Music therapy can reduce psycho-physiologic stress, pain, anxiety, and isolation while undergoing a major surgical procedure. This study determined the effect of music therapy in anesthesia recovery period among post surgical patient at the post anesthesia care unit of Tondo Medical Center. It utilized Pre-Test and Post test design for selected post surgical patients. The statistical measures used were mean, standard deviation and t-test for the pre-test/post-test design. Findings showed that there were significant differences in the physiologic responses after the intervention. Differences between the control and experimental groups were significantly noted. there were significant differences in the experimental group rather than the control group. Music therapy interventions would be an effective complimentary treatment to improve physiologic responses during the anesthesia recovery period.

Although many advances in post-surgical pain management have taken place, research proves a need for increased attention to optimize the patient experience. The purpose of this study was to compare pain levels in the post-operative patient that utilize music as an adjunctive pain control strategy to patients that utilize only traditional pain control methods for the initial 24 post-surgical hours. A conducted literature search utilizing the Grand Canyon University academic database resulted in 15 peer- reviewed articles. Applicable evidence-based data and essential supportive data proved to result from the search. A six-month pilot study plan, with incorporation of grounded theory, has been developed. Research states that music therapy has evidenced decreased pain, anxiety, and emotional responses, had positive effects on physiologic measures, and increased patient satisfaction. Research indicates that the distraction of music permits for improved patient satisfaction with the overall environment, enabling them to be distracted from common discomforts such as noise levels. Furthermore, music studies have demonstrated the necessity to focus on pain management approaches, as current methods are not meeting patient and staff expectations. Utilizing music therapy can have desired effects including decreased patient pain levels, decreased complications, and an improved post-surgical patient experience.

The following discussion will present a problem that exists within some post anesthesia care units. The lack of music within these settings may be responsible for increasing the amount of acute pain perceived by postoperative patients. Along with the problem, a solution that incorporates music within the post anesthesia care units to decrease the amount of pain and pain medications administered will be achieved. The method to implement music to decrease the patients' pain perceptions within the recovery setting will be presented and the method to evaluate the outcomes, as well as how the results will be disseminated will be discussed. Music has been used throughout history to heal and alleviate illness or distress within patients. Research has proven that the use of music throughout the postoperative phase of surgery can provide therapeutic effects by lessening pain sensations and distress within the postoperative patient. Music is an inexpensive, risk-free intervention that nurses can use to decrease the perception of pain and lessen medication side effects within the postoperative patient.

By reducing the patient's perception of pain, they are encouraged to become mobile earlier in the recovery period. This in turn proves to reduce the incidence of intestinal distress, deep vein thrombosis, pneumonia, and renal insufficiency. By reducing the incidence of these post-operative complications, patient length of stays are shorter and reported patient satisfaction is improved. It is hypothesized that the introduction of music in the post-op period will aid in the management of post-operative pain and lower the patient's level of anxiety in an effort to improve overall patient outcomes.

