Book excerpt: Much research in autism spectrum disorders (ASD) has focused on the development of efficacious interventions to address the core deficits of ASD. However, the heterogeneous nature of ASD complicates the development of such interventions. With great heterogeneity in the expression of ASD's core deficits, it is unlikely that there is a one size fits all intervention. It is important for researchers to understand for whom an intervention works. Advancements in data analytics, in particular machine learning, provide new methods to identify subgroups among a given population, and can potentially help to identify for whom intervention works best. Of particular interest are minimally verbal individuals. A targeted social communication intervention known as JASPER (Joint Attention, Symbolic Play, Engagement, and Regulation) has shown promise for improving language outcomes among minimally verbal children with ASD and may provide the context to examine the question of for whom an intervention benefits. This study aims to develop a model predicting expressive language gains among minimally verbal, preschool aged children with ASD that received a targeted social communication intervention. Classification and regression tree (CART) analysis was used to explore the relationship between child characteristics and gains in expressive language. Secondary data analysis was conducted on a sample of 99 minimally verbal, preschool age children with ASD, collected from participants across five previous intervention studies. Expressive language gains (outcome) were calculated using expressive language age equivalents from the Mullen Scales for Early Learning. Predictors for the analyses were taken from child demographics and behavioral assessments completed prior to intervention. The initial list of predictors included race, gender, ASD severity, visual reception age equivalent, fine motor age equivalent, joint attention gestures, requesting gestures, and play skills. Using expressive language age equivalent change scores, 47% (n = 47) of the sample were identified as "super responders," children that exceeded expressive language gains typically expected through maturation. To predict responder status, all initial predictors were used to generate conditional inference forest, from which the most important variables would be chosen for the final model. Conditional inference results identified three variables to be fitted into the final model; play diversity, requesting gestures, and fine motor age equivalent. A final conditional inference tree was created, with play diversity being the only significant predictor of responder status. Participants with an entry play diversity score above 23 predicted super response while scores of 23 or below predicted slow response. The overall model accuracy was 67%, with a specificity of 55% and sensitivity of 78%. As a comparison, stepwise logistic regression was run, and play diversity was again the only significant predictor of responder status ( 2 (1) = 10.686, p = .001). Receiver operating characteristic curves were generated to compare model performance, and comparison of area under the curves for the two models showed no statistical difference (p = .82). Overall accuracy of the conditional inference tree was moderate, and performed similarly to the more traditional logistic regression analysis. However, the conditional inference tree provides a cutoff point that may provide clinical utility over the regression results. Both models identify play diversity as in important predictor of expressive language gains from JASPER, which is a play based social communication intervention. Additionally, our model appears to be more sensitive to identifying slow responders. The role of play diversity and expressive language gains in JASPER is discussed.

Book excerpt: Autism Spectrum Disorder (ASD) is a developmental disorder that can affect thinking, feeling, language, and social skills in a variety of ways, and can emerge in behaviors that change from one individual to another (NIMH, 2016). While speech pathologists have used traditional approaches in order to develop communication skills in individuals with ASD, there has been little research performed combining techniques used from other professions and integrating those techniques into a speech therapy session. A case study was conducted on a school-age child diagnosed with autism to determine if intervention approaches used by other professionals in combination with speech therapy would improve language and pragmatic skills. Over the course of 9 weeks, a single subject ABAB treatment approach was used interchanging a traditional speech therapy approach and a hybrid intervention approach that contained techniques from the backgrounds of both Applied Behavior Analysis and Music Therapy. The Clinical Evaluation of Language Fundamentals - 5th Edition (CELF-V) and criterion referenced assessments were used to collect baseline and post intervention data in the areas of expressive and receptive language as well as pragmatics. Treatment revolved around three treatment goals, which targeted correctly producing present and past tense verbs, identifying objects and their function, and appropriately requesting social interactions from others. The results of the CELF-V exhibited improvement or no growth in five of the six subtests assessed, while the criterion-referenced assessments displayed improvement in the participant's social skills and a decline in the participant's expressive language. Raters were used to assess the participant's pragmatic skills using the Pragmatic Activities Checklist and Pragmatics Profiles subtests. Cohen's Kappa was calculated to compare the reliability of the raters, and revealed that the raters were inconsistent in their scoring of the participant's pragmatic abilities. The results of the study concluded that while there were small gains made in the participant's expressive language and pragmatics from the standardized and criterion-referenced tests, the length of the study was too short to determine if the hybrid intervention approach used in the study could make large gains in the participant's communication skills. The participant's three goals targeted in the study, as well as the parent observations during the study, exhibit positive outcomes in terms of the effectiveness of the study for the participant.

