Book excerpt: A review of experimental and clinical literature indicates a link between staff emotional health and treatment efficacy. This program evaluation was designed to examine the change in staff attitudes, the level of burnout, and opinions about mental illness, which might be brought about by a new training protocol instituted at a large Midwestern psychiatric hospital (TPMHC). Staff members from 2 psychiatric units were involved in a mandatory retraining seminar that focused on crisis prevention and intervention techniques and team building exercises. Thirty-five staff members volunteered to fill out the Maslach Burnout Inventory (Maslach, Jackson, & Leiter 1996), the Attitudes Towards Health Care Teams Scale (Heineman et al., 1999), and the Opinions about Mental Illness (Cohen & Struening, 1962). The volunteer staff members completed these surveys before the training session and again 10 months later. These measures were also completed by 15 volunteer staff members from another unit who received a different amount of training. The scores on these scales were compared before and after training, between units, and with pre and post intervention analyses of security staff involvement in restraint and PRN medication (medication administered as needed) administration. Institutional constraints (e.g., alterations in the training program timing, and an unexpected increase in the length of time between pre and posttest measurements of staff variables) affected the completion of this study. T tests were used to predict positive mean differences in the pre and posttest condition (independent t test) and between groups condition (paired t test). To examine the non-parametric data taken from security reports, a sign test for 2 independent samples was used to compare the medians of the experimental units and the independent unit. A sign test for 2 correlated samples was utilized to compare the medians of the pre and post measurements on the experimental units. Although interference from extraneous variables render conclusions from this study tentative, useful information was gleaned from an examination of the process involved in completing this study. The baseline data collected by this study also provide an excellent point from which to gauge future growth, and examine the effectiveness of future training.

Book excerpt: In their daily work, psychiatric nurses are subjected to patient-perpetrated verbal and physical aggression. They manage a high level of work stress. As compared to their colleagues working in civil settings, forensic psychiatric nurses may experience different rates of patient aggression and work stress. Such experiences have implications for the mental health and productivity of nursing staff. In inpatient settings, homicide by a patient is a rare event. Representing the most severe form of patient aggression, patient homicide may be expected to have significant effects on the psychological well-being of psychiatric nurses. The present cross-sectional study comprised two separate phases. The broad aims of the first phase were to explore and compare the psychological well-being of civil and forensic hospital-based psychiatric nurses and to uncover factors contributing to and buffering against the development of posttraumatic stress reactions and general distress. Owing to the occurrence of a double patient homicide in a forensic psychiatric hospital where this study was underway, the study was expanded to include a second phase. The broad aims of the second phase were to evaluate the effects of patient homicide, on posttraumatic stress reactions and general distress in forensic psychiatric nurses, both working across the entire hospital and working on the unit where the patient homicide occurred. In the first phase, a battery of self-report measures was administered to 97 forensic psychiatric nurses in a forensic psychiatric hospital and 99 civil psychiatric nurses across three civil hospitals. In the second phase of the study, a slightly modified questionnaire was re-administered to 107 forensic nurses from the same forensic psychiatric hospital, five months after the occurrence of patient homicide. Multiple regression analyses indicated that while working in a civil setting is a risk factor for reporting higher levels of work stress, civil and forensic nurses experience similar psychological well-being and demonstrate comparable coping strategies and attitudes. As a group, 14-17% of civil and forensic nurses met diagnostic criteria for posttraumatic stress disorder (PTSD), and 36% scored above the threshold for psychiatric caseness. A tentative model of the development of posttraumatic stress reactions and general distress was developed, illustrating that frequency of aggression and level of work stress have direct effects on both posttraumatic stress reactions and general distress. The moderating roles of coping strategies, attitude to personality disorder and attitude towards patient aggression were also highlighted in the model. Following the occurrence of patient homicide, forensic nurses in general demonstrated resilience, not displaying increased (or decreased) symptoms of posttraumatic stress or general distress. In comparison, there were trends for higher proportions of nurses working on the unit where the homicide occurred to meet criteria for PTSD and to score above the threshold for psychiatric caseness after the patient homicide, as compared to before the homicide. The present study affirms that psychiatric nursing is a challenging and stressful occupation and highlights risk factors for poor psychological well-being in mental health nurses. Limitations of the study, implications for employers and individual nurses, and directions for future research are discussed.

The first was carried out in 1986 by the Health and Safety Commission Health Services Advisory Committee (1987). They conducted a comprehensive survey of the inddence of violence to 5000 workers in five separate health districts. The results from the 3000 people who eventually replied made many in the caring professions worried. One in 200 workers had suffered a major injury following a violent attack during the previous year and a further one in ten needed first aid following an assault. Other surveys also showed high risks: of sodal service staff, 6% had suffered an attack in the past 5 years (Saunders, 1987), and sodal workers were at even high er risk. 29% had been assaulted in the last 3 years (Rowett, 1986). In addition, 4% of general practitioners had experienced an attack resulting in injury in the past year (D'Urso and Hobbs, 1989). Clinical psychologists were also at risk - 53% had been assaulted at least once during their professional career and 18% in the past year (Perkins, 1991). Media reports of extreme violence seem to be the tip of the iceberg. Many staff are attacked and some of these attacks have serious physical or psychological consequences that interfere with the victim' s ability to return to their full working capadty. This loss of highly trained staff should be recognized by employers and the community.

Participants from 15 countries attended. Since care for persons with mental illness (schizophrenia, major depression, bipolar disorder, delusional disorder, atypical psychoses) has been deinstitutionalized, some persons with these disorders are committing crimes and serious violence. Consequently, societies around the world are confronted with a new challenge: to provide mental health care and social services to mentally ill persons in a humane way that will prevent illegal behaviours. Research in this field has been dominated by investigations designed to improve clinicians' accuracy in predicting violent behaviours, with little attention focused on the organization and implemen tation of treatments. The premise of the Advanced Study Institute was that treatments must have em pirically proven efficacy. Both professional ethics and public accountability require empirical evidence that each treatment will alleviate the problem that it targets. However, despite the fact that Western industrial societies provide treatment for mentally ill persons who have offended, there is a very limited base of knowledge on what constitutes effective treatment and how such treatments should be organized and delivered. The Advanced Study Institute was an attempt to stimulate and encour age research that will extend this knowledge base. The goals were to review what is known about mentally ill offenders and about effective treatments for them, and to provide a framework for the orientation of future investigations designed to improve treatment efficacy.
