Book excerpt: Early initiation of skin-to-skin contact immediately following birth has significant health benefits for a newborn during its extra-uterine transition. Research studies have detailed how this simple intervention of skin-to-skin contact (SSC), leads to positive outcomes including: mother/infant interaction and bonding, breastfeeding success, thermo regulation, vital sign stabilization, blood glucose regulation, and pain management. Evidence-based guidelines have been developed to improve care practices and optimize the birth experience and outcomes for mothers and their newborns. In current practices, separation of mother and newborn derives from an approach to accomplish tasks, signifying the need for education among nurses in regards to the benefits of immediate SSC (Blomqvist and Nyqvist, 2011). A search of peer-reviewed articles, relevant literature reviews and several qualitative studies revealed a significant amount of evidence to support the idea that more education is needed to prove how and why immediate initiation of SSC after birth is a critical component to postpartum care. The purpose of this proposal is to educate on benefits of SSC, and guide the implementation of early SSC immediately following birth as a standard of practice within Women and Infant Services (WIS). A SSC committee of educators will be developed to conduct research that is current and relevant to practice. This research will then be presented to leaders within WIS. With support and permission, this author is proposing a mandatory four-hour in-service pilot class highlighting the significant benefits associated with SSC (Appendix A). A talk-aloud will follow the in-service, where healthcare providers can share their questions, comments, and concerns regarding the possible changes to practice and policy. Each member of the class will be given a brief survey and questionnaire (Appendix B). This will serve as a tool to evaluate the effectiveness of the teaching, and determining the need for improvements for further teaching. Patient surveys (Appendix C) will evaluate the effectiveness of new-implemented practices and overall patient experience regarding incorporation of SSC. Stakeholders will be presented with the rational for the proposal and the survey results of the three-month trial of this new implementation practice. The Self-Efficacy Theory (SET) supports this proposal method by connecting confidence in the achievement of goals, and behaviors toward accomplishing goals, as well as identifying variables that may affect the outcome (Current Nursing, 2012). The Attachment Theory also supports this proposal as it recognizes the distinct attachment behaviors of an infant and its mother, developing an innate bond. Skin to skin contact promotes physical comfort and verbal interaction on the part of the mother, creating organic ties between a mother and her newborn. This bond imprints an emotional attachment; therefore, physical separation of infant and mother creates emotional, and physiological distress (Kearvell and Grant 2010). Understanding the impact that physical separation causes between a mother and her infant, it is important to promote this essential element into care.

Book excerpt: Early skin to skin contact (SSC) provides physiologic and psychological benefits to mothers and healthy infants. The instinctive interactions between mothers and their newborns begins immediately after birth and is potentiated with early SSC. Early SSC entails positioning the newborn, wearing only a diaper and hat, prone on the mother's bare chest with a warm blanket on top of the infant immediately after birth (Berg and Hung, 2011). However, routine separation after birth is common institutional process in hospitals that results in many detrimental outcomes such as, discouragement of successful breastfeeding, hypothermia, and decreased bonding. Many times separating mothers and infants stems from a task oriented approach to care and a knowledge deficit related the beneficial effects of SSC (Blomqvist and Nyqvist, 2011). The purpose of this project is to implement routine practices of SSC immediately after birth for all healthy newborns at Campbell County Memorial Hospital (CCMH). A presentation to key stakeholders demonstrating supporting research of SSC displayed in an evidence table will gain enthusiasm and support to move this project forward. The development of a task force will help implement this process. The task force will initially develop a business proposal and power point presentation to help organizational leaders see the potential outcomes of this project. A staff survey will be conducted to gain a baseline of staff perception regarding SSC. Next, the task force will develop a staff education plan including a competency check off list, guidelines, and a flow chart to ensure full understanding of the process. The documentation system will be adjusted to include SSC and chart audits will be instituted to evaluate the process. Weekly staff surveys will evaluate barriers to the process. The development of patient surveys will assess and evaluate the mother's perception and experience of the SSC session. As SSC is implemented in the trial phase, adjustments will be made to improve the process. In addition, the marketing department will promote this new service with brochures and advertisements. By implementing the recommended best practice of SSC at CCMH, improved patient outcomes and improved patient satisfaction will result.
