The number of operations since J annary 1884, has reached 75. In the earlier operations iodoform was used and in the last cases inundation with corrosive sublimate solution, and this seems to have shortened the time required for healing. In the last eleven exenteration with corrosive subli mate the average number of days in the clinic has been seven and a fraction, while in twenty preceding cases treated with iodo doform the average was more than eleven days. The objection that exenteration requires a longer stay in the clinic than enu cleation has no weight if the exenteration can exclude the cases of death following the enucleation, and this question can only be determined by long experience. Painful stumps, through irrita tion of the ciliary nerves, occur occasionally following enuclea tion, and if they be sometimes found after exenteration this cannot condemn the operation; as yet, however, no instance of a painful stump has occurred; the ciliary nerves are, probably at least, as well protected from irritation when the sclera remains as when it is removed. There are no haemorrhages or other changes in the fundus. October 2, 1886. Yesterday I thought the pupils a shade smaller, but this was the only change I could detect. To-day the pupils are decidedly smaller and there is undoubtedly light perception. The child shrinks somewhat from the full glare of day, and from my hand swept rapidly before here eyes. October 3, 1886. Pupils about the same, but I am sure she now recognizes me. October 4, 1886. Vision about the same; pupils smaller. Allowed to go home. October 15, 1886. Father of child tells me that her sight came back gradually but steadily; the sight and the pupils now seem to him normal. November 9, 1886. Dr. Hanson writes to me: "According to request that I inform you as to the final issue of the case of little S. P., whom I sent to you some time ago for quinine amaurosis, I am glad to report that she has entirely recovered. Improvement, which set in rather suddenly, a few days after her return from the city, rapidly progressed to complete restoration of sight." February 23, 1887. Child was brought to my office today. The pupils seemed a little dilated and the ophthalmoscope showed decidedly pale optic nerves with small retinal vessels, HHAs. The mother says the child holds objects very near to examine them and seems sometimes to have difficulty in finding small objects, but she thinks she noticed all this before the attacks of quinine amaurosis, and believes the child to be near-sighted as both she (the mother) and the father are. July 3, 1888. The pupils are now certainly larger than normal, about two-thirds of maximum I should say. The optic discs are bluish-white and the arteries small. Vision = finger counting at 20 feet. The mother says she does not think the child's sight perfect by any means. March 2, 1889. No material change. March 25, 1891. Appearances unchanged. She holds her books very near. Adams both eyes were normal in all respects, the excursions of each separately, were perfect and all the associated movements equally good. She declared that with the right eye she saw two distinct images of a pencil held at twenty inches, separated by an interval of some inches and on the same plane. He further adds that he has since seen a little boy with a high degree of myopia who claimed to be the subject of the same phenomenon. It is definitely stated, however, that this phenomenon was observed in the right eye only when the left was closed. No statement is made, however, as to Whether the closure of the non-affected eye in the woman's case was necessary to develop the phenomenon. The boy is said to have perceived images of double the number of small objects such as coins that were placed before him, only, however, when the left eye was closed. Mr. Adams, erroneously I think, supposed that with a prism placed base upwards or downwards before the non-affected eye the boy should have, if he were not simulating, seen three objects instead of two. He sums up his remarks by saying that up to the present time (1881) he had never met with a case in which uniocular diplopia, in an eye that was physically perfect, was alleged to exist except under circumstances where there was strong ground for doubting the validity of the patient's statements. It would have been pertinent to have asked if Mr. Adams had ever before seen a case in which uniocular diplopia was claimed without lesion of the affected eye. Such practical support certainly portends an enduring future of the prism-diopter, about which so much pro and con has been written since its first appearance in ophthalmic literature.2 In this paper it will be taken for granted that the reader is at least familiar with the principles of the prism-diopter, so that only the relation which exists between it and the lens-diopter will here be repeated, to-wit: A Lens Which Is Decentered One Centimeter Will Produce As Many Prism-Diopters As The Lens Has Diopters Of Refraction. A knowledge of this law will frequently enable the oculist to change the form of his prescription so that it may safely be entrusted for execution to any optician capable of locating the optical center of a lens, and who is provided with no other instrument of measure than a pocket centimeter scale. As the optician, generally speaking, is not allowed to exercise his own judgment in transforming prescriptions, it is necessary that the oculists instructions be explicit respecting the proper method of putting his prescription into the best practical form.
