Book excerpt: Introduction : à l'hôpital, l'équipe mobile de gériatrie (EMG) a pour principale mission d'aider les différents services dans la prise en charge des personnes âgées qui sont des patients complexes car fragiles, polypathologiques et polymediqués. L'EMG réalise des évaluations gériatriques standardisées conduisant à l'émission de recommandations gérontologiques et/ou de conseils thérapeutiques. Ces interventions ont pu être associées à une réduction de la durée de séjour hospitalier des patients. La réhospitalisation est un indicateur important de la qualité des soins de tout service hospitalier. Notre objectif principal est de déterminer s'il existe une association entre la recommandation d'un avis thérapeutique émis par l'EMG et la réhospitalisation. Méthodes : nous avons mené une étude quantitative monocentrique, observationnelle descriptive et analytique, longitudinale, avec saisie rétrospective des données. Nous avons évalué le taux de réhospitalisation selon l'émission ou non d'un avis thérapeutique et décrit les motifs de la première réhospitalisation et le délai avant cette première réhospitalisation sur une durée de 3 ans. Résultats : 825 interventions ont été réalisées, 656 ont pu être analysées dont 482 comportaient un avis thérapeutique. Une réhospitalisation est survenue dans 70,6% des cas, sans différence significative entre les deux groupes. De même, on ne mettait pas en évidence de différence de survie entre les deux groupes. Conclusions : l'émission d'un avis thérapeutique n'était pas associée à une diminution du taux de réhospitalisation et du taux de mortalité.

Book excerpt: Les équipes mobiles de gériatrie extra hospitalières ont été créées afin de développer le lien ville-hôpital. L'unité Mobile Extra Hospitalière de Gériatrie du centre hospitalier Lyon Sud (ou UMEHG-LS) a débuté son activité en janvier 2014. Elle propose une expertise gériatrique pluridisciplinaire au domicile des patients. Elle émet des recommandations afin de rationaliser le parcours de la personne âgée en fonction de ses problématiques. Peu d'études décrivent la prise en charge de ces dispositifs extra hospitaliers existant depuis peu d'années. L'objectif de notre étude était de déterminer le taux de suivi à 2 mois de ces recommandations ainsi que de décrire le profil des patients suivis. Nous avons mené une étude descriptive, monocentrique et rétrospective sur 500 patients pendant une période de 2 ans. Le taux de suivi global des recommandations émises par l'UMEHG-LS était de 65,1%, le meilleur taux de suivi concernait les recommandations d'orientation et les moins suivies concernaient les recommandations "l'environnement". L'âge moyen des patients était de 83.5 ans. La population était composée à 61% de femme. Les patients étudiés prenaient en moyenne 5 traitements (hors psychotropes). La majorité des patients était GIR 3, avec ADL moyen à 3 et IADL à 2. Sur l'ensemble des patients, 96% avait un aidant identifié. Il a été retrouvé une différence statistiquement significative avec moins d'hospitalisations non programmées lorsque les recommandations sont suivies à plus de 65%. Le grand effectif de cette étude permet de mieux cerner la population vue par l'UMEHGLS. Elle confirme une population vulnérable présentant une polypathologie, dépendance, des troubles cognitifs associés à un contexte socio-familial instable. Le rôle préventif de l'UMEHG-LS pour éviter les hospitalisations en urgence et les ruptures de parcours paraît essentiel dans cette population fragile. Ce travail montre aussi l'insuffisance des seules recommandations : elles ont besoins d'être directement étayées pour être appliquées.

