Book excerpt: Alcoholism is one of the prime determinants of the behaviors of a dysfunctional family system. The purpose of this study was to determine the effects of alcoholism on self-esteem, locus of control and role designations. The information obtained in this study will be used for guiding differentiated interventions that will assist in keeping the alcoholic family unit intact and improving its functioning. The study investigated: (a) the set of factors that mediate self-esteem, locus of control and family of origin level of functioning, (b) differences between adult Children of Alcoholics (ACOAs) and Adult Children of Non-Alcoholics (ACONAs), and (c) differences among ACOAs on self-esteem, locus of control, and role designations. The participants were members of a church in the northeastern region of the United States which offers counseling services and educational programs to its congregation. The participants are 88 African American adults aged 18-70 years who completed five survey instruments that provided information on parental drinking patterns and experiences, self-esteem, locus of control, family of origin level of functioning and demographics including role designations, gender, marital status, ethnicity, and specific information on alcohol use. The results indicated no significant difference between ACOAs and ACONAs on self-esteem and level of functioning in the family of origin. However, a significant difference was found between ACOAs and ACONAs on locus of control. There was no significant difference between ACOAs and ACONAs family functioning on self esteem and locus of control. There was no significant difference between the 'most healthy' ACOAs and the 'least healthy' ACOAs on self-esteem and locus of control. There was no significant difference between female and male ACOAs on self-esteem and locus of control; no difference between Internal and External ACOAs on self-esteem when the father was alcoholic; and no difference between hero ACOAs and scapegoat ACOAs on self-esteem and locus of control. These findings indicate that parental alcoholism mediates locus of control orientation only for. In addition, family of origin functioning is not a factor in the self-esteem or locus of control of ACOAs. Furthermore, the family of origin level of functioning does not impact the self-esteem and locus of control of ACOAs. It seems that some factor aside from family functioning is mediating self-esteem in ACOAs. The ACOAs are acquiring adaptive behaviors, high self-esteem and internal locus of control for managing their lives despite being reared in an alcoholic home environment and much the same as ACONAs. These adaptive behaviors are mediated by parental involvement, personality, and external social supports. Future investigations should examine parental behaviors, patterns and involvement with the child to determine their impact on locus of control and an investigation of other potential dysfunctional behaviors should also be considered Finally, an examination of the constructs of family or origin functioning should be considered to dispel any doubts about it as a predictor of psychological distress in children reared in an alcoholic home.

Book excerpt: The focus of this investigation was to examine empirically how women who marry alcoholics and how women who are daughters of alcoholic parents differ from other women in terms of their families of origin and emotional style. Subjects in this investigation were 120 caucasian women who were non-alcoholic, as measured by the Michigan Alcohol Screening Test, and who were either married or had been married at least once to a spouse who as determined to be alcoholic or non-alcoholic, as measured by the Michigan Alcohol Screening Test for Significant Others. The Children of Alcoholics Screening Test was also administered to determine whether the subject was from a alcoholic or non-alcoholic family of origin. The Family Environment Scale (FES) was administered retrospectively to determine the characteristics of each subjects' family of origin. The Test of Emotional Style (TES) examined perceptions of the subjects' comfort with their emotions as adults. Results on severeal scales of the FES indicated that daughters of alcoholics perceived themselves to have had significantly less satisfactory relationships in their families of origin, (i.e. experienced less support for their personal growth and more family chaos). As expected, wives of alcoholics revealed a pattern within their families of origin similar to that of adult daughters of alcoholics. Wives of non-alcoholics were similar to adult daughters of non-alcoholics. Several of the FES scales, especially those on the Personal Growth Dimension, indicated that the families of origin of adult daughters of alcoholics who are also wives of alcoholics were the most dysfunctional, when compared to all other subject groups. On the TES, the finding of greatest magnitude was that adult daughters of alcoholics who married alcoholics were least satisfied with their emotional life whereas their counterparts who marrie a non-alcoholic were most satisfied with their emotionality. Adult daughters of alcoholics who were also wives of non-alcoholics scored lowest on the Moral-Religious subscale of the Family Environment Scale, and perceived their families of origin to have been the most chaotic in comparison with all other groups. It is though that this pattern may have provided adult daughters of alcoholics who married non-alcoholics with a family of origin that facilitated their capacity to seek appropriate role-models outside of the family. Implications of these findings for future research are discussed.

On an individual basis, the combination of sodally approved drinking and the stigma assigned to the chronic alcoholic results in individuals blinding themselves to the existence of the problem in family, friend, and self until it has reached such an advanced or obvious degree that denial is no longer possible. There is the third kind of denial, exemplified by therapeutic de spair, which proclaims thatgaps in knowledge of the cause of alco holism are so great and failures to treat alcoholics successfully so dra matic that there is no assurance that efforts will lead to a positive outcome. This denial is perhaps the most troublesome because it re flects an attitude of therapeutic helplessness. It discourages families from seeking help, and it reinfOlces the tendency of physidans and other human-services workers to overlook the presence of alcoholism as they treat its physical, sodal, and economic consequences. Denial frequently surrounds those who treat alcoholics with an aura of hope lessness, which itself is a negative therapeutic force.

Despite its high prevalence and numerous negative consequences, AUD remains undertreated. In fact, fewer than 1 in 10 individuals in the United States with a 12-month diagnosis of AUD receive any treatment. Nevertheless, effective and evidence-based interventions are available, and treatment is associated with reductions in the risk of relapse and AUD-associated mortality. The American Psychiatric Association Practice Guideline for the Pharmacological Treatment of Patients With Alcohol Use Disorder seeks to reduce these substantial psychosocial and public health consequences of AUD for millions of affected individuals. The guideline focuses specifically on evidence-based pharmacological treatments for AUD in outpatient settings and includes additional information on assessment and treatment planning, which are an integral part of using pharmacotherapy to treat AUD. In addition to reviewing the available evidence on the use of AUD pharmacotherapy, the guideline offers clear, concise, and actionable recommendation statements, each of which is given a rating that reflects the level of confidence that potential benefits of an intervention outweigh potential harms.
