Book excerpt: A dual-task paradigm involving a primary sensorimotor task and a secondary cognitive task was used to examine the effect of age and task difficulty on the relationship between balance and cognition. We investigated twenty healthy children (mean age = 9.0 +/- 0.8), 20 young adults (mean age = 20.1 +/- 1.0), and 20 older adults (mean age = 70.0 +/- 3.6). Equal number of males and females were recruited for each age group. The sensorimotor tasks consisted of standing either on a stationary or a moving platform with eyes closed or open and walking on the treadmill. Three difficulty levels of the cognitive task were performed: simple, Disjunctive, and choice reaction time tasks. Both sensorimotor and cognitive performance was examined separately in the single- and dual-task situations. A motion analysis system was used to measure postural sway. Reaction time and accuracy in the cognitive tasks were collected. The relative changes of performance from the single- to dual-task, dual-task costs, were also calculated. Results showed that children maintained balance performance at the costs of cognitive performance in dual-tasking. Interestingly, children showed a decrease in cost as the cognitive task difficulty increased. The simplest cognitive task was associated with a high cost in the cognitive domain. Children exhibited greater postural sway than young and older adults in the static balance tasks and had greater dual-task costs on balance in the easiest standing task, but decreased costs when performing the sensorimotor task with their eyes closed. Young adults only showed an increase in reaction time in the most difficult standing task. In contrast to children, older adults maintained their cognitive performance at the expense of their balance performance, exhibiting greater postural sway especially in standing on a moving platform with eyes closed and walking. Generally, children and older adults experienced difficulty in dual-task and demonstrated the opposite pattern of performance trade-offs in dual-tasking. Task difficulty played a fundamental role in determining the relation between balance and cognition. The findings of this study suggest that dual-task training in walking might be a good way to enhance ability for performing multiple tasks.

Book excerpt: The increase in the aging population has become one of the most important problems of our society in last few decades. As people grow older, they are at risk of falling and consequent injuries due to the effects of aging. A fall may be the first indication of an undetected illness related to the effects of aging. This study demonstrates the effects of aging on balance, spatio-temporal gait parameters, gaze stability, and cognitive skills under single task conditions and during dual-tasks conditions. In the present study, we included following three groups: Group 1: 30 young healthy adults (aged 20 ± 3 years); Group 2: 30 adults (aged 61.4 ± 4.4 years); Group 3: 30 older adults (aged 75 ± 4.5 years). A computer game based rehabilitation platform has been developed and was used for the single and dual task performance in standing and during treadmill walking. We observed that there was a significant age effect while dual tasking on standing balance, spatial and temporal gait parameters, gaze performance, and cognitive task performance. To conclude, this study shows a vast decline in walking and standing balance and ability to divide attention during dual tasking between the age groups 55-70 years and 71- 85 years and compares both these age groups with the more normative, healthy, young and athletic, 20-30 years old population.

In the literature, numerous attempts have aimed at theorizing about the specific characteristics of executive functions that control interference between simultaneously and/or sequentially active component of task-sets in these situations. However, these approaches have been rather vague regarding explanatory concepts (e.g., task-set inhibition, preparation, shielding, capacity limitation), widely lacking theories on detailed mechanisms and/ or empirical evidence for specific subcomponents. The present research topic aims at providing a selection of contributions on the details of executive functioning in dual-task and task switching situations. Book excerpt: As the aging population degree is deepened, cognitive impairment has become a globally recognized public health problem. As an intermediate state from normal cognition (NC) to Alzheimer's disease (AD), mild cognitive impairment (MCI) has a highly variable cognitive trajectory, which contains three outcomes: 1) progression to AD and other types of dementia; 2) Maintaining stability; 3) Reversal to NC. Reversal of cognitive function can be achieved by taking positive and effective measures. Current studies mostly focus on factors affecting MCI to AD. World Health Organization and Alzheimer’s Disease International have also proposed relatively mature guidelines for risk factors. However, there are still some influencing factors that have not yet formed a unified conclusion. In addition, there are fewer studies and no consensus on the influencing factors for MCI to NC. Current forms of intervention for MCI are mainly non-pharmacological interventions, and there is a lack of randomized controlled trials with larger sample sizes and longer intervention periods to confirm the effect of pharmacological and non-pharmacological interventions. The purpose of this study is to explore the factors that influence the transition from MCI to AD or NC in aging adults, and to examine how the influencing factors make the cognitive function of aging adults with MCI deteriorate, improve or even reverse to NC and how are their reversal rates. This includes effective measures proven through pharmacological and non-pharmacological intervention studies and their impact on reversal. Influence factors include but are not limited to demographic factors (age, socioeconomic status, education), lifestyle (smoking, alcohol consumption, physical activity, nutrition, social participation), mental health (depression, loneliness), diseases (hypertension, diabetes, sleep disorders), biological markers (ß-amyloid, tau protein), and pharmacological factors (cholinesterase inhibitor, Aß monoclonal antibodies, drugs to rebalance the gut flora), etc., and submission of research results based on intervention trials is encouraged.
