The first published study on binaural interference twenty-five years ago demonstrated that some individuals, particularly older individuals, perform more poorly with two hearing aids than with one and/or more poorly with binaural than monaural stimulation on electrophysiologic as well as behavioral measures. Binaural interference is relevant to every audiologist because it impacts the successful use of binaural hearing aids and may explain communicative difficulty in noise or other challenging listening situations in persons with normal-hearing sensitivity as well as persons with hearing loss. This exciting new book written by two highly respected audiologists first traces the history of its study by researchers, then reviews the evidence, both direct and indirect, supporting its reality. This is followed by a discussion of the possible causes of the phenomenon and in-depth analysis of illustrative cases. The authors outline a systematic approach to the clinical detection, evaluation and amelioration of individuals who exhibit binaural interference. Suggestions are furnished on improved techniques for evaluation of the binaural advantage in general and on sensitized detection of the disorder in particular. The book ends with recommendations for future directions. Given the adverse impact of binaural interference on auditory function and its occurrence in a significant subset of the population with hearing loss, as well as in some individuals with normal-hearing sensitivity, research on binaural interference only recently has begun to flourish, and adaptation of audiologic clinical practice to identify, assess, and manage individuals with binaural interference has yet to become widespread. The authors intend for the book to provide impetus for pursuing further research and to encourage audiologists to explore the possibility of binaural interference when patient complaints suggest it and when performing audiologic evaluations. The book is intended for practicing clinical audiologists, audiology students, and hearing scientists.

Book excerpt: Purpose: This study investigated the differences in brain functional activation patterns and in white matter microstructure integrity underlying atypical left ear advantage (LEA) for speech-related stimuli in dichotic listening. While a finding of atypical LEA for speech-related stimuli is often taken as an indication of mixed/reversed hemispheric language dominance and as a marker for auditory processing disorder (APD), validation studies using gold standard techniques failed to predict right hemispheric dominance from LEA. Furthermore, the interpretation made by clinical audiologists has never been tested using objective techniques of evoked potentials or imaging studies It is indeterminate whether a sensory processing deficit such as APD or other supramodal factors may underlie the finding of atypical LEA. Design: Functional magnetic resonance imaging (fMRI) and diffusion tensor imaging (DTI) data were acquired from a cohort of 12 children with auditory processing complaints, manifesting an atypical LEA and 12 typically developing children with right ear advantage (REA), aged 7 to 14 years. Significant clusters showing between-group activation differences (fMRI) and diffusivity measures (i.e., fractional anisotropy, mean diffusivity, radial and axial diffusivity) were computed. Results: fMRI results indicated that differences in brain activation patterns were due to attenuated deactivation of the default mode network (DMN) and increased activation in the dorsal anterior cingulate cortex (dACC) in the LEA compared to the REA group. DTI results indicated altered frontal white matter microstructure, reflected by decreased fractional anisotropy in frontal multifocal white matter regions, specifically in prefrontal cortex bilaterally and left anterior cingulate white matter. Furthermore, results revealed increased mean diffusivity in the left sublenticular part of the internal capsule in the LEA compared to the REA group. Conclusion: Collectively, our findings implicate both sensory deficit (increased MD in the sublenticular part of internal capsule) and supramodal factors (attenuated suppression of the default mode network and altered white matter microstructure in regions involved in attention and cognitive control) to underlie the differences in brain activation patterns and structural connectivity seen between the groups.

At the functional level, however, there are striking differences between the left and right hemispheres. Although popular writings attribute language abilities to the left hemisphere and spatial abilities to the right, differences in hemispheric function appear to be more subtle. According to Ivry and Robertson, asymmetries over a wide range of perceptual tasks reflect a difference in strength rather than kind, with both hemispheres contributing to the performance of complex tasks, whether linguistic or spatial. After an historical introduction, the authors offer a cognitive neuroscience perspective on hemispheric specialization in perception. They propose that the two hemispheres differ in how they filter task-relevant sensory information. Building on the idea that the hemispheres construct asymmetric representations, the hypothesis provides a novel account of many laterality effects. A notable feature of the authors' work is their attempt to incorporate hemispheric specialization in vision, audition, music, and language within a common framework. In support of their theory, they review studies involving both healthy and neurologically impaired individuals. They also provide a series of simulations to demonstrate the underlying computational principles of their theory. Their work thus describes both the cognitive and neurological architecture of hemispheric asymmetries in perception.

The resistance to the notion of RH language processing persists despite the fact that even the earliest observers of Left Hemisphere (LH) language specialization posited some role for the RH in language processing, and evidence attesting to various RH language processes has steadily accrued for more than 30 years. In this volume, chapters pertain to a wide, but by no means, exhaustive set of language comprehension processes for which RH contributions have been demonstrated. The sections are organized around these processes, beginning with initial decoding of written or spoken input, proceeding through semantic processing of single words and sentences, up to comprehension of more complex discourse, as well as problem solving. The chapters assembled here should begin to melt this resistance to evidence of RH language processing. This volume's main goal is to compile evidence about RH language function from a scattered literature.
