Book excerpt: La fin de vie et la maladie grave constituent en France un sujet de santé publique dont l'importance est croissante. 520 000 personnes décèdent chaque année. 70% des français déclarent souhaiter mourir chez eux. Chaque année en France, 150 000 à 200 000 patients requièrent des soins techniques et relationnels intenses. Parmi l'ensemble des dispositifs de prise en charge existants, figure l'hospitalisation à domicile seule susceptible de prendre en charge chez eux des patients gravement malades ou en fin de vie. 7% des patients pris en charge par l'HAD sont en phase terminale. Les soins palliatifs représenteraient environ 30% des prises en charge des HAD en France contre 10% des prises en charge en hospitalisation conventionnelle. Ces situations sont jugées lourdes par l'ensemble des acteurs professionnels mais aussi des personnes de la société civile qui se sont trouvées confrontées un jour à une telle situation. Issu d'une thèse d'université pour l'obtention d'un doctorat en santé publique, l'objectif de cet ouvrage est triple. Il est de produire un état des lieux de la prise en charge de la fin de vie dans trois HAD françaises en adoptant différents points de vue. Il est d'améliorer la compréhension de ce qui fonde le sentiment de satisfaction ou d'insatisfaction des patients, de leurs proches et des soignants, et d'identifier des outils potentiels de pilotage pour les équipes soignantes. Il est enfin de produire des propositions pour améliorer l'existant d'ores et déjà, indépendamment des moyens financiers disponibles.

The reported research and experience of the many disciplines represented had dealt with one or more of three concerns: 1) a major health problem, such as cardiovascular disease, or an important popUlation at risk, such as the elderly or children or workers; 2) some generic aspect of organization and decision making, including trial and evaluation ofinnovative health strategies; and 3) the methodology of research and analysis in system of health service. The challenge to the conference organizers lay in the eliciting and arranging of experiences in such a way that the health services could be seen as purposeful,living, evolving systems.
