Book excerpt: L'objectif de cette thèse de doctorat consiste à étudier les effets du stress maternel prénatal sur le développement de l'enfant en début de vie, plus particulièrement sur le poids de naissance, l'âge gestationnel et l'émergence des caractéristiques émotionnelles à l'âge de trois mois. Dans un premier temps, une méta-analyse a été effectuée portant sur les effets du stress maternel en période prénatale sur l'état de naissance de l'enfant. Cinquante et une études prospectives ont été retenues dans la présente méta-analyse. Les résultats suggèrent que le stress maternel prénatal est significativement associé à l'état de naissance (poids de naissance et âge gestationnel), mais la taille d'effet est très faible {d = -.02). Cette association semble être plus importante lorsqu'on considère l'anxiété spécifique à la grossesse (d = -.16). Toutefois, les tailles d'effet étant très faibles, ces résultats invitent à nuancer les conclusions quant à l'existence d'un effet de programmation du stress sur ces caractéristiques de naissance. Dans un second temps, une étude empirique a été réalisée dans le but d'examiner la contribution des variables prénatales et postnatales dans l'association entre le stress maternel prénatal et les caractéristiques émotionnelles de l'enfant, documentées au moyen d'une mesure d'attention/éveil et de la sécrétion cortisolaire. Les résultats suggèrent que l'état de stress de la mère en contexte prénatal et l'anxiété de la mère en contexte postnatal exerceraient des effets indépendants sur la sécrétion cortisolaire basale des nourrissons de trois mois. Le patron cortisolaire serait par ailleurs associé à l'anxiété maternelle postnatale seulement. Finalement, l'état de stress prénatal, les événements de vie et la sensibilité maternelle contribueraient ensemble à la prédiction de l'attention du nourrisson à trois mois. Globalement, ces résultats suggèrent non seulement que le contexte prénatal est associé au développement émotionnel en bas âge, mais ils soulignent également la contribution du contexte postnatal à l'émergence des caractéristiques émotionnelles des nourrissons. À la lumière de ces résultats, il apparaît souhaitable que les études futures.

En général, on présume que le stress maternel durant la période du nourrisson pourrait exercer une influence sur la réactivité de l'enfant par le biais du comportement maternel à l'égard de l'enfant. La majorité des études répertoriées abordent cette question par le biais de mesures où le stress maternel est évalué de manière rétrospective ou à un seul moment durant la période du nourrisson. Ces études tiennent pour acquis une certaine stabilité dans le stress maternel et, par conséquent, peu d'entre elles ont mesuré le stress de la mère à différents moments durant la période postnatale. Le but de la première étude est d'examiner la présence de trajectoires de stress parental auprès de 107 mères adolescentes alors que leur enfant est âgé de 6 à 18 mois. Les résultats montrent que les mères se distinguent dans leur expérience de stress parental et que l'appartenance à l'une ou l'autre des 3 trajectoires peut s'expliquer notamment par leur âge, l'irritabilité de l'enfant et les symptômes dépressifs. Dans le cadre du deuxième article, l'hypothèse selon laquelle la relation entre le stress parental et la réactivité de l'enfant serait médiatisée par la sensibilité maternelle est étudiée à l'aide de données longitudinales et provenant de sources de données objectives et rapportées par la mère. Les résultats infirment l'hypothèse de médiation. Il semble que la sensibilité maternelle observée soit en lien avec les dimensions de réactivité de l'enfant, à la fois rapportés par la mère et observés. Les résultats de la deuxième étude sont abordés sous l'angle méthodologique et clinique.

C'est un événement perçu comme positif par la société mais il peut être vécu comme traumatisant. La prévalence du trouble de stress-post traumatique lié à l'accouchement varie de 1 à 2 % (Denis et al.,2009 ; Ayers et al., 2008 ; Manzano et al., 2002). Il peut être responsable d'un moins bon investissement mère-bébé (Davies et al., 2008). Dans notre étude observationnelle, prospective, non randomisée, nous avons inclus 22 femmes de plus de 18 ans dans la Vienne. Après avoir recueilli leur consentement, nous avons évalué, au cours du dernier trimestre de grossesse, les dimensions suivantes (T0) : les événements de vie traumatique et l'existence d'un trouble de stress post-traumatique, l'anxiété liée à la grossesse, la dépression du prépartum, la peur de l'accouchement et le lien mère-fœtus. Puis à 4 à 6 semaines en post-partum (T1), un deuxième temps a permis d'évaluer le vécu de l'accouchement et son caractère traumatique, la dépression du post-partum et la relation mère-bébé. Pour chacune des dimensions sus-citées, nous avons utilisé des auto-questionnaires. Seulement 2 femmes ont caractérisé leur accouchement de traumatique mais aucune n'a développé un trouble de stress post-traumatique secondaire à l'accouchement. Le nombre d'événements traumatiques vécu au cours de la vie concernait 50 % de la population dont 45 % d'agressions sexuelles et 27 % physiques.
