Book excerpt: In the recent past, terrorist organizations have used different chemical, biological, and radiological (CBR) weapons to pursue their own agendas. In 1995, the Aum Shinrikyo cult released sarin onto the Tokyo subway system, killing 12 and injuring hundreds. In 2001, anthrax spores were sent through the U.S. Postal Service killing five postal employees and infecting 22 others. The increasing complexity and scale of these incidents suggest the possibility of a large scale attack with a Weapon of Mass Destruction (WMD) causing thousands of casualties on US soil. Since these attacks are difficult to prevent and may happen anywhere and at any time, mass decontamination is one of the key elements to managing the consequences of such an event, saving lives, and limiting the number of injuries. These Guidelines are an update of the United States Army Soldier and Biological Chemical Command (SBCCOM) - now the United States Army Edgewood Chemical Biological Center (ECBC) - January 2000 Guidelines for Mass Casualty Decontamination During a Terrorist Chemical Agent Incident and January 2002 Guidelines for Cold Weather Mass Decontamination During a Terrorist Chemical Agent Incident. These Guidelines supersede these original 2000 and 2002 Guidelines and are expanded to include all chemical, biological and radiological hazards. These Guidelines represent the latest evolution in our approach to mass casualty decontamination. These Guidelines were developed for first responders to provide information and suggested procedures for mass casualty decontamination following a hazardous material (HAZMAT)/WMD attack. There is no perfect solution to mass casualty decontamination and no single process or method can account for all variables (e.g., hazard, time, number of victims, environmental conditions, resources). These updated Guidelines are intended to identify a simple, consistent mass decontamination process that could be applied with reasonable effectiveness to any HAZMAT/WMD incident. In other words, use the fastest approach that will cause the least harm and do the most good for the majority of the people. These Guidelines are based on exposure to all hazards and focus on civilian mass casualty decontamination. These Guidelines primarily focus on chemical, biological, and radiological (CBR) agents, but also include Toxic Industrial Chemicals (TICs), Toxic Industrial Materials (TIMs), and toxins (collectively referred to in this document as HAZMAT/WMD). Guidelines do not cover each type of threat individually, however, the basic principles outlined are applicable to all HAZMAT/WMD situations. This document addresses decontamination of an overwhelming number of victims resulting from a HAZMAT/WMD incident in a population center. Mass casualty decontamination requires a slightly different approach than the individual technical and equipment decontamination applied during typical HAZMAT incidents. These Guidelines are designed for use during the first minutes of a mass casualty HAZMAT/WMD incident to reduce contamination and minimize casualties.

Book excerpt: Individuals may have to rely on themselves to protect their own health and safety--perhaps even their own lives--in the event of a terrorist attack. Even those who know how to take care of themselves in the event of a fire, tornado, or earthquake might not know what to do in case of a chemical, radiological, nuclear, or biological attack. This quick guide attempts to fill in some of these gaps by offering specific actions to take during each of these situations, as well as preparations that can be taken. These actions are appropriate regardless of the likelihood of an attack, its scale, or the current government alert level; are designed to be sensitive to potential variations; and have been defined in terms of simple rules that should be easy to follow. A reference card included at the back of the guide encapsulates the key points and can be removed for display in a prominent place.

Book excerpt: This book provides a comprehensive overview of the medical and operational management of blast and explosive incidents affecting civilian populations. It incorporates global lessons learned from first responders, emergency medicine providers, surgeons, intensivists, and military specialists with deep experience in handling blast injuries from point of injury through rehabilitation. The book begins with background and introductory information on blast physics, explosion types, frequency, and perspectives from the military. This is followed by a section on prehospital management focusing on medical and trauma responses, triage, psychological consequences, and operational considerations. It then examines the roles of the emergency department and ICU with chapters on planning and training, surge capacity, resilience, management of common injury types, contamination, and ventilator strategies. The next section covers surgical treatment of a variety of blast injuries such as thoracoabdominal, extremity and vascular, and orthopedic injuries. The book then discusses medical treatment of various injury patterns including lung, abdominal, extremity, and traumatic brain injury. The final section of the book covers post-hospital considerations such as rehabilitation, mental health, and community resilience. Throughout, case studies of recent incidents provide real-life examples of operational and medical management. Operational and Medical Management of Explosive and Blast Incidents is an essential resource for physicians and related professionals, residents, nurses, and medical students in emergency medicine, traumatic surgery, intensive care medicine, and public health as well as civilian and military EMS providers.

