Les médecins sont mis sous pression, et sont accablés de recommandations de l'Assurance maladie afin d'accroître leur vente. Objectif: Évaluer le ressenti et le vécu des médecins face à l'arrivée des génériques. Méthode: Étude qualitative par groups focus réunissant 30 médecins généralistes. Résultats : On retrouve une augmentation de la charge de travail liée aux explications itératives des médecins sur les génériques. Ils doutent de la qualité, de la sécurité d'emploi des génériques et des réelles économies réalisées en les prescrivant. Le changement des galéniques, la multitude de marques sont des facteurs de risque d'erreur d'observance surtout dans la population âgée. Leur prescription entraîne une relation médecin/malade et ïftéttedfl/pharmacien complexe, surtout depuis le « non substituable » manuscrit. L'arrivée des média» Éôfflïîie 3ème acteur dans la relation médecin/malade, remet en cause la crédibilité du médecin1. La ROSP rte Semble pas les avoir influencé sur leur prescription de génériques mais soulève la question de conflit d'intérêt. Conclusion : On retient u» sentiment évolutif face à l'emploi des génériques. La prescription des génériques retient de ffémbreuses questions comme la qualité et la sécurité de leur emploi. Leur prescription est devenue chronophage et donne lieu à des conflits avec certains patients. Le « non substituable » manuscrit est vécu comme une véritable punition. Une galénique identique, une délivrance de la même1 marque de générique à chaque passage chez le pharmacien éviteraient certaines errçuré. Une meilleure communication entre médecins et pharmaciens faciliterait l'efficience médicale dans l'intérêt du patient. La prescription en DCI éviterait totrte? polémique et Hçonmt entre médecin/malade, malade/pharmacien, médecin/pfôlrôfràcieh, et médecin-Assuran'cë maladie.
