Available in PDF, EPUB and Kindle. Book excerpt: Despite the amazing progress made by the stereotactic technique, particularly regarding the localization of the target, despite the extreme caution, which stereotactic neurosurgeons apply at every step of the procedures, despite the routine roentgenologic and physiologic controls (depth EEG, electric stimulation) preceding the production of a lesion, there remains a certain degree of uncertainty regarding the position, shape and extent of the lesion as well as of the electrode track and also regarding unintended lesions in the vicinity. The final answer to these questions depends on the anatomical control. Thus we must be grateful to the authors, who performed 3700 subcortical stereotactic interventions, that they present a careful comparison of the clinical and of the pathologic anatomic findings, in the cases whose brains could be studied. They demonstrate what degree of exactness can be expected from the stereotactic technique. They also present a careful outline of the morphology, physiology, biochemistry and pathology of the structures that are affected in Parkinson patients. In addition this monograph permits one to familiarize oneself with the methods of stimulation and destruction practiced by the authors. The relevant literature is conscientiously quoted. A study of this mono graph will benefit not only the stereotactic neurosurgeon, but also neurophysiologists, pathologists and practicing neurologists.

Available in PDF, EPUB and Kindle. Book excerpt: Deep brain stimulation for the treatment of patients with Parkinson’s disease was introduced in the 1990s. Initially performed only at academic centers, over the past decade it has become a widespread surgical procedure. A variety of surgical techniques are employed and innovations are introduced frequently. This book is an ideal source of information for the many practicing neurosurgeons who did not learn this surgery during their training but would now like to add it to their practice, as well as an excellent update on exciting new developments in surgery for Parkinson’s disease. This book is designed to provide practicing neurosurgeons with current knowledge on the practical aspects of surgical treatment of patients with Parkinson’s disease. It explains how to identify surgical candidates and determine the optimal surgery, describes the various surgical techniques that are currently employed, and offers insights into how to optimize deep brain stimulation therapy after implantation. The keys to avoidance of surgical complications are carefully elucidated. In addition, an overview is provided of potential advances on the near-term horizon, including closed-loop deep brain stimulation, gene therapy, and optogenetics. All topics are covered by experienced Parkinson’s disease surgeons, in a concise and digestable format. The book will be an ideal source of information for the many practicing neurosurgeons who would like to add deep brain stimulation to their practice, as well as an excellent update on new developments in surgery for Parkinson’s disease.

Book excerpt: Since its introduction about sixty years ago, stereotactic and functional neurosurgery has evolved into a fascinating and interdisciplinary endeavor that combines modern neurosurgery, neurobiology, and neuroimaging with innovative diagnostic and treatment strategies. In this collection, acknowledged experts from Europe and North and South America present their scientific and clinical experience in stereotactic and functional neurosurgery for movement disorders and brain tumors. The contributions present a wide range from the beginnings of human stereotactic neurosurgery to the most modern molecular and restorative strategies to treat diseases of the human nervous system. They clearly demonstrate that the discipline is still young and dynamic with alternative and sometimes competing strategies whose evaluation is underway. They also document that operative lesioning techniques such as thalamotomies, though still chosen under certain circumstances, have been succeeded by novel neuromodulation techniques such as deep brain stimulation in the great majority of clinical cases.

Available in PDF, EPUB and Kindle. Book excerpt: This text presents a comprehensive and state-of the-art approach to stereotactic and functional neurosurgery. Overarching sections include achieving stereotactic precision, defining trajectories and targets, the biophysics of stereotactic therapies, diseases and targets, and the future of functional neurosurgery. Each section is designed to be inclusive of all relevant topics, serving as an unbiased resource to new clinicians in this field or established clinicians that are aiming to better understand complementary methods. Importantly, each section and the associated chapters can be used by basic and translational scientists as well as engineers and industry to better understand and deliver innovation to the field. Chapters within each section methodically analyze traditional and recently emerging concepts and techniques; address underlying principles with examples drawn from specific diseases and applications; and cover patient selection, target selection, available stereotactic methods, nuanced surgical methods, and clinical evidence across treatment options. Written by experts in each area, Stereotactic and Functional Neurosurgery is a definitive guide to the latest developments in stereotactic targeting, electrode implantation, surgical treatment of neurological and psychiatric disorders, the renaissance of stereotactic lesions, and the frontier of restorative neurosurgery for a variety of disorders that have no other therapeutic options.

Book excerpt: There has been a resurgence in stereotactic surgery for the treatment of Parkinson's disease, tremor, and dystonia, and with it a growing need for up-to-date information about these procedures. In Surgical Treatment of Parkinson's Disease and Other Movement Disorders, a panel of highly experienced neurosurgeons, neurophysiologists, neuropsychologists, and neuroanatomists join forces to create an integrated, cutting-edge survey of all the methodologies necessary for successful surgical treatment. On the surgical side they review a variety of techniques for Parkinson's disease, including thalamotomy, pallidotomy and bilateral pallidotomy, subthalamotomy, pallidal and thalamic deep brain stimulation, and subthalamic deep brain stimulation. Surgical therapy for tremor and dystonia is also covered. On the patient side, the authors systematically examine the key issues involved in patient selection, patient assessment before and after surgery, brain target localization, and postoperative programming of brain stimulators. Comprehensive and authoritative, Surgical Treatment of Parkinson's Disease and Other Movement Disorders integrates the diverse techniques now available in this rapidly growing field to provide a practical guide to effective treatment of patients afflicted with these disorders.

Book excerpt: The 10th International Winter Conference on Neurodegeneration (lWCN) has taken place from February 14-16,2002, at the lovely "SchloB Ziethen", an old prussian manor (in Prussia, 'faute de mieux', called 'SchloB', i. e. cas tle or residence). This place is 20 km off Tegel Airport, Berlin's main airport, and has been beautifully restored by baroness Edith von Thiingen (nee von BUlow, i. e. of historical Prussian aristocracy); it lends itself rather perfectly for the IWCN type of small interdisciplinary workshops on neurodegenera tion which combine short lectures with plenty of discussion. In this context, we could quote Alexander von Humboldt's famous opening words for one of the first international scientific meetings held at Berlin, on September 18, 1828: "The main purpose ... does not consist in a mutual reading of manuscripts all to be printed after at least one year in specialised publications, but in the personal communication amongst those who work in similar scientific fields; the oral and thus more stimulating exchange of ideas, might they represent facts, opinions or doubts; the foundation of friendly relations which convey illumination to our sciences, serene grace to our lives and tolerance and mildness to our habits ... Berlin, Sept. 18, 1828 Alexander von Humboldt To the IWCN aficionado, it may come as a little surprise that this is the pub lication of the 10th Winter Conference as this might mean that they must have missed number 9.
