Available in PDF, EPUB and Kindle. Book excerpt: This book presents a comprehensive study of scrotal ultrasound, helping readers cope with the growing number of pathology pictures revealed by accurate ultrasound examinations, and highlighting the novel applications of contrast-enhanced ultrasonography and elastography. This unique reference guide to scrotal ultrasonography draws on the accumulated expertise of the Experimental Medicine Department at “Sapienza” University, where the andrological ultrasonography unit has performed over 10,000 testicular ultrasound examinations for various conditions and explored experimental new imaging techniques. This core experience has been enriched by insightful contributions from several international experts to form one of the most comprehensive collections of ultrasound images, many in full color, of scrotal pathology in the world. The book’s emphasis on functional interpretation of the images, supplemented by clinical data, make it a unique tool for clinical management. This approach is intended to increasingly familiarize clinicians with the potentials of ultrasonography, from the basics to the most advanced approaches, so as to encourage them to incorporate this examination as a central component of the diagnostic pathway

Book excerpt: The decade that has passed since publication of the second edition of this textbook has not only witnessed a tremendous increase in knowledge within the ? eld of and- logy, but also seen the ? eld itself achieve a newfound status within the medical p- fession. Knowledge and status have been of mutual bene? t to the ? eld and the growing critical mass of diagnostic and therapeutic possibilities have caused andrology to be recognized as a medical subspecialty in some countries such as Germany, Poland, and Estonia. The European Academy of Andrology (EAA) served as a pacemaker for this development and continues to strive for establishment of andrology as a clinical ? eld. Well-designed curricula and qualifying examinations have contributed to the of? cial recognition of andrology as a speciality. This recognition of the ? eld helps patients with andrological problems to ? nd the specialist they seek. This textbook summarizes the current state of knowledge in the ? eld of andrology. It is a source of knowledge to all those who are or want to become andrologists. In addition, as andrology is clearly an interdisciplinary ? eld, this book may serve as a compendium and source of reference for all those physicians and biologists active in neighboring areas, who want to obtain an overview of andrology and who require information on special problems. The extensive references are timely and up to date.

Introductory chapters consider imaging instrumentation, clinical evaluation, and clinical and imaging anatomy. The full range of disorders is then discussed in individual chapters organized according to clinical presentation. All clinical and imaging aspects are covered in depth, with full description of symptoms and explanation of the value of different clinical tests and imaging modalities. In addition, underlying histopathological features are presented and correlated with imaging features in order to clarify their pathological basis. For each disorder, therapeutic strategies are discussed and appraised. Adults and children are considered separately whenever necessary, bearing in mind that they often present essentially different scrotal pathology. The many images are all of high quality and were obtained using high-end equipment.

Available in PDF, EPUB and Kindle. Book excerpt: Testicular cancer (TC) is the most common cancer in males aged 20-40 years, with a worldwide incidence of 7.5 per 100,000, but the rates vary considerably between countries and ethnic groups and there is evidence also for an increasing incidence in last decades. About 95% of all TCs are represented by testicular germ cell tumors (TGCTs), which include seminoma and non-seminoma histological types. It is generally assumed that the development of TGCT is under endocrine control. In particular, unbalanced androgen/estrogen levels and/or activity are believed to represent the key events for TGCT development and progression. Furthermore, recent evidence has suggested genetic association of TGCT with variations in genes involved in hypothalamic-pituitary-testicular axis and steroidogenic enzymes. This recent evidence expands the current knowledge on the role of genetic contribution in testicular cancer susceptibility, and supports the hypothesis that variations in hormone metabolism genes might change the hormonal environment implicated in testicular carcinogenesis. Therefore, hormonal carcinogenesis is an important and controversial area of current research in TGCT, and further attention is given to genetic factors influencing hormone-related cancer risk. The genetic component to TGCT is in general strong. In fact, although environmental factors clearly contribute to TGCT development (and probably to its increasing incidence in some geographical areas), the proportion of TGCT susceptibility accounted for by the genetic effects is estimated at 25%. TGCT has high familial risks compared with most other cancer types that are generally no more than two-fold: brothers of individuals with TGCT have an 8- to 12-fold increased risk of disease, and sons of affected individuals have a 4- to 6-fold increased risk. Despite this strong familial relative risk, early results from linkage studies identified a limited relationship with genetic factors, suggesting that TGCT is a genetically complex trait. However, more recently, four genome-wide association studies (GWAS) from the UK and USA have reported association of TGCTs with six new loci (KITLG, SPRY4, BAK1, DMRT1, TERT, and ATF7IP). The strongest association for TGCT susceptibility was found for SNPs in KITLG (ligand for the membrane-bound receptor tyrosine kinase KIT) gene with a greater than 2.5-fold increased risk of disease per major allele, which is the highest reported for any cancer to date. These studies are being now replicated by other researches and attention is given to the relationship between these genetic variations, TGCT risk and frequently associated anomalies of the reproductive tract, such as cryptorchidism and infertility. Finally, over the past few decades, TCGT research has focused also on external environmental causes acting mainly as endocrine disrupters of androgen and oestrogen pathways, even during the foetal development of the testis. It is well known that the testicular dysgenesis syndrome (TDS) hypothesis, proposed ten years ago, suggests that disturbed testicular development in fetal life may result in one or more of four disorders postnatally, named cryptorchidism, hypospadias, poor semen quality, and TGCT. These four disorders are therefore considered as one clinical entity and are linked together by epidemiological and pathophysiological relations. The relative contribution of genetics and environment in TGCT development, and the interactions between endocrine disruptors and variations in genes involved in hormonal carcinogenesis is therefore another interesting area of research.

Book excerpt: This book provides the latest recommendations for ultrasound examination of the entire urogenital system, particularly in the male. The coverage encompasses the role of ultrasound in imaging of disorders of the kidneys, urinary tract, prostate, seminal vesicles, bladder, testes, and penis, including male infertility disorders. In addition, detailed consideration is given to intraoperative and interventional ultrasound and recently developed ultrasound techniques. Each chapter defines the purpose of and indications for ultrasound, identifies its benefits and limitations, specifies the technological standards for devices, outlines performance of the investigation, establishes the expected accuracy for differential diagnosis, and indicates the reporting method. Most of the recommendations are based on review of the literature, on previous recommendations, and on the opinions of the experts of the Imaging Working Group of the Italian Society of Urology (SIU) and the Italian Society of Ultrasound in Urology, Andrology, and Nephrology (SIEUN). The book will be of value for all physicians involved in the first-line evaluation of diseases of the renal/urinary system and male genital disorders.

By showing the differential diagnosis of each lesion, it offers clinicians vital support with diagnosis and treatment. The book is divided into seven main parts: genetic and developmental pathology of the testis; infertility; vascular pathology of the testis; inflammatory pathology; pathology of the rete testis and epididymis; pathology of the vaginal tunic and paratesticular structures; and a final part dealing with miscellanea. Each chapter includes carefully selected figures and a variety of diagrams highlighting the main characteristics of a specific lesion to facilitate its diagnosis.Based on the authors’ experience with hundreds of biopsies, surgical specimens and autopsies, the book presents material that has been gathered over the past 40 years, providing an essential tool for pathologists, urologists, andrologists and pediatricians who face diagnostic problems.
