Building upon two prior studies from the Institute of Medicine (the 1991 Institute of Medicine's report Disability in America and the 1997 report Enabling America), The Future of Disability in America examines both progress and concerns about continuing barriers that limit the independence, productivity, and participation in community life of people with disabilities. This book offers a comprehensive look at a wide range of issues, including the prevalence of disability across the lifespan; disability trends the role of assistive technology; barriers posed by health care and other facilities with inaccessible buildings, equipment, and information formats; the needs of young people moving from pediatric to adult health care and of adults experiencing premature aging and secondary health problems; selected issues in health care financing (e.g., risk adjusting payments to health plans, coverage of assistive technology); and the organizing and financing of disability-related research. The Future of Disability in America is an assessment of both principles and scientific evidence for disability policies and services. This book's recommendations propose steps to eliminate barriers and strengthen the evidence base for future public and private actions to reduce the impact of disability on individuals, families, and society.

Book excerpt: Living independently and participating in one's community are priorities for many people. In many regions across the United States, there are programs that support and enable people with disabilities and older adults to live where they choose and with whom they choose and to participate fully in their communities. Tremendous progress has been made. However, in many cases, the programs themselves â€" and access to them â€" vary not only between states but also within states. Many programs are small, and even when they prove to be successful they are still not scaled up to meet the needs of the many people who would benefit from them. The challenges can include insufficient workforce, insufficient funding, and lack of evidence demonstrating effectiveness or value. To get a better understanding of the policies needed to maximize independence and support community living and of the research needed to support implementation of those policies, the National Academies of Sciences, Engineering, and Medicine convened a public workshop in October 2015. Participants explored policies in place that promote independence and community living for older adults and people with physical disabilities, and identified policies and gaps in policies that can be barriers to independence and the research needed to support changing those policies. This report summarizes the presentations and discussions from the workshop.

Book excerpt: This report reviews federal laws and programs affecting persons with disabilities and assesses the extent to which they: provide incentives or disincentives to the establishment of community-based services for handicapped individuals; promote the full integration of such individuals in the community, in schools, and in the workplace; and contribute to the independence and dignity of such individuals. An introductory chapter provides a description of the demographics of the U.S. disabled population. A list of federal programs serving people with disabilities is prioritized according to federal expenditures, with an estimated number of persons with disabilities served. Summarized are the National Council on the Handicapped's legislative recommendations in 10 issue areas: equal opportunity laws; employment; disincentives to work under social security laws; prevention of disabilities; transportation; housing; community-based services for independent living; educating children with disabilities; personal assistance such as attendant services, readers, and interpreters; and coordination. The recommendations propose modifications in the expenditure of disability-related dollars to improve the ability of persons with disabilities to live with dignity and as independently as possible within their communities. To better understand how the increasing demand for supports and services will affect the nation's workforce, the National Academies of Sciences, Engineering, and Medicine convened a public workshop in June 2016, in Washington, DC. Participants aimed to identify how the health care workforce can be strengthened to support both community living and community participation for adults with disabilities and older adults. This publication summarizes the presentations and discussions from the workshop.

Book excerpt: A crucial contemporary dynamic around children and young people in the Global North is the multiple ways that have emerged to monitor their development, behaviour and character. In particular disabled children or children with unusual developmental patterns can find themselves surrounded by multiple practices through which they are examined. This rich book draws on a wide range of qualitative research to look at how disabled children have been cared for, treated and categorised. Narrative and longitudinal interviews with children and their families, along with stories and images they have produced and notes from observations of different spaces in their lives – medical consultation rooms, cafes and leisure centres, homes, classrooms and playgrounds amongst others – all make a contribution. Bringing this wealth of empirical data together with conceptual ideas from disability studies, sociology of the body, childhood studies, symbolic interactionism and feminist critical theory, the authors explore the multiple ways in which monitoring occurs within childhood disability and its social effects. Their discussion includes examining the dynamics of differentiation via medicine, social interaction, and embodiment and the multiple actors – including children and young people themselves – involved. The book also investigates the practices that differentiate children into different categories and what this means for notions of normality, integration, belonging and citizenship. Scrutinising the multiple forms of monitoring around disabled children and the consequences they generate for how we think about childhood and what is ‘normal’, this volume sits at the intersection of disability studies and childhood studies.
