Book excerpt: Ankylosing spondylitis (AS), currently grouped under the term axial spondyloarthritis (axSpA), is a chronic form of arthritis that causes inflammatory back pain and can gradually result in impaired spinal mobility. It can also cause eye inflammation (uveitis), arthritis of limb joints, inflammation at sites of bony attachments of tendons and ligaments (enthesitis), and associated psoriasis and inflammatory bowel disease. No cure has yet been found; however, early diagnosis and proper medical and lifestyle management are very important in reducing the risk of deformity and disability. Written by a leading expert on the subject who himself has been living with AS since age 12, this book provides clear and practical information on the clinical symptoms and signs, underlying causes, genetic predisposition, diagnosis, treatments, and impact on daily life. The combination of scientific knowledge and personal experience results in a unique book which will prove invaluable for patients and their families and friends, as well as allied health professionals and students wishing to know more about the condition. The second edition has been fully updated to cover the tremendous advancements made in the clinical recognition and effective management since the first edition in 2002. It also includes a glossary of medical terms and links to sources of further information to give patients the tools they need to understand and manage the condition effectively.

Available in PDF, EPUB and Kindle. Book excerpt: Ankylosing spondylitis, the third most common form of chronic arthritis, is a systemic rheumatic disorder characterized by inflam mation of the axial skeleton (spine and sacroiliac joints), and a host of systemic manifestations. With comprehensive care, the vast majority of patients can lead full, productive lives. However, management can succeed only with patient education and exercise. Recent communication from my co-editor, Carson Dick, serves to remind me that there are several unresolved issues concerning drug therapy in ankylosing spondylitis. Clearly, in spite of my views, there are others who do not believe that the non-steroidal anti-inflammatory drugs (NSAIDs) alter favorably the course of disease and they must be administered for prolonged periods and in anti-inflammatry quan tities to be effective. I would agree with Carson Dick that aspirin and phenylbutazone are way down the list of drug priorities following the marketing of other NSAIDs that are effective and safer. I am grateful to my contributors to this volume, all recognized authorities on their particular topic. It has been a privilege col laborating with them on this particular volume. ABOUT THE EDITOR John J. Calabro, MD, is Professor of Medicine and Pediatrics at the University of Massachusetts Medical School and Director of Rheumatology at Saint Vincent Hospital, both in Worcester, Mas sachusetts, USA. He is the author of over 260 scientific articles, including several monographs and a book on arthritis for patients.

Available in PDF, EPUB and Kindle. Book excerpt: Axial Spondyloarthritis is a timely addition to the Oxford Textbooks in Rheumatology series, providing a comprehensive reference to this rapidly evolving field. The conceptual framework of the disease has now evolved beyond ankylosing spondylitis to encompass a broader concept of axial inflammation. Earlier recognition has opened the door to earlier intervention, and the understanding of the biologic basis of axial SpA has seen significant advances in recent years. The first textbook to cover axial spondyloarthritis in this level of detail from a global perspective, this volume offers a practical and complete resource compiled by a multidisciplinary and multinational team of experts. Beginning with a historical perspective on the disease, the textbook provides a comprehensive background in the epidemiology, diagnosis, and classification of axial spondyloarthritis. Immune mechanisms and genetics are covered, along with imaging and a detailed section on the range of treatments options currently available. A section on extra-articular manifestations provides a comprehensive knowledge base for the clinician treating patients with spondyloarthritis, and chapters on the economics of the disease, physical functioning, and patient registries complete the broad coverage of the topic. Practical, easy to use, yet detailed with respect to pathophysiology, diagnostics, and management, this textbook provides a comprehensive reference for rheumatologists, general practitioners, internists, and paediatricians managing patients with axial spondyloarthritis.

Available in PDF, EPUB and Kindle. Book excerpt: Becoming the Iceman is a project inspired by Wim and Justin to show the world that anyone can adopt the ability to become an Iceman or Icewoman. The project's goal is to show that the ability to control the body's temperature is not a genetic defect in Wim, but an ability that can be adopted by everyone. For many generations, we have been taught to fear the cold: "Don't forget your jacket You don't want hypothermia, do you?""Put your gloves on before you get frostbite "Of course, these are consequences of extreme cold exposure, but with the proper understanding, anyone can learn to use the cold as a natural teacher. You may have seen Wim Hof on television running barefoot through the snow or swimming in ice-cold waters. While performing those incredible feats, Wim remained completely warm and comfortable the entire time Wim is the epitome of what can happen if someone uses the cold to train the body. Like any new tool, you must understand how it works before you can use it efficiently. This pertains to the cold as well. You may be wondering, "How can you prove that anyone can learn this ability?Well, as of Fall 2009, Justin Rosales had no experience with the cold whatsoever. He was a college student attending Penn State University. After Justin's friend, Jarrett, showed him one of Wim's videos on You-Tube, he became exceedingly interested in understanding this ability. He wanted to see if it was possible for anyone to learn. In Spring 2010, after speaking to Wim for several months via email, Wim invited Justin to attend his workshop in Poland. After many weeks of working as a dishwasher, Justin was able to pay for the trip and learn the technique of the Iceman. With more training and countless experiences with the cold, Justin began to slowly adapt. The length of time he could remain exposed increased dramatically. He quickly realized that the technique to withstand the cold was, indeed, an ability that could be learned by everyone. This book tells the tale of Wim and Justin's journey to Becoming the Iceman
