Book excerpt: Introduction : la perte des dents maxillaires postérieures va entrainer une résorption de l’os alvéolaire et une pneumatisation du sinus maxillaire. Plusieurs techniques d’augmentation osseuse existent, dont certaines mini-invasives par voie crestale. Objectifs et méthodes : une revue de littérature des articles publiés principalement dans Pubmed depuis moins de 10 ans a permis de décrire différentes techniques mini-invasives d’augmentation osseuse par élévation du plancher sinusien avec un abord crestal. Résultats : les différentes techniques mini-invasives montrent qu’avec de faibles hauteurs de crêtes (3 mm) on obtient des gains de hauteur conséquents (8 à 10 mm) avec de faibles complications et des taux de survie implantaires standards. Discussion : avec une technique sans matériau de comblement et à partir de faibles hauteurs osseuses, on obtient des gains de hauteur permettant de poser des implants de 10 mm, toujours en place dix ans après. Si la technique piézochirurgie est utilisée sans matériau de comblement, on peut penser que la voie crestale peut être une solution thérapeutique simple et sécure. Cependant, l’élévation de la membrane sinusienne est réalisée et aveugle. Conclusion : les résultats cliniques de ces techniques mini-invasives sont très satisfaisants. Il n’y a pas aujourd’hui une technique qui en surpasse une autre. De nombreux développements amélioreront la sécurité des chirurgies mini-invasives, les rendant plus accessibles aux praticiens et plus acceptables aux patients.
