Book excerpt: Persons of Arab (North African/ Middle Eastern) descent have faced unique psychosocial stressors due to the socio-political backlash following the World Trade Center attacks. These are compounded by traditional acculturation challenges, leading to increased psychological distress. This study satisfies an urgent need to better understand the acculturation and mental health experiences of Arab Americans by comprehensively and rigorously investigating hypotheses introduced in the previous literature. Participants were 611 Arab Americans from 35 U.S. states who completed a secure Internet form. Variables assessed were: acculturation (Vancouver Index of Acculturation- Modified Arab Version), acculturative stress (SAFE Acculturation Stress Scale- Revised; included issues of discrimination and American foreign policy), family functioning (Family Assessment Device- General Functioning Subscale), social support (Personal Resource Questionnaire85- Revised), religious coping (Brief Arab Religious Coping Scale), anxiety (Beck Anxiety Inventory), depression (Center for Epidemiologic Studies- Depression Scale), and a socio-demographics survey. Seven research foci were examined. First, 13 socio-demographic indexes hypothesized to relate to acculturation (including SES, generation status, length of residence in the U.S., English vs. Arabic language use, and Arabic TV viewing) were found to relate to American identity in the expected directions, while most indexes did not relate to Arab identity. Second, participants reported significantly higher anxiety and depression compared to normative samples and studies with other ethnic minority groups. Third, as hypothesized, integrated and assimilated individuals reported less stress, anxiety, and depression compared to those who were separated or marginalized. Fourth, as hypothesized, acculturation stress correlated with anxiety and depression, and both family functioning and social support related to less stress and less psychological distress. However, religious coping did not correlate with mental health. Fifth, contrary to the hypotheses, females did not demonstrate greater stress or psychological distress compared to males. Consistent with hypotheses, Christians demonstrated significantly less acculturative stress, anxiety, and depression compared to Muslims. Finally, structural equation modeling was used to develop a tenable model that explained the complex interactions among SES, acculturation, stress, mental health, and coping resources. Specific and practical recommendations are provided for clinical intervention, community programming and policy planning, and future research.

This study explored the experiences of five Arab American mothers of children with ASD. A qualitative interview approach, utilizing in-depth interviews, was used in order to investigate Arab American parents experiences of caring for children with ASD, their cultural beliefs and understandings of their childrens ASD, and their concerns and needs regarding their children. Six themes emerged from the interview data, which offered insight into Arab American parents unique experiences of caring for a child with ASD, including (1) parents journeys toward the acceptance of the diagnosis of ASD, (2) beliefs about the cause of autism, (3) concerns, (4) needs, (5) coping techniques, and (6) unanticipated positive effects. Findings of this study and aspects related to Arab cultural beliefs with the ASD diagnostic process are discussed in detail, as well as implications of the findings for the field of special education and recommendation for future research.

Book excerpt: Autism spectrum disorder (ASD) is a developmental disorder marked by communication limitations and behavioral features that vary broadly across individuals. The identification of ASD has increased by approximately 30% since 2008. As the identification of ASD continues to increase, similarly so does the number of caregivers of children with ASD, making this an important, and growing population. Since children with ASD may have a unique phenotypic profile of behaviors caregivers of children with ASD often face increased levels of stress, and experience higher levels of stress compared to other populations, including caregivers of children of other developmental disabilities. Chronic stress is a known risk factor for a wide range of chronic diseases, including cardiometabolic and mental health outcomes. Chronic stress may also influence health behaviors such as smoking, alcohol use and physical activity through coping mechanisms. The occurrence of chronic diseases and their risk factors in caregivers of children with ASD has not been well studied. There is a need to further investigate specific child behaviors as correlates of caregiver health and health behaviors. This project included two studies. The first study examined stress in caregivers of children with ASD and focused on specific child behaviors, within a locally-recruited sample of N=116 caregivers of children with ASD aged 3-17 years. Caregivers completed the Autism Behavior Inventory-Short as a measure of child behaviors, and the Parent Stress Scale as a measure of total parenting stress. We used linear regression to determine the association between specific child behaviors and caregiver stress. We hypothesized that caregivers of children with higher levels of behavioral difficulties (social-communication, mental health, repetitive behaviors) will report higher levels of self-reported stress, compared to caregivers of children with fewer behavioral difficulties. More behavioral difficulties in each domain was associated with higher levels of parental stress. For the second study, data on health behaviors and health outcomes from the National Health Interview Survey 2016-2017 cycles were used to compare caregivers of children with ASD to caregivers of typically developing children and children with other disabilities (N=10,162). For the child, information on child's diagnosis was reported by the parent. For the parent, health risk behaviors and health outcomes were self-reported. We used logistic regression to determine effects and hypothesized that caregivers of children with ASD will report more adverse health behaviors (i.e., smoking, alcohol use, less physical activity) compared to caregivers of typically developing children and caregivers of children with other disabilities. We also hypothesized that caregivers of children with ASD will have more cardiometabolic outcomes (i.e., type 2 diabetes, obesity, hypertension, high cholesterol) and more mental health symptoms compared to caregivers of typically developing children and caregivers of children with other disabilities. Compared to typically developing caregivers, caregivers of children with ASD had higher odds of physical inactivity, smoking and tobacco use, and heavy drinking, though these relationships varied when compared to other disability groups. The effect estimates for these relationships also were of low magnitude and were relativity imprecise. Similar patterns of association were shown for health outcomes compared to caregivers of children with other disabilities. Overall, our findings suggest that it is the culmination of the child's phenotype that results in increased caregiver stress, rather than individual, unique behaviors. This may support the idea that is the total experience of all behaviors that is important, including those not required for an ASD diagnosis, but that are commonly shared with other developmental disorders. Furthermore, caregivers of children with ASD had more adverse health behaviors and outcomes compared to caregivers of typically developing children, but not when compared to other disability groups, though these estimates had low magnitude and were relatively imprecise. Caregiver stress should be considered for potential interventions to improve their health. Future studies should seek to better understand why child behaviors influence caregiver stress and how caregiver stress may influence health risk behaviors and health outcomes.

