Topics explored include: A Human Rights Approach Toward Eradicating Obstetric Fistula Obstetric Fistula: A Case of Miscommunication – Social Experiences of Women with Obstetric Fistula Classification of Female Genital Tract Fistulas Training and Capacity-Building in the Provision of Fistula Treatment Services Designing Preventive Strategies for Obstetric Fistula Sexual Function in Women with Obstetric Fistula Social and Reproductive Health of Women After Obstetric Fistula Repair Making the Case for Holistic Fistula Care Addressing Mental Health in Obstetric Fistula Patients Physical Therapy for Women with Obstetric Fistula A Multidisciplinary Approach to Obstetric Fistula in Africa is designed for professional use by NGOs, international aid organizations, governmental and multilateral agencies, healthcare providers, public health specialists, anthropologists, and others who aim to improve maternal health across the globe. Available in PDF, EPUB and Kindle. Book excerpt: Introduction: Obstetric fistula is the presence of an opening between a woman’s genital tract and urinary tract or between the genital tract and the rectum due to untreated obstructed labor or iatrogenic causes. This disease is characterized by the leakage of the urine and/or stools through the vagina. Objective: The general objective of this need assessment is to contribute in decreasing the obstetric fistula related morbidity Nepal by providing strong information that could help in struggle against Obstetric Fistula related morbidity. Methods: This is a cross-sectional study of 4 health sites in the Republic of Nepal between, November 1rst 2011 and December 16th 2011. Data were collected on the prevention, the management, the social reintegration, the training and the research related obstetric fistula. Standard assessment tool, structured questionnaire for health workers and patient’s interview were used. We have also performed a literature review on obstetric fistula in Nepal. Results: With the MMR of 281/100,000 live birth, it is estimated that there is 200 to 400 new cases of OF in Nepal each year leading to the overall 4300 prevalent cases. Our assessment shows that, obstetric fistula surgery is practiced in three sites with information verifiable on the registers and files. One fourth site is willing to be really involved in struggle against OF. None of the site is dealing with the five pillar of obstetric fistula. Two of three sites have OF surgery in permanent base while one site actually proceeds by camps. Even where surgery is practiced, there is still a need for training, nursing and surgical technical protocol related to obstetric fistula. Struggle against obstetric fistula is not really coordinated at the country and regional level. Conclusion and recommendations: We recommend elaborating a national strategy of struggle against the obstetric fistulas, to elaborate protocols for care, training tools; organize the struggle against obstetric fistula while founding on the five pillars that are the prevention, management, reintegration, training and operational research. While dealing with the national strategy, it is urgent to empower the facilities really involved in OF surgery and the one committed to start the program.

Book excerpt: This open access edited book brings together new research on the mechanisms by which maternal and reproductive health policies are formed and implemented in diverse locales around the world, from global policy spaces to sites of practice. The authors – both internationally respected anthropologists and new voices – demonstrate the value of ethnography and the utility of reproduction as a lens through which to generate rich insights into professionals’ and lay people’s intimate encounters with policy. Authors look closely at core policy debates in the history of global maternal health across six different continents, including: Women’s use of misoprostol for abortion in Burkina Faso The place of traditional birth attendants in global maternal health Donor-driven maternal health programs in Tanzania Efforts to integrate qualitative evidence in WHO maternal and child health policy-making Anthropologies of Global Maternal and Reproductive Health will engage readers interested in critical conversations about global health policy today. The broad range of foci makes it a valuable resource for teaching in medical anthropology, anthropology of reproduction, and interdisciplinary global health programs. The book will also find readership amongst critical public health scholars, health policy and systems researchers, and global public health practitioners.

These shameful injuries are preventable but this will not occur until there is improved access to quality obstetric care. While improvements in these services are the top priority, there is a great need to train more surgeons, especially nationals, to have the skill and resources to help these long-suffering incontinent women.

