Book excerpt: Numerous studies have examined music therapy and its effect on relaxation in the operating room, post-anesthesia units, and critical care units, but studies on music therapy in endoscopy are limited. This study examined the effect of soft music during a colonoscopy and its' relationship to the amount of conscious sedation required by the patient. A Convenience sample of thirty-eight patients undergoing a colonoscopy in the Ambulatory / Endoscopy unit were examined. Nineteen patients who received the soft music were compared to a group of nineteen patients who did not receive the music therapy. A medication log sheet was utilized to record the amount of conscious sedation that each patient received. Each patient that agreed to participate in the study signed a consent form. The t-test was utilized to test the level of significance for this study. Each of the three medications used was examined individually. The first medication, Demerol, revealed a t-value of 1.9 which was not significant at p=.06. The second medication, Versed, resulted in a t-value of 2.6, p=.01 which was statistically significant. Recommendations for further study include a larger sample size, a reduction in visual distractions, limiting study to a more narrow age group, and examining gender differences.

It utilized Pre-Test and Post test design for selected post surgical patients. The statistical measures used were mean, standard deviation and t-test for the pre-test/post-test design. Findings showed that there were significant differences in the physiologic responses after the intervention. Differences between the control and experimental groups were significantly noted. there were significant differences in the experimental group rather than the control group. Music therapy interventions would be an effective complimentary treatment to improve physiologic responses during the anesthesia recovery period.

Patients experience anxiety related to the uncertainty of any surgical procedure. Increased levels of pain and anxiety can lead to negative effects both physiologically and psychologically. Narcotics and sedatives are most commonly administered to alleviate both of the conditions. When given in large amounts and coupled with anesthetic drugs, adverse effects such as over-sedation can occur. This is an adverse effect that can be problematic and costly for not only the patient, but the nurse and facility as well. Respiratory depression and over sedation go hand-in-hand. When over sedation occurs, a patients respiratory effort can be greatly impacted. Over-sedation and other adverse effects can prolong not only the recovery period, but ultimately delay patient discharge. By adding a non-pharmacologic intervention such as music therapy to the treatment plan, the nurse will be able to reduce pain and anxiety, thus reducing the amount of narcotics and sedatives required. This can be used as a nursing driven intervention to be implemented in the pre-operative phase and follow the patient through the post-anesthesia phase. With the appropriate implementation plan, stakeholder acceptance, and resources, music therapy can be a beneficial influence on physiological parameters and psychological state. Music therapy can provide relaxation, lessen muscle tension, and provide a healing influence for patients. Music therapy can have a great impact on not only patient satisfaction, but anesthesia outcomes as well. Literature has shown that music therapy used as a complementary therapy has been proven as an inexpensive way to reduce perioperative pain and anxiety.

Postoperative pain can reduce patients' mobility, affecting their motivation to participate in any rehabilitation activities. For patients having total join replacement, rehabilitation plays an important role in promoting their early recovery. Therefore, it is crucial for healthcare professionals to develop and evaluate intervention that can better control patients' post-operative pain. In the clinical setting that I am working, pharmacological method is the major means of postoperative pain management. However, many Chinese patients are reluctant to use analgesic to control their post-operative pain because of the side effects and adverse reactions of the drugs. In this dissertation, music therapy, a non-pharmacological method that can be managed by nurses, is adopted for postoperative pain control. It is used to promote rehabilitation for patients who have undergone total joint replacement. Objectives: The objectives of this study are (1) to review the published research articles that investigated the effects of music therapy in reducing the post-operative pain and promoting rehabilitation for patients having total joint replacement; and (2) to establish an evidence-based guideline for the use of music therapy by nurses to control postoperative pain and to facilitate rehabilitation for patients having total joint replacement. Methods: A comprehensive literature search on four electronic databases including CHINAL, Medline (OvidSP), PubMed and the British Nursing Index were conducted. A total of eight RCTs and two non-randomized controlled trials were eventually identified. Results: All the reviewed studies showed that music therapy has a statistically significant effect on reducing postoperative pain. Sedative or relaxation music (music which has no lyrics, sustained melodic quality; rate of 60-80 beats per minutes; absence of strong rhythms or percussion) are recommended in the guideline. The target setting is a total joint replacement centre in a public hospital of Hong Kong. The target clients are adult patients (aged 19 or above) that are referred by the orthopedic out-patient department and are pending for their total knee/ hip replacement in the center. The transferability and feasibility of the literature are high. The guideline is developed based on the evidence in reviewed literature. A pilot testing plan is established to detect the potential barrier and friction of the guideline before the implementation. After that, an evaluation plan for patients, healthcare providers and system outcomes was also proposed. Conclusion: An evidence based guideline is developed for the total joint replacement center. It is anticipated that, with the use of this guideline by nurses, it will not only facilitate better rehabilitation for patients having total joint replacement, but also enhances nurses' autonomy in their nursing practice. These effects can impact the present and future surgical experiences for all patients. Many patients feel a lack of control postoperatively, which increases their anxiety and pain perceptions. Pain medications that are commonly used within the postoperative phase of surgery can be effective, but introducing an alternative method, such as the use of music may decrease the pain perceptions and amount of pain medications administered to each recovering patient. The following discussion will present a problem that exists within some post anesthesia care units. The lack of music within these settings may be responsible for increasing the amount of acute pain perceived by postoperative patients. Along with the problem, a solution that incorporates music within the post anesthesia care units to decrease the amount of pain and pain medications administered will be achieved. The method to implement music to decrease the patients' pain perceptions within the recovery setting will be presented and the method to evaluate the outcomes, as well as how the results will be disseminated will be discussed. Music has been used throughout history to heal and alleviate illness or distress within patients. Research has proven that the use of music throughout the postoperative phase of surgery can provide therapeutic effects by lessening pain sensations and distress within the postoperative patient. Music is an inexpensive, risk-free intervention that nurses can use to decrease the perception of pain and lessen medication side effects within the postoperative patient.