As such, this study investigated factors influencing mental health with the Arab American population in the United States. There is a growing body of evidence suggesting that biculturalism may be a protective factor (Mezzich, Ruiperez, Yoon, Liu, & Zapata-Vega, 2009), thus the first hypothesis postulated that after controlling for demographic variables that will act as covariates, biculturalism will predict well-being. As Arab Americans are minorities that have encountered high levels of racism and discrimination (Zogby, 2001), hypothesis two suggested that after controlling for demographic variables that will act as covariates, race-related stress and perceptions of a racist environment will predict well-being. Recent research with Arab Americans has shown that acculturation impacts experiences with racism (Awad, 2010), thus, hypotheses 3 and 4 proposed that biculturalism may moderate the effects of race-related stress on well-being and biculturalism may moderate the effects of perceptions of a racist environment on well-being. Finally, as Arab Americans are an underserved population with high levels of stigma towards mental health services (Al-Krenawi & Graham, 2000), the exploratory hypothesis assumed that biculturalism, race-related stress, perceptions of a racist environment, and well-being will predict attitudes towards seeking professional psychological help. Several measures were used to investigate these hypotheses. Cortes, Rogler and Malgady Bicultural Scale (Mezzich et al., 2009) measured the degree to which one identified with host and home-cultures. Well-being, conceptualized as self-esteem, depression, and life-satisfaction, was measured using the Rosenberg Self-Esteem Scale (Rosenberg, 1965), the Center for Epidemiologic Studies Depression Scale (Radloff, 1977) and the Satisfaction with Life Scale (SWLS; Diener, Emmons, Larsen, & Griffin, 1985). Race-Related Events Scale (RES) evaluated exposure to race-related stress (Waelde et al., 2010a), while Perceptions of a Racist Environment determined participants' perceived racism (Waelde, 2010b). Finally, the modified Attitudes Towards Seeking Professional Psychological Help Scale ascertained help-seeking attitudes (Turner & Fisher, 1970). These self report measures were administered to participants who identified themselves as Arab American (N=166). Participants were mainly recruited from Arab American organizations and completed either a written or online version of the survey. Multiple regression analyses revealed that biculturalism does not influence well-being. However, household income impacted depression levels, such that as income rises, depression decreases. Additionally, race-related stress does not predict well-being; however, perceptions of a racist environment does, such that as perceived racism increases, depression increases and self-esteem decreases. Biculturalism also does not have a moderating effect on either race-related stress or perceptions of a racist environment on well-being. Finally, biculturalism, well-being, race-related stress, and perceptions of a racist environment did not impact attitudes towards help-seeking. The major limitation of this study is that the sample is not generalizable to all Arab Americans. Future studies on Arab Americans should have a larger and more diverse Arab sample, as research on this underserved, growing minority is essential. Expanding literature on Arab Americans helps shed light on their specific mental health stressors, which inevitably increases awareness and cultural competency in clinical settings.

Book excerpt: Autism Spectrum Disorder (ASD) is a lifelong developmental disorder that affects children in the areas of behavior, communication and socialization. Currently, 1 in 44 children across the U.S. has an ASD. Displays of behaviors that depart from social norms can lead to the social stigmatization of children with ASD, which can extend to parents and caregivers. Consequently, these parents or caregivers may become the subject of negative social reactions and alienation or affiliate stigma. This affiliate stigma can contribute to parents' feelings of stress and mental health challenges. Research demonstrates that stigmatization of individuals with ASD is present in the African American community and exerts negative effects on the well-being of children with ASD and their families. However, most of the research has not yet adequately examined the relationship between the stigma process and parental efforts in raising a child with ASD for African American families. Grounded in Link and Phelan's conceptualizations on how social stereotyping contributes to stigmatization, Kinnear and colleagues studied the process of parents' difficulties with stigma and its impact on their difficulties in raising a child with ASD. However, Kinnear's sample population was mostly racially white (84.5%). Consequently, this study aimed to close the gap in research by examining whether there is a significant relationship between parental difficulties with stigma in relation to African American parental difficulties in raising a child with ASD. This cross-sectional study involved primary data collection through an online survey. A sample of 2,276 African American adult parents was recruited through national disability and autism-specific organizations. Study hypotheses were tested using multiple regression analyses (MRA) with selected controls. For African American study participants, parental perceptions of stereotypes, family isolation, and family exclusion did not impact their difficulty of raising a child with ASD in the full replicated model. However, when the targeted dependent variable was changed within the model process, only family isolation did not predict difficulty of raising a child with ASD. Social workers and other mental health professionals can benefit from an increased understanding of the relationship between the stigma process and difficulty with raising a child with ASD that this study provides. Further discussion on study findings with an expanded model, implications for practice, limitations, and recommendations for future research are presented as well.
