Book excerpt: L'hôpital de jour (HdJ) de détection de la fragilité et de prévention de la dépendance permet de détecter les patients âgés fragiles et de mettre en place des mesures préventives personnalisées pour lutter contre la perte d'autonomie et retarder le processus d'entrée dans la dépendance. La revue de médication est réalisée quotidiennement à l'hôpital de jour par le pharmacien clinicien en concertation avec le gériatre. Elle permet de détecter les prescriptions potentiellement inappropriées (PPI) selon des critères explicites et implicites, puis de proposer aux médecins traitants une optimisation de la prescription selon les types de problèmes détectés. L'objectif principal de cette étude était de montrer l'impact de cette revue sur l'optimisation des prescriptions par le médecin traitant. Le taux d'acceptation des propositions a été déterminé 3 mois après avoir vu le patient en contactant les pharmaciens d'officine et les médecins traitants. 191 patients ont été inclus. 69% d'entre eux présentaient au moins une PPI (0-9). Parmi les 141 patients ayant eu au moins une proposition d'optimisation, 1 patient sur 2 avait au moins 57 % des interventions proposées acceptées trois mois après le passage à l'HdJ. Les optimisations concernaient le plus souvent l'arrêt ou le remplacement d'un médicament dont la balance bénéfice/risque est défavorable (56.5% des patients), et l'arrêt d'un médicament non indiqué (26%). Lorsque les médecins n'avaient pas modifié la prescription, cela concernait le plus souvent les médicaments de la classe cardiovasculaire (20%), neurologique (17.7%) et du système digestif (12%). Les arguments les plus fréquents étaient qu'ils ne souhaitaient pas modifier la prescription du spécialiste (15% des patients), que le médicament était bien indiqué (11%), ou que la tentative d'arrêt s'était soldée par un échec (9%). Notre étude montre que de nombreux patients avaient une prescription potentiellement inappropriée. Cependant, dans plus de la moitié des cas, les médecins généralistes acceptent les recommandations préconisées pour optimiser les prescriptions. L'hospitalisation des patients à la plateforme fragilité semble donc être un dispositif efficient pour optimiser la prescription médicamenteuse en soins primaires.
