Book excerpt: Generalized anxiety disorder (GAD) is a chronic condition characterized by an excessive and uncontrollable worry. GAD has a highly negative impact on all aspects of life, and the costs for society are considerable. Cognitive behaviour therapy (CBT), delivered face-to- face or online, has been shown to be effective in treating GAD. However, there are still room for improvement, with treatment content and format of delivery being two areas to further investigate. Internet-based treatments are often based on a generic CBT model of GAD and include treatment strategies such as cognitive restructuring and relaxation. Furthermore, the impact of therapist support has shown inconclusive results. Overall, this thesis aimed to evaluate the effects of a new internet-based treatment for GAD, based on acceptance, mindfulness, and valued action, delivered with different types of support. Study I was a small pilot trial with a within-group design. The study included 16 participants diagnosed with GAD who received the newly developed treatment program with weekly therapist support. Statistically significant and large effect sizes were observed on self-rated worry at post-treatment, and the effects were maintained at a three-months follow-up. Study II included 103 participants diagnosed with GAD who were randomized to nine weeks of treatment with the treatment program and weekly support, or a waiting-list control group. Statistically significant moderate to large effects were observed on primary and secondary outcomes in favour of the treatment. The exception was a measure of quality of life, with no significant difference between the two groups. The treatment effects were maintained at a six-months follow-up. Study III was a pilot trial including 33 participants diagnosed with GAD. A within-group design was used to investigate the effects of the treatment program delivered with automated messages and support on demand. Significant and large within-group effects were observed on all outcome measures at post-treatment with the exception of quality of life, for which there was a small effect. Study IV was a pilot factorial design trial that compared the acceptance-based treatment program against a self-tailored treatment, as well as two types of support: scheduled support and support on demand. The study included 85 participants with GAD randomized to four different treatment groups. Significant moderate to large effects were observed in all treatment groups, with no statistically significant differences between the groups on self-rated outcome measures. Receiving scheduled support was rated as more positive than support on demand, and self-tailored treatment was rated as more positive than the acceptance-based treatment. Treatment satisfaction was high in all studies. In conclusion, the studies indicate that an internet-based treatment based on acceptance, mindfulness, and valued action is a viable option in the treatment of GAD. Sammanfattningsvis är acceptansbaserad internetförmedlad behandling ett effektivt alternativ vid behandling av GAD.

Although evidenced-based behavioral treatments are available, less than 20% of those with SAD receive treatment (Grant et al., 2005). The disparity between the number of individuals affected and those obtaining treatment is due to a number of factors, including limited accessibility to therapists practicing evidence-based interventions such as cognitive behavior therapy, geographic factors, and financial cost. Internet-based interventions may be utilized to overcome some of these barriers. Guided Internet-based therapeutic interventions have been demonstrated to be effective for social anxiety (e.g., Andersson et al., 2006). The optimal role (if any) of the therapist in such programs, including the amount of therapist time necessary for effective treatment, remains unclear. The purpose of this pilot study was to develop a novel Internet self-help CBT intervention and to assess the preliminary efficacy and acceptability of the program with minimal therapist support delivered through a common videoconferencing platform, for the treatment of SAD in adults. The intervention program is derived from an acceptance-based CBT program that utilizes traditional behavioral interventions (e.g., exposure) within the context of a model emphasizing mindfulness and psychological acceptance. Thirteen participants received the Internet-based self-help intervention consisting of eight weekly modules, and a brief weekly videoconferencing therapist check-in. Participants were assessed at pre-treatment, mid-treatment, and post-treatment on both outcome and process measures. Participants rated the treatment program as highly acceptable. The results indicate that participants experienced a significant reduction in SAD symptoms and improvements in functioning and quality of life. Implications and future directions are discussed.

Book excerpt: This dissertation investigated the effectiveness of a newly designed intervention for adolescents with test anxiety. This CBT-based intervention is comprised of two parts: self-guided psychoeducational computer modules, and an online social networking group. The intervention was designed to provide adolescents with an effective means of reducing test anxiety symptoms, delivered through a familiar medium--the Internet. This study was conducted in classrooms in Houston, TX, with high school student participants. Participants were randomly assigned to one of three groups: those receiving the computer modules component only, those receiving the computer modules and social networking group, and those receiving no treatment (control). Test anxiety levels were assessed with the self-report Test Anxiety Inventory (Spielberger, et al., 1980). Changes in test anxiety levels over time were compared to changes in a measure of academic achievement. The author compared changes in test anxiety level among the three groups using repeated measures ANOVA. It was hypothesized that participants in the two treatment groups would show improvements in test anxiety symptoms over time, while those in the control condition would not. It was further hypothesized that decreases in test anxiety symptoms would correlate with increases in academic achievement. Participants in all three groups showed some decrease in anxiety symptoms. There were no statistically significant differences between groups; however, there were some intriguing trends in the computer module only group, which showed a clinically meaningful, but not statistically different, decrease in anxiety. There was no association between changes in test anxiety symptoms and academic achievement. Results of this study suggest that this intervention may not currently present an effective option for treating the symptoms of test anxiety. Results may be used to make data-driven improvements to address the limitations and unforeseen weaknesses of this intervention approach.