Book excerpt: Up to 40% of later-born siblings of children with autism spectrum disorder (ASD) are diagnosed with: a) ASD (7-19%), which is defined by social communication impairments; or b) other language and/or cognitive delays (14-20%) by age three. Vocal complexity, a measure of the quality and maturity of vocal communication, seems to be a strong predictor of later expressive language in typically developing (TD) and children with ASD, but no studies have examined this relation for these toddlers at high familial risk for ASD (high risk; HR toddlers). Also, previous studies have not examined the role of vocal complexity occurring within preverbal (non-word) vocalizations, distinct from that occurring within words. Low preverbal vocal complexity may signal early language difficulties and need for intervention in HR toddlers. Vocal complexity may be malleable with intervention. This study examined vocally complex communication within an ongoing longitudinal randomized controlled trial (RCT) of the ImPACT intervention (compared to business as usual; BAU) in 54 12-27-month-old HR toddlers. It found that: 1) nonword vocal complexity was a value-added predictor of later expressive language ability in HR toddlers; 2) families in the ImPACT condition did not display greater parent verbal responsiveness (PVR) to toddlers' vocally complex communication acts, greater toddler vocal complexity, or greater toddler expressive language outcome compared to families in the BAU condition; and 3) for toddlers in both treatment conditions, there was an indirect effect of parents' verbal responsiveness on toddlers' later expressive language ability through toddlers' midpoint vocal complexity. This study provides insight into the role of vocal complexity as a risk indicator of language delay and discusses the mechanisms by which vocal complexity may or may not be malleable in response to a parent-implemented social communication intervention.

Book excerpt: Minimally verbal children are considered the enigmatic and, unfortunately, the neglected end of the autism spectrum. This subpopulation has likely garnered this title due to their exclusion from research studies, which has inevitably affected their evidence base. The paucity of proper measurement tools that sensitively and accurately assess behaviors has been one limiting factor towards the improved knowledge of these children. Short of creating and validating a new measurement tool for this subpopulation, this study took an alternative and more immediate approach: examine an existing social communication measure (ESCS) with repurposed quantitative methodologies, item response theory (IRT) and machine learning algorithms (CART and random forests). The final sample consisted of 453 minimally verbal children culled from four different intervention studies. The IRT models analyzed the frequency of social communication gestures from the ESCS and returned an objective difficulty hierarchy regarding initiations of joint attention and behavior regulation gestures. The best-fitting and final model was a zero-inflated negative binomial model (ZINBM) which determined that joint attention gestures were, on average, more difficult than behavior regulation gestures. Joint attentional shows and gives were especially tough, and behavior regulation reaches were the easiest gestures for this sample. The ZINBM separately modeled children with some gestures and children who did not present with any gestures and determined that behavior regulation reaches and gives were likely the first gestures a child will eventually exhibit among children with no gestures. Classification and regression trees (CART) were used to understand the clinical meaning behind frequencies of social communication gestures. Influential cut points were identified by the recursive partitioning algorithm of CART and determined which frequencies were able to classify children into more or less robust language outcomes at baseline. On average, a single behavior regulation point was sufficient to classify children into more robust language outcomes. For many of the trees, responding to bids joint attention around one-third of the time or more was also predictive of more robust language outcomes. Variable importance was examined with a random forest algorithm, which matched the results from the classification trees. Overall, this study demonstrated that the use of IRT and CART yielded additional information, beyond traditional scoring and analytic techniques, regarding the presentation of social communication gestures among minimally verbal children. This study also discussed the methodological contributions and potential future applications of IRT and CART within this field.

Often, SLPs, especially those who work in state-funded Early Intervention Programs (Part C-Early Intervention, IDEA, 2004), do not have access to training on autism-specific early intervention models (Stahmer, Collings, & Palinkas, 2005). In addition, children who are receiving Early Intervention in community settings often do not have available options for autism-specific interventions, which are offered in many university settings. As a result, the facilitation strategies of focused stimulation, vertical structuring, and expansions are often being implemented in therapy with these children. To our knowledge, no study has measured the effects of these commonly used language strategies on the verbal language in minimally verbal children with ASD. The purpose of this study was to measure the effects of focused stimulation, vertical structuring, and expansions on the acquisition of verbal language in minimally verbal children with ASD. Three toddlers diagnosed with autism were recruited for this study. Intervention took place twice a week for 20-minute sessions, followed by two-generalization probes 4 weeks post intervention. Two out of our three participants demonstrated gains in their verbal language inside of the intervention environment. Results of this study conclude that these intervention strategies are capable of being implemented in community-based intervention settings with successful outcomes.