Book excerpt: Introduction : Avec le vieillissement de la population, les pays développés sont de plus en plus confrontés aux réadmissions hospitalières qui sont problématiques pour des raisons médicales et économiques. La prévention de cette complication est primordiale dans le parcours de soin du patient quand on sait les risques de complications nosocomiales, de perte d'autonomie et de décès. Les réadmissions précoces sont fréquentes et concernent entre 5 et 35% des patients. Cependant, les études sur l'incidence des hospitalisations à trente jours des patients âgés rentrés à leur domicile après un passage aux urgences restent rares. Objectif : L'objectif principal de ce travail est d'évaluer l'incidence des hospitalisations précoces dans les 30 jours, des patients âgés, fragiles sortis des urgences et ayant bénéficiés du dispositif de soins à domicile par l'intermédiaire d'une IPA. Les facteurs associés à l'hospitalisation étaient secondairement étudiés. Méthode : Il s'agit d'une étude prospective, monocentrique ayant inclus sur une période d'un an, des patients âgés de plus de 75 ans sortis des urgences, stabilisés sur le plan médical et évalués par une IPA à domicile dans les 24-48H de leur sortie. Les patients étaient suivis pendant un mois à l'aide d'un questionnaire de suivi permettant une analyse des données médicales, des intervenants au domicile, de la consommation de soins et de la qualité de vie liée à la santé. Résultats : Au total, une hospitalisation précoce s'est produite pour 22 patients sur 143 patients âgés sortis des urgences, soit une proportion de 15,7%. Les données sociodémographiques n'étaient pas associées à un risque d'hospitalisation. Les facteurs de risque mis en évidence en analyse bivariée étaient : les troubles cognitifs préexistants, le score de comorbidité de Charlson, les chutes à répétitions, le nombre de traitement sur l'ordonnance. Conclusion : Notre étude de cohorte confirme l'importance de la notion de réadmission hospitalière précoce des patients âgés après un passage aux urgences. Cette donnée fait partie des critères de qualité de prise en charge de ce sous-groupe de patient. Malgré des limites, nos résultats mettent en avant un profil de patient âgé gériatrique à risque de réadmission. Ainsi, la mise en œuvre d'actions ciblées de repérage et de prévention pourrait améliorer la qualité de soins de ces patients.

Nous avons mené une étude quantitative monocentrique observationnelle descriptive et analytique, avec saisie rétrospective des données. La fréquence et le détail des avis thérapeutiques émis en 2011-2012 par l'EMG du CHU d'Angers, France, ont été décrits. Une durée d'hospitalisation courte a été définie par le plus petit quintile, soit d"jours. 825 interventions ont été réalisées, dont 791 ont pu être analysées, parmi lesquelles 580 (73%) comprenaient un avis thérapeutique. Les facteurs associés à l'émission d'un avis thérapeutique reflétaient la fragilité : âge élevé, polymédication, polymorbidité, chutes, troubles cognitifs. Ces avis consistaient majoritairement (57,8%) en une combinaison d'ajout et de retrait de médicaments. Les avis concernaient les psychotropes (48,3%), puis les morphiniques, les laxatifs, l'hydratation et les médicaments à visée cardiovasculaire. L'émission d'un avis thérapeutique était positivement associée à une durée d'hospitalisation courte (OR=1,53 ; p=0,049), en particulier lorsqu'il concernait les inhibiteurs du système rénine-angiotensine-aldostérone et les inhibiteurs calciques. La prévention de la chute dans cette population constitue un enjeu actuel de santé publique ainsi qu'une des missions des EMG. Actuellement, il existe peu d'études concernant l'évaluation des interventions de ces unités. Or, leur évaluation est complexe mais indispensable pour adapter les pratiques futures face au vieillissement croissant. Nous nous sommes intéressés au travail de l'EMG du centre hospitalier (CH) de Saint Philibert (SP) qui offre un regard gériatrique particulier par relecture du dossier médical. Ainsi, l'EMG-SP propose des recommandations chez les patients de 75 ans et plus, admis aux urgences pour chute. Notre objectif était d'évaluer la participation des patients aux propositions de l'EMG-SP. Méthode : nous avons réalisé une étude observationnelle, prospective et nous avons décrit une cohorte de 100 patients chuteurs de février 2016 à janvier 2017. Quatre mois après la chute, nous avons évalué la participation des patients aux recommandations de l'EMG en contactant par téléphone les médecins traitants ainsi que par recoupement des données avec le dossier informatique. Parallèlement, nous avons interrogé ces médecins généralistes afin d'évaluer la qualité du lien ville-hôpital. Résultats : nous avons observé une participation aux recommandations de l'EMG-SP chez 64 % des patients. Au total, 115/153 recommandations proposées étaient suivies (75,2 %). Ces recommandations concernaient fréquemment la prise en charge médicale: 36 consultations de gériatrie (taux de suivi 44 %), 16 hospitalisations de jour (taux de suivi 56 %) et 24 modifications thérapeutiques (taux de suivi 88%). Enfin, la majorité des médecins généralistes (63 %) a apprécié la prise en charge proposée par l'EMG-SP. Conclusion : le suivi global des patients aux recommandations de l'EMG-SP est satisfaisant. De plus, notre travail met en lumière un partenariat réussi entre les médecins traitants et l'EMG. Toutefois, il semble nécessaire de soutenir et de renforcer ce lien ville-hôpital. Des travaux futurs pourront être menés en s'appuyant sur ce partenariat dans le but continu d'allier qualité de vie et vieillissement.
