Intra cellular mechanisms maintain cellular viability and provide the means necessary for their specific cellular functions. The interaction between cells is maintained by mechanisms involving extracellular signalling. Such extracellular mechanisms may include special properties of the cell surface which involve immediate cell contact, but may also represent mechanisms which act at a distance and are mediated via special secretions and/or re ceptors. Recent studies on cell-cell contact have tended to stress cell sur face components directly mediating cellular interactions; the extracellular medium as a metabolically active compartment has been rather neglected. However, it represents a vital medium through which cells communicate, being important in, for example, chemotaxis in primitive organisms, and in devel~ment and in the coordination of multiple functions in multi cellular 0 ganisms. It is not surprising, therefore, that a number of mole cular me anisms have developed together with increasing biological complexi during evolution. Two aspects of the extracellular space have received increasing attention in the last few years. First, several macro molecules such as collagen, laminin and fibronectin have been identified as components of an extracellular matrix giving a structural dimension to the extracellular compartment.

A wide variety of immunomodulators are now being studied in terms of treating infectious diseases, as well as malignancy and autoimmune diseases.

Book excerpt: The Workshop series of the "Koningin Wilhelmina Fonds" is a feature of the interest of this Foundation to promote research and education in the field of cancer, aiming to improve prognosis of the cancer patient. For almost a century, surgery has now been the main treatment modality for the cancer patient. During the past 50 years radiation therapy has developed as a second important modality of treatment. However, neither of the two modalities are able to cure the majority of cancer patients, as the disease is so often metastasized at presentation. Cancer chemotherapy is a modality which does have the potential to cure patients with this advanced stage of disease. In recent years the three modalities have been combined more and more, and here is one of the reasons why the prospectives for the cancer patient have improved so much. In addition, certain types of cancers, such as testicular car cinoma, childhood tumors, choriocarcinoma, non Hodgkin lymphomas and others, can now indeed often be cured by chemotherapy alone, even if the disease is advanced. The combination of drugs and the use of new drugs have greatly contributed to this development. There are two main groups of new drugs: analogs and new structures. A great number of analogs of conventional structures have been developed during the past ten years. A typical example of this group is adriamycin, which is an analog of dau nomycin.

Book excerpt: The idea to compile recent results on the ectoenzymes aminopeptidase N/CD13 and dipeptidylpeptidase IV/CD26 arose from the great interest given by readers world-wide to the two proceedings volumes edited by us in 1997 and 2000 (Ansorge and Langner, 1997; Langner and Ansorge, 2000). These volumes contained the presentations at two symposia held in Magdeburg (Germany) in 1996 and 1999 under the title "Cellular peptidases in immune functions and diseases", which was also the name of the Sonderforschungsbereich in Magdeburg, sponsored by the Deutsche Forschungsgemeinschaft between 1995 and 2001. Our groups in Magdeburg and Halle during the last two decades have provided results on these two enzymes in cells of the hematopoietic system that justify a review in an edited monograph like the present one (see the reviews by Kahne et at. , 1999; Lendeckel et at. ,1999; Riemann et at. , 1999). There are, however, many other groups in Europe, US and Japan which made important contributions to this field and particularly in topics improving the understanding of physiological and pathophysiological roles ofAPN/CD13 and DPIV/CD26. Therefore we decided to invite some of them to contribute reviews of their results to this book. Having worked for about 40 years in the field of proteolysis, for both of us to see the development of activities and knowledge from protein chemistry and enzymology to physiology and pathophysiology and even to therapy is very stimulating and fascinating. Of course, this development also reflects the dramatic improvement and refinement of methods.

Book excerpt: The observation in the 1950s that nitrogen mustard and other toxic chemicals could induce antitumor responses in patients with refractory lymphoma initiated a massive search for active chemotherapeutic agents. The initial observations stimulated a search for new chemotherapeutic agents which might have increased antitumor activity with less toxicity for normal tissues. To aid in the search for these new chemicals and to attempt to distinguish among the many toxic chemicals which might be candidates for clinical studies, the National Cancer Institute, the pharmaceutical industry, and the cancer research laboratories of most Western nations developed systems for "screening" drugs for antitumor activity. Perhaps the most extensive screening program was established by the National Cancer Institute (1). This screening program has evolved over the last two decades, an evolution which has been repeatedly reviewed (2-5). Various screening programs in use have examined over 500,000 compounds as potential anticancer agents. From these, there are now approximately forty anticancer drugs in clinical use. The utiliy of these compounds and their toxicities have been reviewed on many occasions. It is now apparent that more active and less toxic anticancer drugs are needed. It is also clear that the current screening programs are identifying compounds with similar levels of activity and with continuing moderate to severe toxicity (6).

Book excerpt: Immunopharmacology as a field of scientific endeavor had its origins more than thirty years ago in the application of antibody-based techniques to assays of hormones and drugs in tissues and body fluids. More recently, the field has been redefined to include a primary focus on the immune system as a target of xenobiotic action. Advances in the field of immunology have made it apparent that the immune system, like other organ systems, declines in its function as a result of aging, viral infections like AIDS, and other immunotoxic influences, giving rise to secondary immunodeficiency. Deficiencyof the immune system in turn leads to infections, autoimmune diseases, and an increased incidence of certain cancers. The notion of treating the failing immune system is relatively new; however, more than a decade of research on cancer and AIDS has created the burgeoning new clinical field of immunotherapy. Immunopharmacology then stands as the preclinical and clinical science of immune manipulation. As such, like its parent field of pharmacology, it includes within its scope basic studies of immune mechanisms as they relate to the pathogenesis of inflammation and immunologic disturbances. As with pharma cology, the perspective is always a therapeutic one. Studies of immune and inflammatory processes emphasize the use of pharmacologic probes and drugs to elucidate the underlying biochemical pharmacology.

It is certainly not the size of their genomes or the individual features of their reproductive cycle, although in toto striking features that are exclusive to the herpesviruses abound. Unquestionably, the pre-eminent feature is the relationship of herpes viruses with their natural hosts. As described in preceding volumes, all herpesviruses seem to be able to colonize and to remain in a latent, nonproductive form for life of their hosts. Once established in the host, the relationship is best described as that of an armed truce. What happens when this truce breaks down or when the host encounters the virus for the first time is the subject of this volume. We have focused primarily on the five human herpesviruses [herpes simplex virus 1 (HSV-l), herpes simplex virus 2 (HSV-2), cytomegalovirus (CMV), varicella zoster virus (VZV) and Epstein-Barr virus (EBV)] because much more is known about them than about any other herpesviruses, and because it is of interest to compare both the diversity of manifestations of infections with human herpesviruses and the spectrum of human responses to these viruses.
