Decades of work by researchers, practitioners, and policy-makers has shown that health is shaped by a myriad of factors, including the biophysical environment, climate, political economy, gender, social networks, culture, and infrastructure. Yet while there is emerging interest within the natural and social sciences on the social and ecological dimensions of human disease and health, there have been few studies that address them in an integrated manner. Ecologies and Politics of Health brings together contributions from the natural and social sciences to examine three key themes: the ecological dimensions of health and vulnerability, the socio-political dimensions of human health, and the intersections between the ecological and social dimensions of health. The thirteen case study chapters collectively present results from Africa, Asia, Latin America, the United States, Australia, and global cities. Section one interrogates the utility of several theoretical frameworks and conventions for understanding health within complex social and ecological systems. Section two concentrates upon empirically grounded and quantitative work that collectively redefines health in a more expansive way that extends beyond the absence of disease. Book excerpt: Eastern Africa’s livestock keepers face many challenges, not least the widespread prevalence of endemic diseases which both undermine animals’ productivity and increase livestock mortality. Tsetse- transmitted trypanosomosis causes significant economic losses, in particular in cattle. This study analyses these losses in a spatially explicit framework for the six tsetse-infested countries of the Intergovernmental Authority on Development (IGAD) region: Ethiopia, Kenya, Somalia, South Sudan, Sudan and Uganda. The cattle production systems of the region are diverse, ranging from pastoralism to agropastoralist and mixed crop-livestock farming. Some areas make extensive use of draught cattle or of high yielding crossbred dairy cows. Based on these features, twelve cattle production systems in the region were characterized and mapped. In these systems, the potential incomes from cattle production were modeled for a situation with and without trypanosomosis; the models looked at mortality, fertility, other productivity parameters and cattle population growth and expansion. The results of the analysis were used to generate a map of the potential benefits of controlling the disease. Estimates were then made of the costs of tsetse and trypanosomosis control using a range of techniques, namely: trypanocidal drugs; control or localized elimination of tsetse flies using insecticide-treated cattle or targets, aerial spraying and the sterile insect technique. The mapped potential benefits and mapped estimated costs were combined in order to produce a series of benefit-cost maps which illustrate what techniques are likely to be the most economically attractive in different areas of the study region. The suite of tools and economic analyses documented in this paper provide essential information to decision makers for comparing and prioritizing interventions in the region.

Book excerpt: "The presence, or absence, of neglected tropical diseases (NTDs) can be seen as a proxy for poverty and for the success of interventions aimed at reducing poverty. Today, coverage of the public-health interventions recommended by the World Health Organization (WHO) against NTDs may be interpreted as a proxy for universal health coverage and shared prosperity - in short, a proxy for coverage against neglect. As the world's focus shifts from development to sustainable development, from poverty eradication to shared prosperity, and from disease-specific goals to universal health coverage, control of NTDs will assume an important role towards the target of achieving universal health coverage, including individual financial risk protection. Success in overcoming NTDs is a "litmus test" for universal health coverage against NTDs in endemic countries. The first WHO report on NTDs (2010) set the scene by presenting the evidence for how these interventions had produced results. The second report (2013) assessed the progress made in deploying them and detailed the obstacles to their implementation. This third report analyses for the first time the investments needed to achieve the scale up of implementation required to achieve the targets of the WHO Roadmap on NTDs and universal coverage against NTDs. INVESTING TO OVERCOME THE GLOBAL IMPACT OF NEGLECTED TROPICAL DISEASES presents an investment strategy for NTDs and analyses the specific investment case for prevention, control, elimination and eradication of 12 of the 17 NTDs. Such an analysis is justified following the adoption by the Sixty-sixth World Health Assembly in 2013 of resolution WHA6612 on neglected tropical diseases, which called for sufficient and predictable funding to achieve the Roadmap's targets and sustain control efforts. The report cautions, however, that it is wise investment and not investment alone that will yield success. The report registers progress and challenges and signals those that lie ahead. Climate change is expected to increase the spread of several vector-borne NTDs, notably dengue, transmission of which is directly influenced by temperature, rainfall, relative humidity and climate variability primarily through their effects on the vector. Investments in vector-borne diseases will avoid the potentially catastrophic expenditures associated with their control. The presence of NTDs will thereby signal an early warning system for climate-sensitive diseases. The ultimate goal is to deliver enhanced and equitable interventions to the most marginalized populations in the context of a changing public-health and investment landscape to ensure that all peoples affected by NTDs have an opportunity to lead healthier and wealthier lives."--Publisher's description.

The public health impact and financial consequences of these diseases are particularly hard on the already overburdened economies of developing countries especially in the tropics. Many of these disease agents utilize insect hosts (vectors) to achieve their transmission to mammals. In the past, these diseases were largely controlled by insecticide-based vector reduction strategies. Now, many of these diseases have reemerged in the tropics, recolonizing their previous range, and expanding into new territories previously not considered to be endemic. Habitat change, irrigation practices, atmospheric and climate change, insecticide and drug resistance as well as increases in global tourism, human traffic and commercial activities, have driven the reemergence and spread of vector borne diseases. While these diseases can be controlled through interventions aimed at both their vertebrate and invertebrate hosts, no effective vaccines exist, and only limited therapeutic prospects are available for their control in mammalian hosts. Molecular technologies such as transgenesis, which is the subject of this book, stand to increase the toolbox and benefit disease management strategies.