Book excerpt: This study comared the effectiveness of two different treatment protocols, discrete trial training (DTT) and a naturalistic developmental behavioral intervention (NDBI), on spontaneous use of trained words in four 2-to-3-year-old children with autism. All participants used more words trained using a NDBI than those trained using DTT. Secondary data were collected on the effects of a multifaceted treatment combining DTT with a NDBI on receptive, expressive, and pragmatic language outcomes and joint attention skills. Outcomes were measured using standarized tests, the Receptive/Expressive Emergent Language Test-Third Edition, and the Languge Use Inventory. Additional non-standardized measures collected were total number of utterances produced, number of trained utterances produced, and pragmatic use categories appearing in language samples during generalization probes. Participants made gains in total number of utterances produced and ability to respond to bids for joint attention. Two profiles of overall improvement emerged, with two participants improving their performance by at least 40% across the majority of measures, and two participants making smaller gains across a more limited number of measures. These differences in response to treatment suggest several areas for future study.

The research purpose of this experiment was to implement (Faded) ALS as a faded prompt to examine the effects on expressive vocabulary productions for minimally verbal children with ASD. This investigation was an ABCD single-subject design evaluated across a single participant, which evaluated the impact of this intervention approach on expressive vocabulary as measured through total number and the varied number of both AAC and verbal vocabulary productions. Current literature demonstrates that ALS has had secondary positive effects on verbal productions. This study aimed to specfically examine the direct impacts on verbal productions, to take the next logical step to expand upon research in the speech-language pathology field. This investigation aims to build on the currently available literature to determine how fading ALS would effect expressive vocabulary production. This experiment included a single participant. Results revealed that this participant exhibited a significant increase and maintenance in both the varied and total number of AAC and verbally-produced vocabulary following the intervention. Faded aided language stimulation appeared to be linked with an increase and maintenance of expressive vocabulary for a minimally-verbal child with ASD.

Book excerpt: Early Expressive language exposure is associated with child language acquisition and advantageous long-term developmental outcomes. The measurement of expressive language in a child's immediate environment is critical to the early identification of children who are at risk of low expressive language exposure, such as children with language delays or disabilities including Autism Spectrum Disorder (ASD). A team of researchers, educators, and engineers developed Language ENvironmental AnalysisTM (LENA), a computerized language processing software, to ease the language sampling process. Recently, language acquisition researchers have proposed that LENA technology has greater application in clinical practice to collect naturalistic language acquisition data to inform educational services. Therefore, the present dissertation investigated the use of LENA technology in applied research, uses LENA technology to take naturalistic repeated measures in a clinical setting, and makes suggestions for clinical research (see Chapter 2, 3, and 4). Chapter 2 was a systematic literature review of the use of LENA technology in applied research to identify information on study methodology, LENA technology implementation, and the use of novel variables derived from LENA technology in applied research. The results showed that researchers are increasingly using LENA technology with more diverse populations and using more diverse LENA measures to measure the language environment. Specifically, the number of different languages, ethnicities, and abilities have increased in research using LENA technology to evaluate language environments. However future research needs to include better descriptions of procedural information, which is critical for an emerging body of research.Chapter 3 depicted the expressive language trajectories of preschool-aged children with ASD who are receiving Early Intensive Behavioral Intervention (EIBI). Overall, the present investigation demonstrates that minimally verbal pre-school aged children with ASD demonstrate growth in expressive language trajectory in EIBI and that EIBI may be equally beneficial, in terms of expressive language growth, for children of all expressive language levels. The authors observed a waxing and waning of child language growth over the course of a year, consistent with prior research that highlights the importance of measuring individual child expressive language trajectories in research.Finally, Chapter 4 introduced LENA technology as a behavioral observation measure to behavior analytic practitioners to improve the language environment of young children with ASD. Discussions of what LENA programs are available for behavior analytic use with young children with ASD, how practitioners can use LENA technology in behavior analytic services, and considerations for practical use of LENA in behavior analytic services. The authors provided examples throughout the paper to illustrate the use of LENA technology in behavior analytic practice to improve the language environments of young children with ASD.

Children with autism are challenged by the most essential human behaviors. They have difficulty interacting with other people-often failing to see people as people rather than simply objects in their environment.