The Hong Kong Hospital Authority reported nearly 7000 patients suffered from acute myocardial infarction (AMI) in year 2010 to 2011. Percutaneous transluminal coronary angioplasty (PTCA) is a minimal access surgical treatment for coronary artery disease but studies have shown that patients experienced different levels of anxiety before and during PTCA which led negative impact to the patients. Music can be regarded as a safe, cost-effective therapy to reduce one's anxiety level. It can be carried out by nurses without any specific technique. However, there was no systemic review for using music therapy to patients undergoing PTCA. Therefore, this dissertation aims to evaluate the best available evidence on using music therapy for patients undergoing PTCA. Four electronic databases, Medline, CINAHL, Embase & PsycINFO, were searched for studies to investigate the efficacy of music therapy in patients undergoing PTCA. There were seven studies were eligible with data extracted and quality assessment performed by the critical appraisal skill programme (CASP) checklist. Four studies were graded as high quality, which consistently demonstrated a statistically significant more reduction in the anxiety level of patients who received music therapy either before and/or during PTCA than those who had no music therapy. Consequently, an evidence-based guideline of using music therapy for patients undergoing PTCA was developed according to the guideline development process of SIGN (2010). The use of music therapy is transferable and feasibly in the local setting with little manpower concern. The total cost estimated for running the innovation for a year was HKD 83,775 which was considered as cost-effective to reduce patients' anxiety level and avoid associated adverse events. A 14-months programme including communication with the stakeholders, pilot testing, staff training and clinical application of the proposed music therapy was designed. Qualitative and quantitative data on patient, healthcare provider and system outcomes would be accounted throughout the programme. The effectiveness of the guideline would be determined by the reduction of patient's anxiety level by using the Chinese version of State-Trait Anxiety Inventory (STAI) (Shek, 1993). Moreover, healthcare provider outcome would be evaluated by questionnaire and the expenditure of the proposed music therapy would be monitored closely. Book excerpt: Background and Goal of Study: This prospective, randomized, open-label, controlled study was designed to compare dexmedetomidine-based to midazolam-based sedation after coronary artery bypass graft (CABG) surgery in the cardiac intensive care unit. Materials and Methods: After approval of IRB and written informed consent, forty patients scheduled for elective CABG surgery were randomly assigned to receive midazolam (0.03-0.1 mg/kg i.v. bolus intermittently) or dexmedetomidine (0.7 u03bcg/kg/hr. i.v. continuously). The study drug was administered from the time of sternal closure until weaning from mechanical ventilator. An infusion rate of dexmedetomidine or additional dose of midazolam was determined based on maintaining a RASS (Richmond Agitation-Sedation Score) more than 3. We recorded sedation score, time to extubation, ICU stay time, hemodynamics with cardiac index, and various side effect every 2 hours. Results and Discussion: The patient treated with dexmedetomidine showed significantly lower level of sedation than midazolam (P=0.008, Fig. 1). Average times to spontaneous respiration (865u00b1181 min vs 884u00b1272 min, P=0.862), to weaning from mechanical ventilator (1,020u00b195 vs 973u00b1276 min, P=0.642), and to ICU staying (75u00b18 vs 69u00b18 hr, P=0.177) were similar between midazolam and dexmedetomidine group, respectively. The hemodynamic variables (MAP, cardiac index, pulmonary artery pressure and etc.) and incidence of side-effects (delirium, shivering, nausea, vomiting, and pain) did not differ between the groups. Conclusions: Sedation with dexmedetomidine 0.7 u03bcg/ kg/hr i.v. provide effective sedation for post-CABG surgical patients, in particular, who need a deeper level of sedation. Nevertheless, patients who received sedation with dexmedetomidine did not show delayed recovery and any other side effects.