Book excerpt: This book informs that the practice of child marriage is harmful and inimical to the health, education, social wellbeing, socio-economic empowerment, and the fundamental rights of the child. It shows the conflicts between the practice and the public policy commitments and efforts of the government of Nigeria on the right to health, education, human/fundamental rights, the MDGs, the Nigeria Vision 20:2020, the goals of NEPAD and the transformation of Nigeria. It exposes the unsustainable arguments seeking to justify child marriage. This book calls on all individuals, legal persons, state and non-state actors and development partners including YOU to join and support the advocacy to end child marriage in Nigeria. For those who are yet to appreciate the girl-child as a person of great value like the male counterpart, this book is here to change such orientation.

Book excerpt: The evaluation of reproductive, maternal, newborn, and child health (RMNCH) by the Disease Control Priorities, Third Edition (DCP3) focuses on maternal conditions, childhood illness, and malnutrition. Specifically, the chapters address acute illness and undernutrition in children, principally under age 5. It also covers maternal mortality, morbidity, stillbirth, and influences to pregnancy and pre-pregnancy. Volume 3 focuses on developments since the publication of DCP2 and will also include the transition to older childhood, in particular, the overlap and commonality with the child development volume. The DCP3 evaluation of these conditions produced three key findings: 1. There is significant difficulty in measuring the burden of key conditions such as unintended pregnancy, unsafe abortion, nonsexually transmitted infections, infertility, and violence against women. 2. Investments in the continuum of care can have significant returns for improved and equitable access, health, poverty, and health systems. 3. There is a large difference in how RMNCH conditions affect different income groups; investments in RMNCH can lessen the disparity in terms of both health and financial risk.

Available in PDF, EPUB and Kindle. Book excerpt: Obstetric fistula is a birthing injury caused by prolonged obstructed labor that results in urinary and fecal incontinence. It is nearly non-existent in the Global North. In contrast Niger, in West Africa, has one of the highest rates of fistula in the world. In Western humanitarian and media narratives, fistula is presented as deeply stigmatizing, resulting in divorce, abandonment by kin, exile from communities, depression and suicide. In Fistula Politics, Alison Heller illustrates the inaccuracy of these popular narratives and shows how they serve the interests not of the women so affected, but of humanitarian organizations, the media, and local clinics.

Book excerpt: Having a child remains one of the biggest health risks for women worldwide. Fifteen hundred women die every day while giving birth. That's a half a million mothers every year. UNICEF's flagship publication, The State of the World's Children 2009, addresses maternal mortality, one of the most intractable problems for development work. The difference in pregnancy risk between women in developing countries and their peers in the industrialised world is often termed the greatest health divide in the world. A woman in Niger has a one in seven chance of dying during the course of her lifetime from complications during pregnancy or delivery. That's in stark contrast to the risk for mothers in America, where it's one in 4,800 or in Ireland, where it's just one in 48,000. Addressing that gap is a multidisciplinary challenge, requiring an emphasis on education, human resources, community involvement and social equality. At a minimum, women must be guaranteed antenatal care, skilled birth attendants and emergency obstetrics, and postpartum care. These essential interventions will only be guaranteed within the context of improved education and the abolition of discrimination.

Available in PDF, EPUB and Kindle. Book excerpt: Despite recent advances in important aspects of the lives of girls and women, pervasive challenges remain. These challenges reflect widespread deprivations and constraints and include epidemic levels of gender-based violence and discriminatory laws and norms that prevent women from owning property, being educated, and making meaningful decisions about their own lives--such as whether and when to marry or have children. These often violate their most basic rights and are magnified and multiplied by poverty and lack of education. This groundbreaking book distills vast data and hundreds of studies to shed new light on deprivations and constraints facing the voice and agency of women and girls worldwide, and on the associated costs for individuals, families, communities, and global development. The volume presents major new findings about the patterns of constraints and overlapping deprivations and focuses on several areas key to women s empowerment: freedom from violence, sexual and reproductive health and rights, ownership of land and housing, and voice and collective action. It highlights promising reforms and interventions from around the world and lays out an urgent agenda for governments, civil society, development agencies, and other stakeholders, including a call for greater investment in data and knowledge to benchmark progress.
