Consideramos que los fundamentos de la terapia sistémica individual no difieren de los que se aplican en pareja o en familia, sin embargo, su desarrollo comprende un estado no presencial y completamente auténtico del entorno. Las bases biológicas de la depresión mayor se entrelazan con los aspectos sistémicos, lo cual conlleva al tratamiento fármaco y psicoterapéutico. La valoración de la evolución se realiza mediante escalas y cuestionarios que permiten considerar la calidad de vida internamente y en relación con los sistemas de pertenencia. La respuesta al tratamiento puede ser relativamente rápida, de dos a cuatro semanas, evidenciada por las variaciones en el puntaje de los instrumentos de seguimiento. El reconocimiento de la propia situación y la experticia del tratante deben orientarse hacia la ausencia de síntomas y de signos de depresión, la remisión, que se logra en tiempo más prolongado según el caso y que previene el deterioro subsecuente a la presencia de estados residuales.
