Among the recommendations are: that BBC guidance needs a clearer delineation of responsibilities for risk management; that the main themes of risk management are not aligned with corporate objectives; that the BBC should update its assessments of the risks of working in hostile environments, as the abduction of journalist Alan Johnson showed; by failing to comply with its own Broadcasting Code, the BBC was fined by Ofcom over the a live phone-in competition on Blue Peter, and illustrates that some programme makers are ignoring the BBC's own editorial guidelines, exposing the corporation to reputational risk; the BBC has not related its risk to corporate objectives or assigned all risks to named owners; that BBC managers at all levels are not sufficiently engaged in the management of risk; there is still no fully satisfactory regime under which the BBC is accountable to Parliament for the value for money with which it spends licence fee payers money.

The Programme is designed to transform the way in which BBC staff create, use and share video and audio material. It involves the development of new technology to allow staff to manage content efficiently on their desktops, in order to give greater accessibility of digital content for audiences on TV, online and radio. The BBC has made good progress in delivering the programme in-house since it terminated its contract with Siemens. It is now on course to deliver the complete technology by summer 2011. With hindsight, the BBC should not have let the contract for its Digital Media Initiative to Siemens without testing the contractor against other suppliers, especially as there was a high degree of innovation involved. The Programme is no longer expected to deliver the overall net financial benefit of £17.9 million originally anticipated. The BBC approved the Programme on the basis that it would cost £81.7 million and deliver benefits of £99.6 million, but now forecasts costs of £133.6 million and benefits of £95.4 million - a net cost of £38.2 million. The Committee welcomes the Trust's assurance that it would now take a more challenging approach when considering procurements but are concerned with the ease with which the BBC found over £50 million in savings to make up for the losses it suffered through late delivery of the project and its own increased delivery costs. This suggests the need for a more vigilant approach to value for money.

However, the scale of some of the allowances paid to staff to relocate to Salford is difficult to justify. There were 11 cases where the cost of relocating staff exceeded £100,000 per person, with one costing £150,000. The BBC also failed to make a proper record of the exceptions it made to its allowance policy. The longer term success of the move to Salford depends on the BBC achieving the wider benefits it promised. These include reducing the gap between Northern and Southern audiences in the BBC's market share and stimulating economic and other regional benefits, including creating up to 15,000 jobs. The BBC should set clearly defined expectations for its relationships with its commercial partners and make clear that they must pay their fair share of tax. The BBC's decision to enter into a 10-year contract with the Peel Group for studio space at Salford seems to take little account the fast pace of change in the broadcasting industry. The BBC could end up having to pay for studio services it no longer needs and become overly dependent on them. There is also dismay at the abandonment of the BBC's Digital Media Initiative at a cost to the licence fee payer of £100 million. Book excerpt: The Public Accounts Committee concludes that the BBC's Digital Media Initiative (DMI) was a complete failure. The DMI was a transformation programme that involved developing new technology for BBC staff to create, share and manage video and audio content and programmes from their desktops. Siemens were contracted to build the DMI system, but the contract was terminated and brought in-house in 2009. But the BBC failed to complete the DMI Programme and in May 2013 cancelled it at a cost to licence fee payers of £98.4 million. The BBC was far too complacent about the DMI's troubled history and the very high risks involved in taking it in-house. The DMI was 18 months behind schedule when the BBC took it in-house from Siemens. The BBC did not obtain independent technical assurance for the system design or ensure that the intended users were sufficiently engaged with the Programme. Poor governance meant that these important weaknesses went unchallenged, even when things started to go badly wrong. Projects like the DMI need to be led by an experienced senior responsible owner who has the skills, authority and determination to see the project through to successful implementation. The BBC needs to report using clear milestones that give the Executive and the Trust an unambiguous and accurate account of progress and any problems. The BBC Executive should apply more rigorous and timely scrutiny to its major projects to limit potential losses and the BBC Trust must be more proactive in chasing and challenging the BBC Executive's performance.

Book excerpt: Tax lost through the hidden economy could be over £2 billion and involve some 2 million people. HM Revenue & Customs (HMRC) spent 41 million in 2006-07 on encouraging people and businesses into the formal economy, detecting and imposing sanctions on those operating in the hidden economy, achieving a return/cost ration of 4.5:1. HMRC detects some 30,000 hidden economy cases a year, a detection rate of only around 1.5 per cent, but the amount of tax recovered has increased by 13 per cent in real terms since 2003-04. Areas of risk include: self-employed builders and decorators who often receive cash payments; individuals who trade on the internet; and buy-to-let landlords. To increase detections HMRC is making more use of data matching techniques, and the Tax Evasion hotline received over 120,000 calls in 2006-07, but progress in investigating cases has been slow with only 2000 completed against a target of 5,500. HMRC can impose penalties of up to 100 per cent of tax owed, but usually imposes a lower penalty or waives them. Prosecutions are not given much publicity, limiting their wider deterrent effect. Advertising campaigns to encourage people to declare tax owed have led to 8,300 registrations bringing in extra tax of £38 million over three years. Offshore Disclosure arrangements have been even more successful following landmark rulings requiring financial institutions to release details of around 400,000 offshore accounts. Some 45,000 people came forward bringing in around £400 million at a cost of £6 million, a return of £67 for every £1 spent.

Book excerpt: Since their introduction under the Criminal Justice Act 2003, community orders have offered courts the ability to impose a range of 12 possible 'requirements', including accredited programmes (such as anger management courses or alcohol and drug rehabilitation), unpaid work in the community and supervision by the National Probation Service. There is little information available nationally on the effectiveness of community orders. On the key measure of reconviction, figures from the Ministry of Justice showed that for those sentenced to community orders, their actual reconviction rate was significantly lower than those sentenced to custodial sentences for similar offences. There is, though, no basic information such as national data on whether offenders have completed their community orders, nor on why offenders have failed to complete them. The National Probation Service has set national standards but these are applied inconsistently. The Ministry's current method of funding Probation Areas is unsatisfactory and slow to respond to changes in demand from the courts and it is felt that there is a need for a more flexible system. On the basis of a report by the Comptroller and Auditor General, the Committee examined the Ministry of Justice on increasing effectiveness of community orders; building the confidence of both the court and the community in community orders; improving the funding formula; and tightening adherence to the requirements of orders.

It looks at protection of consumers through removal of price controls by regulators, examines the benefits of this decision to different groups of consumers and the challenges of regulating these markets. The regulators Ofcom, Ofgem and Postcomm have statutory objectives requiring them to protect consumers through the introduction of competition, where appropriate. Between 2002 and 2006, each removed retail price controls from the following: fixed line telephone provision; gas and electricity supply; special delivery postal services for business account users. Once price controls are removed, regulators rely on consumers to switch suppliers, so in theory rewarding companies who offer good service and competitive prices. For this to work, consumers need good information about the different suppliers, must be able to switch supplier easily, have confidence in the market to believe changing supplier will make a difference and, when necessary, obtain redress if the company behaves anti-competitively. Regulators need to ensure the competition is working effectively and that there is protection for vulnerable consumers, especially at a time of large increases in energy prices and telecoms prices above those of most countries.
