Book excerpt: Objectif : Les patients atteints de rhumatisme inflammatoire chronique (RIC) peuvent présenter une sensibilisation centrale (SC), de par la chronicité de leur douleur. L'inventaire de sensibilisation centrale (ISC) est un questionnaire validé, qui, au-delà de 40/100, évoque une SC. Nous évaluons le caractère prédictif de réponse thérapeutique d'un ISC positif à un premier traitement par anti-TNF dans une cohorte de 100 RIC à 3 mois du début du traitement. Méthode : étude observationnelle, prospective, contrôlée, bi-centrique incluant 100 patients (présentant un RIC actif, naïf de biothérapie) et 100 témoins. Évaluation de l'ISC avant et à 3 mois de l'initiation de l'anti-TNF. Résultats : un ISC positif est retrouvé chez 60% des patients contre 9% des témoins avant initiation de l'anti-TNF. A 3 mois, 47 patients (62,7%) répondent au critère de bonne réponse thérapeutique défini par une amélioration du DAS 28 > 1,2 ou de l'ASDAS >1,1. Le calcul de l'odd ratio évaluant la relation entre un ISC > 40/100 et l'efficacité du traitement mesurée sur l'amélioration du DAS 28 ou de l'ASDAS retrouve une valeur significative de 0,28 (IC 95% 0,098 - 0,83). De plus, l'âge et l'activité du rhumatisme sont des facteurs prédictifs respectivement de bonne et mauvaise réponse thérapeutique. Enfin, l'ISC est corrélé à l'ensemble des questionnaires algofonctionnels mais également à la CPM. Conclusion : Chez les patients atteints de RIC, un ISC supérieur à 40/100 est prédictif de moins bonne réponse thérapeutique à 3 mois de l'introduction d'un traitement par anti-TNF. La sensibilisation centrale gagne donc à être dépistée et prise en charge afin d'améliorer la qualité de vie des patients.
