Our data suggest that breast cancer cells secrete various levels of VEGF and express in addition to the Flt-1 and Flk-1/KDR receptors, a novel VEGF receptor type which needs to be identified and characterized. These data lead us to hypothesize that VEGF, secreted by breast cancer cells, is a multi-functional protein which acts in an autocrine fashion and can simultaneously activate specific VEGF receptor signaling pathways in breast cancer cells, thereby regulating breast cancer cell growth, tumor angiogenesis and subsequently tumor growth. In order to test this hypothesis, we propose to focus on two basic aims: (a) To identify and characterize the VEGF receptors expressed in breast cancer cell, and to analyze their expression in primary breast tissues. We will elucidate the signaling events upon VEGF stimulation in breast cancer cells and identify which activated signaling molecules are essential for the VEGF-mediated efforts on breast cancer cell growth; and (b) To study the effects of VEGF and VEGF receptor expression on the regulation of tumor angiogenesis and signal transduction pathways in breast cancer cells.

Consequently numerous VEGF inhibitors are being studied. Some such as bevacizumab extend progression-free survival but have not yet been shown to extend overall survival in breast cancer patients. We are pursuing a totally different approach to targeting VEGF: rather than inhibit VEGF our goal is to convert VEGF to act as a cell death factor. Toward this aim we created a chimeric receptor (R2Fas) composed of domains from VEGF receptor 2 fused to the intracellular domain of the Fas cell death receptor. As controls we generated two inactive versions of R2Fas with mutations in the VEGF receptor 2 domain or the Fas domain. Replication-defective adenoviruses expressing each of the receptors were generated and used to direct expression of the receptors in human breast cancer cell lines. We found that expression of the R2Fas chimeric receptor but not the inactive control receptors could induce apoptosis in several human breast cancer cell lines in vitro. These studies suggest that a receptor such as R2Fas which converts VEGF to act as a cell death factor could yield a new and more aggressive approach to targeting overexpressed VEGF in breast cancer.

Book excerpt: The mortality of breast cancer is associated with the consequences of metastases most commonly to bones. Tumor development and metastasis depends not only on the intrinsic properties of the tumor cells, but also on the microenvironment. Key parameters within the microenvironment include the extra-cellular matrix and the levels of oxygen present. Both collagen-coating and hypoxia have been found to significantly modulate both cancer cell growth and apoptosis. Furthermore, in vivo, blocking of the endothelin (ET) pathway, including ET-1, a pleiotropic molecule with angiogenic, anti-apoptotic, and mitogenic properties, was associated with limited tumor growth and metastasis. Similarly, blocking the vascular endothelial growth factor (VEGF) pathway also affected tumor growth, mostly through the abrogation of angiogenesis. It is unclear however how these two pathways within tumor cells are affected by the presence of exogenous ET-1 or VEGF. Using in vitro 2D culture conditions, the effects of ET-1 and VEGF on cobalt chloride (CoCI[subscript]2) chemically-generated hypoxia cultures, in both MCF7 and MDA-MB-231 human breast cancer cells were investigated. The potential role of the extra-cellular matrix is addressed using non-coated or collagen-coated vessels. The results indicate that in response to hypoxia, MCF7 breast cancer cells had overall reduced expression of ET-1 and VEGF pathway genes. Additionally, the metastatic MDA-MB-231 cell line had increased expression of the genes associated with the ET pathways including ET-1 and endothelin receptor A (ET[subscript]AR), regardless of the coating conditions. The hypoxic conditions led to drastic decreases in cell proliferation of both MCF-7 and MDA-MB-231 cells in positively-charged vessels but to a much lesser extent in collagen-coated vessels. Also, the hypoxic conditions were associated with reduced apoptosis in both breast cancer cell lines tested but especially in MDA-MB-231 cells. This study provides new insights into the complex interactions between breast cancer cells and both their physical and chemical microenvironment.

