To date, it is recommended to use an improved program for sepsis, including sepsis screening for acutely ill, high-risk patients and standard operating procedures for treatment in hospitals. Sepsis screening tools are designed to promote early identification of sepsis using clinical scores and biomarkers. The combination of clinical scores and biomarkers increases the diagnostic and prognostic accuracy of sepsis, improving patient clinical management. Various clinical variables and tools are used for sepsis screening, such as vital signs, signs of infection, systemic inflammatory response syndrome (SIRS) criteria, quick Sequential Organ Failure Score (q-SOFA) or Sequential Organ Failure Assessment (SOFA) criteria, National Early Warning Score (NEWS), or Modified Early Warning Score (MEWS).

Les effets de son intégration dans la pratique clinique méritent aujourd'hui d'être évalués. Évaluer si l'utilisation systématique du Q-SOFA aux urgences chez tout patient suspect d'infection permet d'améliorer la prise en charge des patients septiques en raccourcissant le délai d'administration de l'antibiothérapie initiale. Nous avons réalisé une étude rétrospective au centre hospitalier de Béthune-Beuvry. Après intégration du Q-SOFA dans le Service des Urgences nous avons comparé les délais avant et après cette modification de service parmi les patients qui avaient développé un choc septique en réanimation dans les 24 premières heures de leur hospitalisation. Le critère de jugement principal (CJP) est le délai d'administration de l'antibiothérapie initiale, les critères de jugement secondaires (CGS) sont la proportion de patient pour laquelle la fréquence respiratoire est noté dans le dossier, la proportion de patient pour laquelle le Q-SOFA est calculable, la durée de séjour au service d'accueil des urgences, la durée de séjour en réanimation, et la mortalité à la sortie de réanimation. Le délai médian concernant le CJP est de 224 minutes IQR [126-351] dans le groupe 1 et de 199 minutes IQR [156-277] dans le groupe 2 avec un p à 0.794 donc non significatif. L'étude ne met pas en évidence de raccourcissement significatif de ce délai, mais d'autres études sont nécessaires pour confirmer ou infirmer ce résultat avec des effectifs plus importants concernant l'échantillon de population étudié. La prise en charge du sepsis est amenée à évoluer avec le développement de nouveaux tests diagnostic et pronostic, ce qui pourrait permettre d'améliorer le pronostic de cette pathologie grave.

Book excerpt: The objective structured clinical examination (OSCE) has become widely employed in intensive care medicine (ICM) exams such as the UK Final Fellowship of Intensive Care Medicine (FFICM), the European Diploma of Intensive Care (EDIC) exam, or the Australia and New Zealand Fellowship of the College of Intensive Care Medicine (CICM). This book is not only an essential tool for the rehearsal of OSCEs in preparation for these exams, but also for American and Indian exams, and any other exams within the field of intensive care medicine. The OSCE is a style of examination that requires the candidate to present specific information in a short space of time matched to a prescriptive mark scheme. This book allows candidates preparing for an ICM OSCE to format their knowledge for this type of examination whilst practising and rehearsing against time. The 60 OSCE stations provide a broad array of topics from past exams ranging across the training ICM curriculums.
