Most treatments for breast cancer are necessary to reach a cure but may be associated with adverse late and long-term effects that may impact a survivor's quality of life. The purpose of this research was to analyze the relationship between the types of breast cancer treatments received and a survivor's reported quality of life. The study population included 55 women ages 18 years and above diagnosed with early-stage, curable breast cancer that completed at least treatment for their cancer. Afaf Meileis' middle-range Transition Theory was the nursing theoretical framework for this study. The research design was non-experimental, quantitative, and retrospective. A descriptive correlational design was used to describe relationships among variables. The Quality of Life in Adult Cancer Survivors (QLACS) survey with demographic and treatment questions was mailed to eligible participants. Likert-scale responses and study variables were recorded and analyzed. Participants had the worst quality of life scores in the areas of dealing with stress, fear of recurrence, fatigue, and aches and pains. Differences in quality of life scores were noted between patients who received and those how had not received the most common breast cancer treatment options. Statistically significant relationships were noted between the study variables and quality of life scores. These research results provide a practitioner educating a newly diagnosed breast cancer patient new knowledge about the differences in quality of life by receiving or not receiving common breast cancer treatments." -- From page v.

Book excerpt: The economic burden of breast cancer for women under 50 in the United States remains largely unexplored, in part because young women make up a small proportion of breast cancer cases overall. To address this knowledge gap, we conducted a web-based survey to compare data from breast cancer survivors 18–39 years of age at first diagnosis and 40–49 years of age at first diagnosis.We administered a survey to a national convenience sample of 416 women who were 18–49 years of age at the time of their breast cancer diagnosis. We analyzed factors associated with financial decline using multivariate regression.Survivors 18–39 years of age at first diagnosis were more likely to report Stage II–IV breast cancer (P < 0.01). They also quit their jobs more often (14.6%) than older survivors (4.4%; P < 0.01) and faced more job performance issues (55.7% and 42.8%, respectively; P = 0.02). For respondents in both groups, financial decline was more likely if the survivor had at least one comorbid condition (odds ratios: 2.36–3.21) or was diagnosed at Stage II–IV breast cancer (odds ratios: 2.04–3.51).

Book excerpt: The objective of this research study is to explore trajectories of health-related quality of life (HRQoL) in older breast cancer survivors, along with their predictors. HRQoL is important because patients who show severe symptoms may wish to consider therapies or treatment plans that lead to better HRQoL. Older people are more vulnerable to low HRQoL scores since old age is associated with deteriorating health, multiple comorbidities, and low-socioeconomic status. To examine the HRQoL trajectory among older women with breast cancer, we used the data queried from the Surveillance, Epidemiology and End Results Medicare Health Outcomes Survey database. A total of 1,089 older ([greater than or equal to] 65 years) women who were diagnosed with breast cancer in 1998-2012 and participated in the survey before and after the cancer diagnosis were identified. HRQoL was measured using SF-36/VR-12 questionnaire and summarized as Physical Component Summary (PCS) Score and Mental Component Summary (MCS) Score. Latent Class Growth Mixture Modeling was conducted to identify distinct groups of women with a similar trajectory of HRQoL. The results showed that there were three latent classes of HRQoL trajectories for PCS: the high-declining (46.5% of the sample), mid-declining (36.0%), and the low-improving (17.5%). Two latent classes of HRQoL trajectories were identified for MCS: high-stable (76.5%) and low-declining (23.5%). The results showed that age at diagnosis, BMI, level of education, geographic region, tumor grade, tumor size, and number of comorbidities were some of the major predictors of health-related quality of life. These predictors were further explored using multinomial logistic regression analysis which identified number of comorbidities as the most significant predictor for HRQoL-PCS scores and level of education as the most significant predictor for HRQoL-MCS scores. This suggests that future research needs to be conducted, identifying the most common comorbidities in older breast cancer survivors to develop interventions that better the physical HRQoL in patients, in addition to the development of mental HRQoL interventions for patients that are less educated.
