Book excerpt: "Hypoxaemia is a major contributor to child deaths that occur worldwide each year; for a child with pneumonia hypoxaemia increases the risk of death by up to 5 times. Despite its importance in virtually all types of acute severe illness, hypoxaemia is often not well recognized or well managed, more so in settings where resources are limited. Oxygen therapy remains an inaccessible luxury for a large proportion of severely ill children admitted to hospitals in developing countries. This is particularly true for patients in small district hospitals, where, even if some facility for delivering oxygen is available, supplies are often unreliable and the benefits of treatment may be diminished by poorly maintained, inappropriate equipment or poorly trained staff with inadequate guidelines. Increasing awareness of these problems is likely to have considerable clinical and public health benefits in the care of severely ill children. Health workers should be able to know the clinical signs that suggest the presence of hypoxaemia and have more reliable means of detection of hypoxeamia. This can be achieved through more widespread use of pulse oximetry, which is a non-invasive measure of arterial oxygen saturation. At the same time oxygen therapy must be more widely available; in many remote settings, this can be achieved by use of oxygen concentrators, which can run on regular or alternative sources of power. Having effective systems for the detection and management of hypoxaemia are vital in reducing mortality from pneumonia and other severe acute illnesses. Oxygen therapy is essential to counter hypoxaemia and many times is the difference between life and death. This manual focuses on the availability and clinical use of oxygen therapy in children in health facilities by providing the practical aspects for health workers, biomedical engineers, and administrators. It addresses the need for appropriate detection of hypoxaemia, use of pulse oximetry, clinical use of oxygen and delivery systems and monitoring of patients on oxygen therapy. In addition, the manual addresses practical use of pulse oximetry, and oxygen concentrators and cylinders in an effort to improve oxygen systems worldwide."--Publisher's description.

Book excerpt: With the rapid expansion of the Asia-Pacific economy in the last decade and the recovery after the recent crisis, severe demands will be placed on energy services and the environment. Coping with the volatile oil prices that persist in the market introduces an additional factor into the energy supply and demand equation, not just for countries in this region but also worldwide. Inevitably there will be implications for environmental issues too. The future will see a continuing challenge to balance growth with sustainability in the economic, social and environmental sectors. This conference, a sequel to the immensely successful APCSEET conferences held in Singapore and Australia, is aimed at meeting that challenge by addressing the pertinent issues related to sustainable energy and environmental protection. It provides a forum for participants from academia, industry and government agencies to interact, report on research in progress, and identify opportunities in the fields of sustainable energy and environmental technologies. The presentations include not only technical issues such as air pollution control, wastewater treatment, solid waste management, renewable energy and cleaner production, but also education and policy issues.

Book excerpt: From 14-16 May 2024, WHO convened 133 participants from 64 Member States and 69 partners at the National Oxygen Scale-Up Framework Meeting in Dakar, Senegal. The objective of this meeting was to co-develop, with all attendees, a template for planning and costing national oxygen scale-up strategies which could then be used by Member States to strengthen their national medical oxygen ecosystems, a key operative of WHA76.3 Increasing access to medical oxygen. During the meeting, participants engaged in knowledge exchange, highlighting best-practice and lessons learned from current national scale-up efforts via plenary sessions and breakout group discussions. Topics covered were partnerships, needs identification, sustainable financing, measuring impact and outcomes, workforce planning, and integration with health programmes and across relevant sectors. Additionally, participants had the opportunity to share their own oxygen scale-up experiences via a poster showcase. To reach the objective, participants were invited to vote on the necessary components of a previously developed zero-draft oxygen scale-up template using a modified Delphi methodology. Through two rounds of voting, consensus was reached on necessary components with one significant modification and the addition of 6 new items. This report summarizes the three day meeting and includes all associated materials.

This report, strongly influenced by the COVID-19 pandemic, paints a bleak picture of the future and describes a contested, fragmented and turbulent world. It specifically discusses the four main trends that will shape tomorrow's world: - Demographics-by 2040, 1.4 billion people will be added mostly in Africa and South Asia. - Economics-increased government debt and concentrated economic power will escalate problems for the poor and middleclass. - Climate-a hotter world will increase water, food, and health insecurity. - Technology-the emergence of new technologies could both solve and cause problems for human life. Students of trends, policymakers, entrepreneurs, academics, journalists and anyone eager for a glimpse into the next decades, will find this report, with colored graphs, essential reading.

Available in PDF, EPUB and Kindle. Book excerpt: A close look at the evolution of American political alliances in Asia and their future While the American alliance system in Asia has been fundamental to the region's security and prosperity for seven decades, today it encounters challenges from the growth of China-based regional organizations. How was the American alliance system originally established in Asia, and is it currently under threat? How are competing security designs being influenced by the United States and China? In Powerplay, Victor Cha draws from theories about alliances, unipolarity, and regime complexity to examine the evolution of the U.S. alliance system and the reasons for its continued importance in Asia and the world. Cha delves into the fears, motivations, and aspirations of the Truman and Eisenhower presidencies as they contemplated alliances with the Republic of China, Republic of Korea, and Japan at the outset of the Cold War. Their choice of a bilateral "hub and spokes" security design for Asia was entirely different from the system created in Europe, but it was essential for its time. Cha argues that the alliance system’s innovations in the twenty-first century contribute to its resiliency in the face of China’s increasing prominence, and that the task for the world is not to choose between American and Chinese institutions, but to maximize stability and economic progress amid Asia’s increasingly complex political landscape. Exploring U.S. bilateral relations in Asia after World War II, Powerplay takes an original look at how global alliances are achieved and maintained.
