Nous avons donc voulu savoir s'il existait des déterminants sur lesquels agir pour améliorer le suivi par le médecin traitant après le passage aux urgences. Objectif : identifier les principaux déterminants de la consultation des patients auprès de leur médecin traitant précocement après leur sortie des urgences. Méthode : étude observationnelle prospective réalisée en mai 2017 à Saint Palais sur les patients considérés sortants après leur passage aux urgences. Résultats : 102 patients ont pu être inclus après leur passage aux urgences. 74,5% d'entre eux étaient venus d'eux-mêmes aux urgences pour des motifs principalement traumatologiques (57%). 30% avaient vu leur médecin traitant dans les 7 à 10 jours suivant leur passage aux urgences. Les motifs les plus cités de cette consultation étaient de ne pas être soulagé (36%) et d'informer leur médecin (28%). Une consultation par le médecin traitant (MT) aurait souvent pu éviter nombre de passages. Les résultats publiés sont en faveur d’une rupture du réseau de soins entre les urgences et le MT. Nous avons étudié les facteurs qui influencent la consultation des patients auprès du MT après leur passage au SAU du Centre Hospitalier Intercommunal de Villeneuve-Saint-Georges (CHIV). Le territoire de cet hôpital présente de grands écarts socio-démographiques. Il s’agit d’une enquête quantitative par recueil de données du dossier médical et questionnaire hétéro-administré. La population étudiée était composée de patients ayant consulté aux urgences adultes du CHIV entre novembre et décembre 2019. Cent deux dossiers patients ont finalement été étudiés. Le taux de la consultation de suivi chez les MT était de 43,1% à un mois. Les facteurs qui favorisaient la consultation de suivi étaient l’âge (60 à 74 ans), le sexe féminin et les patients adressés par le 15 et 18. Les facteurs défavorables sont les catégories des ouvriers ou sans-emploi ; auto-consultation. L’évolution d’un symptôme douloureux avait une place importante dans la consultation de suivi. Les facteurs qui motivent les patients du bassin du CHIV à retourner voir leur MT ne sont pas exactement les mêmes que ceux développés dans les autres études. Le facteur inattendu qui semble émerger est l’évolution des symptômes qui ont déclenché la consultation initiale.

Book excerpt: Le parcours patient hôpital-ville après un passage au service d'accueil des urgences est un parcours simple et souvent bref. Il peut néanmoins être le début d'une prise en charge plus longue et complexe, où le médecin traitant doit pouvoir reprendre le fil du suivi de manière efficace et coordonnée. Il est intéressant d'en comprendre l'utilisation par les usagers pour améliorer l'efficience des soins coordonnés. L'objectif principal de cette étude est d'analyser le taux de consultations chez le médecin traitant lorsqu'une réévaluation est indiquée par l'urgentiste. Les objectifs secondaires sont de chercher les facteurs influençant cette réévaluation et d'en déduire des pistes possibles d'amélioration. Nous avons mis en place une étude épidémiologique observationnelle descriptive prospective et monocentrique du 21/02/2022 au 01/04/2022 au service d'accueil des urgences de La Ciotat (13). Nous avons contacté ces patients 15 jours après leur sortie pour un bref questionnaire téléphonique. Le critère de jugement principal était le « devenir-patient » (consultation réalisée par le médecin traitant dans les 15 jours). Sur les 404 patients rappelés, 178 ont pu être joints et inclus dans l'analyse. Le taux de consultation brut à 15 jours est de 65%, celui chez le médecin traitant de 47% (soit 72% des consultants). La moitié des patients qui ne re consultent pas disent « aller mieux ». Une part non négligeable de patients attend encore un rendez-vous 15 jours après la sortie des urgences (imagerie ou médecin). Les facteurs positifs significatifs de réévaluation sont : l'âge et l'impression d'un compte rendu de passage aux urgences (+20% de réévaluation). Les patients respectent globalement les consignes de surveillance données lors de la sortie mais n'appréhendent pas complètement le fonctionnement du parcours patient coordonné. La valeur ajoutée du compte rendu de sortie du patient est probablement sous-estimée et son impression devrait être plus systématique. L'optimisation informatique doit permettre une amélioration de la production et de la transmission d'information. Les réseaux de santé locaux doivent améliorer la connaissance du parcours de leurs patients et des patients de leur parcours.
