Book excerpt: This monograph contains 20 selected papers presented at the Symposium on Subcellular Basis of Contractile Failure which was held in Ottawa during May 11-13, 1989 and is designed for the benefit ofthose who were unable to attend this event. It is now increasingly becoming clear that an excessive amount of calcium is intimately involved in the pathogenesis of a wide variety of heart diseases. Accordingly, the investigations concerning the role of calcium chan nels and their regulatory mechanisms in heart function as well as of the intra cellular calcium overload in cardiac dysfunction are presented here. Since sodium is also considered to influence the cardiac contractile force by chang 2 ing the intracellular concentration of calcium through the Na +-Ca+ exchange 2 mechanism in the cell membrane, the role of Na +-Ca+ exchange in heart func tion as well as pathology of contractile failure is discussed. In view of the new ly discovered implications of the oxygen free radicals in cellular injury, papers concerning the role of these radicals in heart disease are included in this book. For the purpose of clarity, different chapters have been organized under three main headings: (I) Role of cations in heart function, (II) Cardiac hypertrophy and cardiomyopathies, and (III) Ischemic heart disease and cardiac failure.

Although their pathology is not much different from that of the overt group of diseases, the clinical approach to their diagnoses remains a puzzle. This has mainly emerged as a result of the problems faced in trying to pick up the vulnerable subjects from among the general public. More definitive candidates to be evaluated are those belonging to one of the high risk groups. There are no statistical figures available on the incidence of occult atherosclerotic disease in the "non risky" general public and this is definitely an area that needs further investigation. Atherosclerotic disease whether they are in the cerebral, coronary, renal or lower limb arteries are theoretically interrelated because the basic patho logical changes are usually similar. However there still remain some differ ences which are mainly due to variation in the flow pattern in the arteries. Thus it will be important clinically and from the management point of view to investigate for the presence of occult disease in other arteries if an atherosclerotic disease in a certain artery has been discovered. This approach is of a great significance especially from the preventive point of view in order to avoid catastrophic events which may have resulted from pathology in the other region.

Pulsed Doppler, continuous wave Doppler, and more recently, color flow mapping techniques have been used to study the pathophysiology, and to make quantitative measurements thereof. To those already working in pediatric echocardiography, this book is intended to serve as a guide to the applications of the various Doppler modalities in specific abnormalities. The other group for which this text is intended are the adult cardiac sonographers who see the occasional patient with congenital heart disease in their echocardiography laboratories. The incidence ofcongenital heart disease is not decreasing. As more patients survive to child bearing age, the adult cardiologist sees more and more referrals from the pediatric cardiologist to follow up the parent with corrected congenital heart disease, and in many smaller institutions, to examine their offspring as well.
