Book excerpt: This book equips the intraoperative monitoring provider (both technologist and physician) with details of commonly monitored surgical procedures of the spine. This knowledge better allows the monitorist to identify critical phases of the surgery and correlate those with neuroanatomical and functional risk at each phase. Chapters are authored by a surgeon and neurophysiologist, with the surgeon contributing procedural content and the neurophysiologist contributing information on how the monitoring integrates within the context of the procedure. It is the first book that incorporates content about the surgical procedure directly with the intraoperative monitoring plan for the surgery. This text will have a wide audience throughout the medical specialty of intraoperative neurophysiological monitoring; it will be of interest to technologists, interpreting physicians or neurophysiologists, anesthesiologists, and surgeons; essentially anyone on the surgical team that lacks a detailed knowledge of both the surgical procedure and the neurophysiology.

Written by a leading authority in the field, Dr. Aage Moller has updated this important title to again offer all the leading-edge knowledge needed to perform electrophysiological recordings in the operating room, to interpret the results, and to present the results to the surgeon. The field known as "intraoperative monitoring" has expanded rapidly to cover other uses of neurophysiology and electrophysiologic recordings during surgical operations that affect the brain, spinal cord, and other parts of the nervous system. These new areas are covered in this new edition. To better represent the content of the book and the field as it now stands, many of the chapters have been revised and new material has been added. While the general organization of the book is maintained, chapters such as monitoring of motor systems have been revised and extended with new material, including more detailed description of the anatomy and physiology of motor systems and new information about intraoperative monitoring.

Book excerpt: This widely praised, first-of-its-kind book has been thoroughly updated, expanded, and enriched with extensive new case material, illustrations, and link-outs to multimedia, practice guidelines, and more. Written and edited by outstanding world experts, this was the first and remains the leading single-source volume on intraoperative neurophysiological monitoring (IOM). It is aimed at graduate students and trainees, as well as members of the operative team, including anesthesiologists, technologists, neurophysiologists, surgeons, and nurses. Now commonplace in procedures that place the nervous system at risk, such as orthopedics, neurosurgery, otologic surgery, vascular surgery, and others, effective IOM requires an unusually high degree of coordination among members of the operative team. The purpose of the book is to help students, trainees, and team members acquire a better understanding of one another’s roles and thereby to improve the quality of care and patient safety. From the reviews of the First Edition: “A welcome addition to reference works devoted to the expanding field of nervous system monitoring in the intraoperative period... will serve as a useful guide for many different health care professionals and particularly for anesthesiologists involved with this monitoring modality...An excellent reference...[and] a helpful guide both to the novice and to the developing expert in this field.” ‐‐Canadian Journal of Anesthesia “Impressive... [The book] is well written, indexed, and illustrated...The chapters are all extensively referenced. It is also very good value at the price....I would recommend this book to all residents and especially to all neuroanesthesiologists. It will make a worthwhile addition to their library.” ‐‐Journal of Neurosurgical Anesthesiology

Available in PDF, EPUB and Kindle. Book excerpt: Spinal disorders in very young children may be caused by a variety of conditions. The treatment of such conditions is often challenging due to the age of the patient and the progressive nature of the deformity. There also may be associated problems such as congenital anomalies, respiratory insufficiency, and neurological problems. Depending on the etiology of the deformity, these children are often cared for by multiple specialists including pediatricians, pediatric orthopaedists or orthopaedic spine surgeons, neurologists, pediatric surgeons, pediatric neurosurgeons, oncologists, and/or pulmonologists. Health professionals in all of the mentioned disciplines are involved in the management of these patients, which is why compiling a comprehensive textbook that is not limited to orthopedic specialists is essential. This textbook will effectively help to standardize the care of these patients. Furthermore, other professionals such as nurses, physical therapists and healthcare professionals in training are usually not familiar with these conditions and are in need of a reference book to consult when caring for children with spinal deformities.

