Son réseau de soutien peut ètre appelé à jouer un rôle important pour l'aider à vivre cette transition avec plus de facilité. Le but de cette étude exploratoire consiste à mieux comprendre comment opèrent les différentes formes de soutien social durant les diverses étapes de ce processus. Huit (8) résidents de 64 ans ou plus, vivant dans un milieu d'hébergement alternatif de type projet novateur, les Habitations St-Christophe, sont interviewés. À l'aide d'une analyse par théorisation ancrée, les formes les plus importantes de soutien reçu sont identifiées à partir des perceptions des sujets de l'aide fournie pendant la transition. Différents modèles de transition et de soutien sont analysés. Un certain nombre d'enseignements sont présentés au niveau des interventions à mettre en place à l'intention des aînés et de leurs proches aidants. Un modèle d'intervention en cinq étapes est proposé en conclusion.

Dans ce contexte, une catégorie d'acteurs joue un rôle crucial : les proches aidants (conjoints, enfants, fratrie, voisins, amis) qui, globalement, procurent 80 % de l'aide reçue par les personnes âgées en perte d'autonomie. Constatant qu'ils sont indispensables mais risquent souvent l'épuisement, les pouvoirs publics, les associations et certains acteurs privés cherchent à développer des actions d'information, de formation, de répit ou encore de soutien psychologique, dédiées aux aidants. En s'appuyant sur des enquêtes de terrain conduites dans plusieurs départements, le présent ouvrage met en évidence les problématiques spécifiques à traiter pour développer de manière structurée et cumulative une politique publique de soutien et d'accompagnement des aidants. Il apparaît qu'une telle politique gagne à être construite au niveau infrarégional et non seulement par le biais d'appels à projets lancés au niveau national. Les auteurs proposent une grille d'analyse des besoins des aidants, et de leur évolution, susceptible d'instrumenter le diagnostic des professionnels amenés à les orienter vers tels ou tels services. L'ouvrage offre enfin une démarche de configuration des services sur les territoires prenant en compte l'hétérogénéité de ces derniers en termes de besoins, de ressources et de compétences. Cet ouvrage fournira aussi des pistes pour traiter les questions comparables dans le champ du handicap. Enfin, il éveillera l'intérêt des personnes, aidantes ou non, désireuses de mieux comprendre les enjeux et les difficultés de la mise en œuvre d'une politique d'aide aux aidants.
