Dame Carol Black estimated that the annual economic cost of ill-health in terms of working days lost and worklessness was over £100 billion. The Confederation of British Industry (CBI) estimated that last year 172 million working days were lost due to absence, costing employers £13 billion. Against a backdrop of a wider economic downturn both taxpayers and businesses can ill afford to bear these largely unnecessary costs. But the cost of ill-health cannot be measured in pounds and pence alone. There are about 2.6 million people on incapacity benefits and 600,000 people make a new claim each year; of these, half had been working immediately before they moved onto benefit. Once out of work it is likely that an individual's health will worsen and they and their families are more likely to fall into poverty and become socially excluded. Therefore, health-related inactivity prevents individuals from fulfilling their potential, causes needless financial hardship, and damages the communities in which people live. This response is built around three key aspirations that demonstrate how the Government will rise to the challenges set by Dame Carol: creating new perspectives on health and work (chapter 2); improving work and workplaces (chapter 3); and supporting people to work (chapter 4). Chapter 5 details how the Government will measure progress against its targets, and chapter 6 sets out future steps that the Government intends to take: better integrating skills, health and employment provision; reviewing the incentives for individuals, employers and the state to tackle sickness absence to ensure they are optimally balanced; and continuing to develop strategies to address the specific needs of those with poor mental health.

Book excerpt: Providing an overview of researchers' and practitioners’ “confessions” on the fascinating phenomenon of failed or derailed organizational health and well-being interventions and contextualizing these confessions is the aim of this innovative volume. Organizational intervention failures, paradoxes and unexpected consequences can offer a lot of rich and extremely useful practical lessons on intervention design and implementation and possibly on the design of future research on organizational interventions. This volume presents lessons learned from derailed interventions and provides possible solutions to those tasked with implementing interventions. It provides an open, practical and solutions-focused account of researchers' and practitioners' experiences in implementing organizational interventions for health and well-being.

Book excerpt: The aim of this review was to provide an evidence base for policy development on vocational rehabilitation - defined as whatever helps someone with a health problem to stay at, return to and remain at work. The focus was on adults of working age, the common health problems that account for two-thirds of long-term sickness (mild/moderate musculoskeletal, mental health and cardio-respiratory conditions) and work outcomes (staying at, returning to and remaining in work). Data from some 450 scientific reviews and reports were included in evidence tables. The review demonstrates that there is a strong scientific evidence base for many aspects of vocational rehabilitation, a good business case for it and more evidence on cost-benefits than for many health and social policy areas. Generic and condition-specific findings are reported, and practical suggestions offered for the differing types of people affected by health problems. Vocational rehabilitation should be a fundamental element of government strategy to improve the health of working age people.

Book excerpt: While specific services have demonstrated effectiveness in assisting individuals with disabilities achieve successful employment outcomes, there is currently limited knowledge regarding the widespread use of evidence-based practices (EBP) within the state-federal vocational rehabilitation system. This study identified specific organizational and service practices within the public Mississippi Vocational Rehabilitation (VR) program intended to help inform further development of evidence-based practices for use within the VR program nationally. The Mississippi VR program was selected for study given that its employment outcome rate consistently exceeds 70%, compared with a national outcome average of 58% among other state VR programs. The nature of this research was exploratory and findings illustrate and appropriately reflect examples of key practice as identified by participants, rather than infer explanation or causality. A case study using modified Consensual Qualitative Research (CQR) methodology allowed researchers to identify organizational components contributing to a culture of performance including (a) a data-driven philosophy, (b) a focus on rapid service intervention, (c) active partnership with employers, (d) specialized caseloads and coordinators, (e) business model and agency structure, (f) active and intentional internal and external agency communication, and (g) a commitment to ongoing staff training at all levels in the organization. Additionally, soft skills' training was identified as a service practice recommended for further study to determine its efficacy, effectiveness, and efficiency within the state-federal VR program. A substantive lack of extant evidence-based practice specific to the VR system emerged through the findings and further development of specific evidence-based service delivery practices for implementation within the VR system is recommended. Findings also reflect previous research results indicating a lack of practitioner awareness and identification of evidence-based practice, therefore training and education of rehabilitation counselors on the tenets and skills involved in identifying, adopting, and implementing EBP is warranted. Finally, systematic study of organizational practices and frameworks to assist administrators, counselors, policy makers, and researchers appropriately identify, implement, and measure EBP within the VR system should to be considered.

Available in PDF, EPUB and Kindle. Book excerpt: As occupational therapy celebrates its centennial in 2017, attention returns to the profession's founding belief in the value of therapeutic occupations as a way to remediate illness and maintain health. The founders emphasized the importance of establishing a therapeutic relationship with each client and designing an intervention plan based on the knowledge about a client's context and environment, values, goals, and needs. Using today's lexicon, the profession's founders proposed a vision for the profession that was occupation based, client centered, and evidence based--the vision articulated in the third edition of the Occupational Therapy Practice Framework: Domain and Process. The Framework is a must-have official document from the American Occupational Therapy Association. Intended for occupational therapy practitioners and students, other health care professionals, educators, researchers, payers, and consumers, the Framework summarizes the interrelated constructs that describe occupational therapy practice. In addition to the creation of a new preface to set the tone for the work, this new edition includes the following highlights: a redefinition of the overarching statement describing occupational therapy's domain; a new definition of clients that includes persons, groups, and populations; further delineation of the profession's relationship to organizations; inclusion of activity demands as part of the process; and even more up-to-date analysis and guidance for today's occupational therapy practitioners. Achieving health, well-being, and participation in life through engagement in occupation is the overarching statement that describes the domain and process of occupational therapy in the fullest sense. The Framework can provide the structure and guidance that practitioners can use to meet this important goal.

Book excerpt: This text provides an overview of vocational rehabilitation (VR) practice, making it the perfect companion for students and practitioners with an interest in supporting people back to work and improving their sense of health and well-being. The book is divided into three parts: the first covers the policy context of VR in the UK, defining VR, outlining the development of national standards in the sector, and looking at issues such as the economy and worklessness, and the legal background. The second part examines models of VR practice and relevant standards. It explores the nature of developing services in the public and private sectors, illustrated by case studies from a range of disciplinary backgrounds. The final part presents a detailed introduction to the knowledge and skills required in providing a VR service, including consideration of the multidisciplinary processes and stages involved. Introduction to Vocational Rehabilitation includes numerous case studies and a dedicated chapter of issues and questions to aid reflection. Comprehensive and evidence-based, this is the first multidisciplinary textbook for students and practitioners from a range of backgrounds, including occupational therapy and health, physiotherapy, human resources, nursing, social work and health psychology.