Available in PDF, EPUB and Kindle. Book excerpt: In recent decades, the advances in and consolidation of Intraoperative Neurophysiological Monitoring (IOM) in several highly complex surgical areas have been undeniable. Currently all modalities of neurophysiological tests (SSEP, MEP, EMG, PEATC, VEP, BRAIN MAPPING, ETC) are performed in the operating room, where they are used to provide trans operative information on the patient’s neurological status in real time (monitoring), and to identify neural structures which are at immediate risk (mapping). With the inarguably positive impact of IOM on surgical outcomes, there is an increasing interest in reliable, technically focused literature. This volume provides cutting-edge information in the field of IOM, and highlights new neurophysiological tools being used in various surgeries. The book’s initial sections cover a range of topics, including an anatomical overview, electrical safety, and detailed technical descriptions of the neurophysiological tests used in IOM. The subsequent sections address e.g. the brain, brainstem, spinal cord, vascular and peripheral nerves, epilepsy, head and neck, movement disorders and special topics. Some chapters are accompanied by videos of surgeries and IOM so that the reader will have the real sensation of having been in the operating room and will gain an overview of the key steps. Written by experts in the field of IOM, the book offers a valuable resource for both experienced and early-career neurophysiologists, neurosurgeons, vascular and orthopedic surgeons, and surgeons involved with pelvic procedures. Further, its goal is to provide a real rapport, never before attained, between neurophysiologists and surgeons with a sole focus: excellence in terms of the final outcome.

Book excerpt: Intraoperative neurophysiologic monitoring has shown a steady increase in use for surgeries in which neural structures may be at risk of injury. Some of the surgical techniques used carry inherent risks, and these risks have changed the way in which neurophysiologic monitoring has impacted patient safety and quality of care during surgical procedures. It is therefore crucial that those performing and interpreting intraoperative neurophysiologic monitoring are adequately trained. This book is a comprehensive guide to the current practice of intraoperative neurophysiology with chapters on various modalities and clinical uses. Separate chapters devoted to anesthesia, operating room environment, special considerations in pediatrics and the interpretation and reporting of neurophysiologic data are useful and complementary. Questions and detailed answers on the topics covered can be found on the accompanying website for study review. This book will be useful to the trainee as well as the neurophysiologist already in practice.

Book excerpt: This handbook is the most authoritative and up-to-date reference on spine technology written for practitioners, researchers, and students in bioengineering and clinical medicine. It is the first resource to provide a road map of both the history of the field and its future by documenting the poor clinical outcomes and failed spinal implants that contributed to problematic patient outcomes, as well as the technologies that are currently leading the way towards positive clinical outcomes. The contributors are leading authorities in the fields of engineering and clinical medicine and represent academia, industry, and international government and regulatory agencies. The chapters are split into five sections, with the first addressing clinical issues such as anatomy, pathology, oncology, trauma, diagnosis, and imaging studies. The second section, on biomechanics, delves into fixation devices, the bone implant interface, total disc replacements, injury mechanics, and more. The last three sections, on technology, are divided into materials, commercialized products, and surgery. All appropriate chapters will be continually updated and available on the publisher’s website, in order to keep this important reference as up-to-date as possible in a fast-moving field.

Available in PDF, EPUB and Kindle. Book excerpt: The symposium from which this book originates represents a sig nificant watershed in the field of intraoperative neural monitoring, since the participants concluded that electrophysiologic monitoring techniques should be considered a "standard of care" for surgical pro cedures that place the central nervous system (CNS) at risk for injury. Specifically, it was agreed that the somatosensory-evoked potential (SEP) is a remarkably reliable and sensitive indicator of several aspects of CNS function, and should be routinely employed as an intraopera tive monitor during many neurosurgical and orthopedic procedures. The significance of this conclusion cannot be overstated, for at the time of this writing, intraoperative monitoring methods based on evoked-potential analyses are still considered experimental and are not in routine use. The reasons for this are not clear, given the accu mulation of literature and expertise on this subject over the past five years. Granted, the cost of electrophysiological monitoring equip ment is high, but only initially. The benefits of injury prevention far outweigh these costs, from both medical and economic viewpoints. It is our sincere hope and goal that the medical community be made aware of the value of intraoperative neural monitoring.