Materials and Methods: This is a prospective, randomized, double-blind study conducted in the operating theatre of visceral surgery at Sahloul Teaching Hospital over a period of 4 months. Patients aged more than 18 undergoing a scheduled surgery under general anesthesia were included. Patients undergoing urgent surgery or presenting hearing or cognitive disorders were excluded. Before induction, patients wore headphones linked to an MP3 player. They were randomly allocated into 2 groups: Group M (with music during surgery) and group C (without music). Hemodynamic parameters, quality of arousal, pain experienced, patient's satisfaction, and awareness incidence during anesthesia were recorded. Results and Discussion: One hundred and fourty patients were included and allocated into 2 groups that were comparable in demographic characteristics, surgical intervention type and anesthesia duration. Postoperative pain can reduce patients' mobility, affecting their motivation to participate in any rehabilitation activities. For patients having total join replacement, rehabilitation plays an important role in promoting their early recovery. Therefore, it is crucial for healthcare professionals to develop and evaluate intervention that can better control patients' post-operative pain. In the clinical setting that I am working, pharmacological method is the major means of postoperative pain management. However, many Chinese patients are reluctant to use analgesic to control their post-operative pain because of the side effects and adverse reactions of the drugs. In this dissertation, music therapy, a non-pharmacological method that can be managed by nurses, is adopted for postoperative pain control. It is used to promote rehabilitation for patients who have undergone total joint replacement. Objectives: The objectives of this study are (1) to review the published research articles that investigated the effects of music therapy in reducing the post-operative pain and promoting rehabilitation for patients having total joint replacement; and (2) to establish an evidence-based guideline for the use of music therapy by nurses to control postoperative pain and to facilitate rehabilitation for patients having total joint replacement. Methods: A comprehensive literature search on four electronic databases including CHINAL, Medline (OvidSP), PubMed and the British Nursing Index were conducted. A total of eight RCTs and two non-randomized controlled trials were eventually identified. Results: All the reviewed studies showed that music therapy has a statistically significant effect on reducing postoperative pain. Sedative or relaxation music (music which has no lyrics, sustained melodic quality; rate of 60-80 beats per minutes; absence of strong rhythms or percussion) are recommended in the guideline. The target setting is a total joint replacement centre in a public hospital of Hong Kong. The target clients are adult patients (aged 19 or above) that are referred by the orthopedic out-patient department and are pending for their total knee/ hip replacement in the center. The transferability and feasibility of the literature are high. The guideline is developed based on the evidence in reviewed literature. A pilot testing plan is established to detect the potential barrier and friction of the guideline before the implementation. After that, an evaluation plan for patients, healthcare providers and system outcomes was also proposed. Conclusion: An evidence based guideline is developed for the total joint replacement center. It is anticipated that, with the use of this guideline by nurses, it will not only facilitate better rehabilitation for patients having total joint replacement, but also enhances nurses' autonomy in their nursing practice. Book excerpt: This randomized controlled trial examined the effects of Music Therapy (MT), Music Medicine (MM), or Attention Control (AC) on physiological and psychological parameters of stress for adult and older adult patients receiving care in the Intensive Care Unit of a community general hospital. Previous studies have indicated effectiveness of music therapy or music medicine for these medical patients, but few data are available for music therapy interventions. This study was an attempt to add to available information about the effects of music therapy compared to the effects of music medicine or attention control for this patient population. Participants (twenty-eight adults, ranging in age from 37-83 years; not mechanically ventilated at the time of session) were randomly assigned to music therapy, music medicine or the attention control group. Repeated measures of heart rate, blood pressure, respiratory rate, oxygen saturation, and anxiety and pain levels were collected before the session, immediately after the session and at 60 minutes post-session. Anxiety was measured using the Faces Anxiety Scale, and pain was self-reported via a Visual Analog Scale. Post-session length of stay was collected from the participants' medical records. Overall, there were no significant interactions among study groups and outcome measures. There was a statistically significant difference between length of stay for music therapy participants and attention control. Over time from pre-session to post-session, statistically significant decreases in anxiety scores were measured for both music medicine and music therapy groups. Pain scores decreased for both music medicine and music therapy groups, however not significantly. Some medically beneficial effects of music therapy or music medicine were evident in the data.

Dependent variables were amount of pain medication administered (measured in morphine equivalents), hospital length of stay, and perceptual data. Subjects were fifty (N=50) women undergoing gynecologic surgery requiring a hospital stay of at least one night. Subjects were randomly assigned to an experimental music group (N=25) or control group (N=25). Results showed no significant differences for pain medication between groups. The length of hospital stay was significantly shorter for the music group. Subjects in the music group also reported their post-PACU pain levels to be significantly lower than the control group. Further results and implications are discussed.

Mechanical ventilation leads to decreased comfort. Pain and anxiety may increase during this treatment modality, and the literature suggests this may directly affect patient comfort levels. Music therapy as a nursing intervention within the context of comfort, pain, and anxiety of mechanically ventilated patients was investigated. This quasi-experiemental study used a pre-test and post-test design with subjects serving as their own control. The convenience sample consisted of 2 men and 3 women who were mechanically ventilated and fit the selection criteria. Dependent variables measured included comfort, pain, and anxiety. Physiologic dependent measures included heart rate, respiratory rate, and systolic and diastolic blood pressure collected at timed intervals. Comfort was measured using a modified version of the Hospice Comfort Questionnaire. Pain was measured using the Numerical Graphic Rating Pain Scale. Anxiety was measured using the Faces Anxiety Scale. Important preliminary quantitative results are provided by this pilot study. The mean, standard deviation, significance, and paired t-tests were compared for each tool to determine changes in scoring before and after the intervention and control. This was also completed for physiological data including systolic and diastolic blood pressure, respiratory rate, and heart rate. Paired t-tests showed no significant mean differences between two points of measurement on systolic, diastolic, heart rate, and respiratory rate in both the intervention and control groups. Comfort, anxiety, and pain scores before and after the intervention and control also did not demonstrate significance. The small sample size makes generalization of these findings impossible to the entire population of acute mechanically ventilated patients in Billings, Montana.
