Book excerpt: This article provides a critical review about the challenges that taxes on sugary drinks as an instrument of health policy must face to reverse the trend of the current epidemics of obesity. We analyzed the experiences of the leading countries, particularly Mexico, and reflect on the counterweight exerted by the industry against obesity policies, and on the power of lobbyists.Those tax policies for public health have to overcome the enormous strength of the industry, which is exerted in several-science and research, brand reputation, influence on regulators-levels. We suggest that a specific tax on sugary drinks has enough potential to reduce noncommunicable diseases and risk -diabetes mellitus, hypertriglyceridemia, lipoproteins, LDL, blood pressure- via reduced consumption thanks to the high price elasticity of those drinks. Furthermore, the effects are amplified even in the medium term, once established new habits to healthier eating. These taxes could encourage business innovation without inflicting costs of lost jobs and contributeto reducing the social gradient in obesity.

Book excerpt: This paper analyses the potential signalling effect from a tax on sugar-sweetened beverages (SSBs) that Catalonia introduced in May 2017, with a view to understand the ways in which, and the extent to which taxes discourage SSBs consumption. A questionnaire was distributed in November 2019 in two neighbourhoods from Barcelona with different mean income levels. We explored associations between variables constructed from the questionnaire and declared reduced consumption. Our goal was to disentangle the different effects from the price increase itself and from the signalling effect of the price increase on the reduction of SSBs consumption, analyzing results that account for socio-demographic characteristics, in order to provide policy recommendations. Respondents mainly declared that a higher awareness of SSBs’ health risks made them curb their consumption of SSBs (98.5 percent). Only 10.6 percent of them declared that the new price was considered high enough to have discouraged their SSBs consumption. Nevertheless, the tax in Catalonia seems to have had a signalling effect through the increase in prices (100 percent pass-through of the law), and much less through the awareness of the existence of the tax. The fact that people had noticed the increase in prices is associated to a recent knowledge of SSBs’ health risks, which is itself related to reduced consumption in the sample. Implementing higher and more salient SSBs taxes on the shelves, and having public campaigns making sure taxes are known among the entire population, aiming at behavioural changes among young populations, would enhance their signalling effect.

Book excerpt: Sugar-sweetened beverage excise taxes are an effective evidence-based noncommunicable diseases (NCD) prevention policy. Along with tobacco and alcohol excise taxes, they are a tool to attain the Sustainable Development Goals, and are recommended by the World Health Organization to modify behavioral risk factors associated with obesity and NCDs, as featured in the WHO Global Action Plan. Taxes on sugar-sweetened beverages have been described as a triple win for governments, because they 1) improuve population health, 2) generate revenue, and 3) have the potential to reduce long-term associated healthcare costs and productivity losses. Taxation of sugar-sweetened beverages has been implemented in more than 73 countries worldwide. In the Region of the Americas, 21 PAHO/WHO Member States apply national-level excise taxes on sugar-sweetened beverages and seven jurisdictions apply local sugar-sweetened beverage taxes in the United States of America. While the number of countries applying national excise taxes on sugar-sweetened beverages in the Region is promising, most of these taxes could be further leveraged to improve their impact on sugar-sweetened beverages consumption and health. This publication provides economic concepts related to the economic rationale for using sugar-sweetened beverage taxes and the costs associated with obesity; key considerations on tax design including tax types, bases, and rates; an overview of potential tax revenue and earmarking; evidence on the extent to which these taxes are expected to impact prices of taxed beverages, the demand for taxed beverages, and substitution to untaxed beverages; and responses to frequent questions about the economic impacts of sugar-sweetened beverage taxation.