Vascular endothelial growth factor (VEGF) is secreted by breast cancer cells(2) to stimulate angiogenesis through the over-expression of its receptors (VEGFR1 & 2) and proliferation of the vascular endothelial cells (VEC) of the surrounding tissue. In contrast, normal VECs express low level of VEGFRs. Strategies for targeting VEGFR to inhibit growth of breast cancer(3-5) have been tested, but had limited success. On binding to its receptors, VEGF is internalized via the receptor mediated endocytosis pathway way to the endosomal compartment of the endothelial cells. Our preliminary data using fluorescein-conjugated VEGF(165) showed that it was internalized into cytoplasmic vesicles inside human umbilical vein endothelial cells (HUVEC) (left panel A) and translocated to near the nucleus which formed a ring of fluorescence surrounding the nucleus (left panel B). Therefore, the aim of the present project is to target the overexpressed VEGFRs on the endothelial cells of the breast tumor, to deliver lethal levels of Auger electron emitting radionuclide Indium-(111) (In111) into breast cancer associated VECs which could be an effective radiotherapy for breast cancer.

Perhaps the most studied angiogenic process occurs in solid tumors whose growing mass and expanding cells create a constant demand for additional supply of oxygen and nutrients for survival. However, other physiological and clinical conditions, such as wound healing, ischemic events, autoimmune and age-related diseases also involve angiogenesis. Angiogenesis is a well-structured process that begins when oxygen and nutrients are depleted, leading to the release of chemokines and growth factors that attract immune cells, particularly macrophages and endothelial cells to the site. Macrophages that are recruited to the site, as well as tissue cells and endothelial cells, secrete pro-angiogenic mediators that affect endothelial cells and promote angiogenesis. These mediators include growth factors such as vascular endothelial cell growth factor (VEGF), matrix metalloproteinases (MMPs), as well as low levels of mediators that are usually seen as pro-inflammatory but are pro-angiogenic when secreted in low levels (e.g. nitric oxide (NO) and TNFa). Thus, macrophages play a major role in angiogenesis. Macrophages exhibit high plasticity and are capable of shifting between different activation modes and functions according to their changing microenvironment. Small differences in the composition of activating factors (e.g. TLR ligands such as LPS, anti-inflammatory cytokines, ECM molecules) in the microenvironment may differently activate macrophages to yield classically activated macrophages (or M1 macrophages) that can kill pathogen and tumor cells, alternatively activated macrophages (or M2 macrophages) that secrete antiinflammatory cytokines, resolution macrophages (rM?) that are involved in the resolution of inflammation, or regulatory macrophages (e.g. Myeloid-Derived Suppressor Cells - MDSCs) that control the function of other immune cells. In fact, macrophages may be activated in a spectrum of subsets that may differently contribute to angiogenesis, and in particular non-classically activated macrophages such as tumor-associated macrophages (TAMs) and Tie2-expressing monocytes (TEMs) can secrete high amounts of pro-angiogenic factors (e.g. VEGF, MMPs) or low levels of pro-inflammatory mediators (e.g. NO or TNFa) resulting in pro-angiogenic effects. Although the importance of macrophages as major contributors and regulators of the angiogenic process is well documented, less is known about the interactions between macrophages and other cell types (e.g. tumor cells, normal epithelial cells, endothelial cells) that regulate angiogenesis. We still have only limited understanding which proteins or complexes mediate these interactions and whether they require cell-cell contact (e.g. through integrins) or soluble factors (e.g. the EGF-CSF-1 loop), which signaling pathways are triggered in each of the two corresponding cell types, and how this leads to secretion of pro- or antiangiogenic factors in the microenvironment. The regulation of such interactions and through them of angiogenesis, whether through post-translational modifications of proteins or via the involvement of microRNA, is still unclear. The goal of this Research Topic is to highlight these interactions and their regulation in the context of both physiological and pathological conditions.