Book excerpt: With our highly connected and interdependent world, the growing threat of infectious diseases and public health crisis has shed light on the requirement for global efforts to manage and combat highly pathogenic infectious diseases and other public health crisis on an unprecedented level. Such disease threats transcend borders. Reducing global threats posed by infectious disease outbreaks – whether naturally caused or resulting from a deliberate or accidental release – requires efforts that cross the disaster management pillars: mitigation, preparedness, response and recovery. This book addresses the issues of global health security along 4 themes: Emerging Threats; Mitigation, Preparedness, Response and Recovery; Exploring the Technology Landscape for Solutions; Leadership and Partnership. The authors of this volume highlight many of the challenges that confront our global security environment today. These range from politically induced disasters, to food insecurity, to zoonosis and terrorism. More optimistically, the authors also present some advances in technology that can help us combat these threats. Understanding the challenges that confront us and the tools we have to overcome them will allow us to face our future with confidence.

Book excerpt: A Complete Training Solution for Hazardous Materials Technicians and Incident Commanders! In 1982, the authors Mike Hildebrand and Greg Noll, along with Jimmy Yvorra, first introduced the concept of the Eight-Step Process© for managing hazardous materials (hazmat) incidents when their highly regarded manual, Hazardous Materials: Managing the Incident was published. Now in its revised fourth edition, this text is widely used by fire fighters, hazmat teams, bomb squads, industrial emergency response teams, and other emergency responders who may manage unplanned hazardous materials incidents. As a result of changing government regulations and consensus standards, as well as the need for terrorism response training, Mr. Noll and Mr. Hildebrand have modified and refined their process of managing hazmat incidents and added enhanced content, tips, case studies, and detailed charts and tables. The Revised Fourth Edition contains comprehensive content covering: • Hazard assessment and risk evaluation • Identifying the problem and implementing the response plan • Hazardous materials properties and effects • Identifying and coordinating resources • Decontamination procedures • The Eight-Step Process© • Personal protective equipment selection • Procedures for terminating the incident The Revised Fourth Edition’s dynamic features include: • NFPA 1072 and 472 Correlation Guide for the Hazardous Materials Technician and Hazardous Materials Incident Commander levels • Correlation matrix to the National Fire Academy’s Fire and Emergency Services Higher Education (FESHE) Bachelor’s (Non-Core) Managerial Issues in Hazardous Materials Course (C0274) • Realistic, detailed case studies • Practical, step-by-step skill drills • Important hazardous materials technician and safety tips Also available to support Hazardous Materials: Managing the Incident, Fourth Edition: • Hazardous Materials: Managing the Incident, Fourth Edition Field Operations Guide • Hazardous Materials: Managing the Incident, Fourth Edition Student Workbook • Navigate TestPrep: Hazardous Materials: Managing the Incident • Hazardous Materials: Awareness and Operations, Third Editio

This book was released on 2014-08-21 with total page 132 pages. Available in PDF, EPUB and Kindle. Book excerpt: Chemical, Biological, Radiological, Nuclear, and high-yield Explosive (CBRNE) events have the potential to destabilize governments, create conditions that exacerbate violence or promote terrorism. This can trigger global repercussions. These events can quickly overwhelm the infrastructure and capability of the responders, especially in countries that do not have the specialized resources for response like those available in the United States. When a CBRNE incident occurs in a partner nation or other foreign country, the U.S. is often called upon to provide assistance. Interoperability - the ability to work together - among U.S. agencies, foreign governments, and responders involved in the effort is key to an efficient response. The effectiveness of the U.S. response and approach to CBRNE events in partner nations depends on the capability of the U.S. government to provide timely and appropriate assistance and the resilience of the partner nation to a CBRNE event. An All-of-Government Approach to Increase Resilience for International Chemical, Biological, Radiological, Nuclear, and Explosive (CBRNE) Events is the summary of a workshop convened in June 2013 by the National Institute of Standards and Technology and the National Research Council to discuss ways to strengthen the U.S. ability to prepare for and respond to CBRNE events that occur in U.S. partner nations. The workshop brought together diverse experts and stakeholders to identify capabilities that are necessary for responding to an international CBRNE event; discuss best practices and resources needed for improved interoperability of the U.S. and partner nation during response to a CBRNE event; and identify key questions that need to be addressed in follow up activities focused on improving U.S. CBRNE response in partner nations.
