of the social context of African American students at PWUs which ultimately impedes students' academic persistence and achievement (Gloria, Kurpius, Hamilton, & Wilson, 1999) as well as psychological wellness (Prelow, Mosher, & Bowman, 2006). The psychological ramifications of social and educational conditions for African American students at PWUs, along with higher attrition rates would substantiate the tremendous use of campus mental health services by this student population. Yet, the literature reveals that even when services are easily accessible and are provided for free or at extremely discounted prices, African American students choose not to seek professional psychological help (Nickerson, Helms, & Terrell, 1994). Drawing upon the psychosociocultural theoretical framework proposed by Gloria and Rodriguez (2000), this study examined if variables specific to the PWU environment - minority student stress, out-group comfort, and cultural congruity - served as predictors of attitudes towards seeking professional psychological help in a sample of African American collegians. This study also analyzed if counselor racial preference served as a mediator between the predictors and help-seeking. Survey data were collected from 198 Black college students attending a large, PWU in the Southwest. Results revealed that cultural congruity was the only significant predictor of help-seeking attitudes, and counselor racial preference was not a significant mediator. Exploratory analyses indicated gender differences in the relationship between the psychosociocultural variables and counselor racial preferences. Implications for practice and research in counseling psychology are discussed.

Undergraduates of various ages and class standings were given surveys within their demographics pertaining to: acculturation, social support, stigma, and depression. The above factors were compared to ethnicity. The results supported that African Americans have a higher level of depression-stigma overall, especially those who have been enculturated into their traditional culture; none of the additional hypotheses were supported by the research.

Book excerpt: There were no significant differences between anticipated satisfaction of individual counseling and group counseling or between racially heterogeneous and racially homogeneous counseling group memberships. Findings of this study underscore previous research stating that African Americans prefer same-race counselors. The results also suggest that the type of counseling and composition of counseling groups may not be as salient to African American women as counselor-client racial similarity. Implications of this research suggest that the visual and physical presence of African American counselors within mental health and counseling agencies could make professional counseling more attractive, accessible, and credible for African American women. In addition, counselor-client racial matching may also reverse the underutilization of mental health services among people of color seeking professional counseling.

Book excerpt: "While African- American comprise of 13% of the United States Population (United States Census Bureau, 2010), Shin (2005) argues that individuals from the African-American population have been found to have less access to mental health services, a lower probability of receiving needed services, and a greater likelihood of receiving poor quality of care, thus influencing retention during treatment and the efficacy of counseling. One reason for this might be an issue related to being seen by someone who is not an African-American and thus the therapeutic alliance, a key to successful therapy, is weakened. Research is showing how vital therapeutic alliance is to outcome in counseling, so tracking the outcome of counseling with African-Americans based on the ethnic match of their counselors could give insight into the role match plays in making counseling effects (Duncan, Miller, Wampold, & Hubble, 2011). The present study examines the relationship of client-therapist match during treatment to therapeutic alliance, treatment outcome, and change in depressive and anxious symptoms. There was a total sample of 1286 clients from Richmont Graduate University (RGU)'s network of counseling centers in the Atlanta and Chattanooga areas. The subjects included 60 therapists, 9 were African-American and 51 Caucasian. There were 28 African American clients.
