Book excerpt: ABSTRACT: Children undergoing medical procedures frequently experience severe distress, which may be ameliorated through procedural support music therapy. Parent and nurse behaviors have been noted to influence child behaviors during medical procedures, yet no prior studies were found which examined the impact of procedural support music therapy on caregiver behaviors. The purpose of the present study was to examine the effects of a live, cognitive-behavioral music therapy intervention on the distress and coping behaviors of young children undergoing immunization, as well as the behaviors of parents and nurses. Participants were children between the ages of 48 and 72 months (N = 58) who underwent immunizations, as well as their parents and the nurses who administered the immunizations. Each parent completed a demographic inventory describing their child's previous medical experiences. The researcher classified children as “high distress” or “low distress” based on parents' responses to the demographic inventory. Parent/child dyads were then randomly assigned to receive music therapy (n = 29) or standard care (n = 29) during their immunization, with an equal number of high distress (n = 10) and low distress (n = 19) children assigned to each condition. Afterward, each parent rated their child's distress compared to previous medical experiences and rated the level of pain their child experienced on a scale from 0 to 10. All procedures were videotaped. Research assistants created transcripts from the videotapes of procedures. Trained observers watched the videos and labeled each child and adult behavior using the categories of the Child-Adult Medical Procedure Interaction Scale-Revised (CAMPIS-R). Child behaviors were classified as coping, distress, or neutral. Parent and nurse behaviors were classified as coping-promoting, distress-promoting, or neutral. Rates and proportions of child and adult behaviors were calculated. Book excerpt: Pre-operative anxiety is a common and anticipated response to patients expecting to undergo anesthesia in preparation for surgery. Pre-operative parental anxiety can lead to increased anxiety in children, which is associated with physiologic alterations in the child. Pre-operative video education is an inexpensive method to increase knowledge and decrease pre-operative parental anxiety related to the perioperative process. The first goal of this project was to evaluate the effectiveness of a pre-operative video in increasing knowledge of the perioperative experience in parents of children anticipating surgery by viewing a pre-operative video. Parental knowledge was measured before and after viewing the pre-operative video. Scores were analyzed using a paired sample t-test to determine if there was a significant difference between pre and post-test parental knowledge. There was a significant difference between the two scores (p = 0.004). The mean knowledge score pre-intervention (M = 14.16, SD = 2.035) was lower than the mean knowledge score post-intervention (M = 15.40, SD = 1.118), t (24) = 3.228. The second goal of this project was to evaluate the effectiveness of viewing a pre-operative video in decreasing pre-operative parental anxiety in parents of children anticipating surgery by viewing pre-operative video. Parental anxiety was measured before and after viewing the pre-operative video. A paired sample t-test was used to determine if there was a significant difference between pre and post-test anxiety levels. There was a significant difference between pre and post-test parental state anxiety scores (p = 0.000). The mean anxiety score pre-intervention (M = 41.6, SD = 12.783) was higher than the mean anxiety score post-intervention (M = 33.04, SD = 9.914), t (24) = 4.381. Therefore, this project successfully addressed knowledge and stress (or anxiety as measured by the STAI). The video intervention allowed parents to learn, and even see, what their child would experience the day of surgery. By providing parents with this opportunity, their knowledge of what they and their child could expect the day of surgery increased, thereby decreasing the anxiety they were experiencing regarding the upcoming surgery. Children often show preoperative anxiety in the process of moving away from their parents to the operating room. Complementary therapies have been used to decrease children's anxiety, but to decrease the anxiety is still challenging. The aim of study is to evaluate the effect of wagon to stretcher car for reducing children's anxiety in the operating room. Methods: We assigned 60 children (aged 2-7 years) undergoing general anesthesia for the elective surgery to two groups. Stretcher group underwent general anesthesia after transferred to the operation room by conventional stretcher car, Wagon group used a wagon to transfer children. Each child's anxiety was evaluated three times using the Modified Yale Preoperative Anxiety Scale (mYPAS): at waiting room (T0), on the hallway to the operating room (T1), and at operating room (T2). Results: In the Stretcher group, there were significant increase in mYPAS during transportation after separation of parents (T1, P = 0.003) and in the operating room (T2, P = 0.004) compare to waiting room (T0). Repeated measured ANOVA showed that wagon prevented the increase of mYPAS during transportation (P = 0.012), but did not in the operating room (P = 0.087) compared with stretcher car. There was significant difference in numbers of children who increased mYPAS scores during transportation after separation of parents (T1, P = 0.014) and in the operating room (T2, P = 0.08) compare to waiting room (T0). Children in the Wagon group showed a reduction (P= 0.003) in mYPAS scores at hallway to the operating room and in the operating room compared to those in the control group. Age group significantly affected at mYPAS scores (P = 0.047). Younger children group showed higher mYPAS scores than older children group at T1 (P = 0.033). Wagon prevented the increase of mYPAS during transportation in older children group (P = 0.018), but did not in younger children group (P = 0.169). Conclusions: A majority of children scheduled for surgery had an anxiety during transportation to the operating room and before induction of anesthesia in the operating room. Younger children had more anxiety than older children. Wagon as a transporting method decreased this anxiety suggesting that it may be a good alternative choice for reducing childrenu2019s preoperative anxiety.
