Book excerpt: To investigate the role of nitric oxide (NO) in the treatment of diabetes with American ginseng extract (AG), a double-blind, placebo-controlled study was conducted in 24 type 2 diabetic subjects. They were randomized to take either AG 2.25g/day or placebo for 8 weeks, then crossover with at least 4 weeks washout. Blood pressure was determined and fasting blood was taken to assess NO, glycemic parameters and lipid peroxidation. During AG treatment, NO increased significantly compared to placebo (P = 0.031). The change was negatively correlated to HbA1c (R = -0.42, P = 0.043), which decreased (P = 0.026) following AG. Fasting blood glucose, lipid peroxidation and blood pressure were improved with AG (P

Book excerpt: Type 2 diabetes (T2D) is a complex metabolic disorder characterized by impaired glucose metabolism and pancreatic β-cell dysfunction. No effective treatments are available for T2D, although there have been many developments in the therapeutic arena. Nitric oxide (NO) is an endocrine agent with multiple and important biological roles in most mammalian tissues. NO has emerged as a central regulator of energy metabolism and body composition. NO bioavailability is decreased in T2D. Several of the pharmaceuticals used in T2D affect the NO system and perhaps even more so by the drugs we use to treat diabetic cardiovascular complications. Experimental works in animal models of T2D show promising results with interventions aimed to increase NO signaling. However, translation into human studies has so far been less successful, but more large-scale prolonged studies are clearly needed to understand its role. This book is a collection of reviews that deal with the role of nitric oxide in type 2 diabetes, providing a unique overview of NO signaling, and pointing out key areas for more detailed research. Since the preliminary evidence is suggestive of complementary actions of American (AG) and Korean Red Ginseng (KRG) in improving glycemia, this project was designed to investigate the therapeutic potential of AG and KRG in combination.Following a randomized, double-blind, placebo-controlled parallel design in a population with diabetes at two centres, the combined use of AG and KRG for 12 weeks was safe, but did not significantly affect glycemic control, blood lipids or blood pressure. However, there was a trend toward lower glycated hemoglobin by 0.7% (p=0.1) and office systolic blood pressure by 5 mm Hg (p=0.052) compared to placebo. These findings encourage further investigation of the mechanism and roles of AG, KRG and their effective components. They also highlight limitations in ginseng research and the need to impose strict regulations to facilitate its standardization.

Suboptimal risk management supports evidence-based adjunctive approaches to standard therapy to help attain treatment targets. Ginseng ranks among most highly used herbal supplement with equivocal clinical evidence. Korean red ginseng (KRG) and American ginseng (AG) may have cardio-metabolic benefits relating to blood pressure (BP) and vascular function in addition to regulating hyperglycemia. In particular, ginsenoside Rg3, present in KRG, was identified to have BP lowering potential. The objective of this thesis was to asses: 1) the effect of single dose ginsenoside Rg3-enriched KRG administration on hemodynamic indices in healthy individuals in a pilot randomized controlled trial (RCT) and subsequently 2) the effect of 12-week co-administration of Rg3-enriched KRG and AG intervention on systolic BP (primary outcome) in addition to metabolic risk factors and safety monitoring in individuals with type 2 diabetes and hypertension in a RCT, and 3) the impact of the combined intervention on related vascular function markers and pulse pressure wave components within the 12-week study. Results: Single dose Rg3-enriched KRG administration significantly improved the hemodynamic profile 3h-post intake including augmentation index, central systolic, diastolic and mean arterial BP. A 12-week combined Rg3-KRG and AG intervention resulted in a reduction of 24-h-systolic BP (-3.98±2.0 mmHg, p=0.04) relative to control. Additionally, HbA1c (-0.35±0.2%, p=0.02) and lipids, including non-HDL-C (-0.54±0.2 mmol/l, p=0.01) and triglycerides (-0.43±0.2 mmol/l, p=0.01) were reduced, in absence of adverse effects. Central systolic BP reduction (-4.7±2.2 mmHg, p=0.04) was paralleled by improvement in systolic waveform components, but no effect on measured markers of endothelial function, arterial stiffness or pressure augmentation. Use of standardized ginseng combination may serve as a viable natural health product adjunct to pharmacotherapy for cardiometabolic management of type 2 diabetes.
