The major findings and trends presented identify areas where progress has been made in adopting and strengthening the written policies, as well as opportunities for improvement. New to this report are data relating to the reporting and assessment provisions required by the Healthy, Hunger-Free Kids Act. These data are especially relevant to USDA's (U.S. Department of Agriculture's) proposed rule for competitive foods and beverages, which was released in February 2013, and to the forthcoming proposed rule from USDA related to wellness policies. They also may help inform USDA's efforts to provide technical assistance to school districts regarding their wellness policies, which is required by the Healthy, Hunger-Free Kids Act. In addition, this report helps inform future policies for preventing childhood obesity and will be useful to advocates and state and local officials seeking to create a healthier school environment. This report highlights progress that has been made to implement, strengthen, and/or increase the comprehensiveness of the required wellness policy elements between school years 2006-07 and 2010-11. Consistent with prior reports, the data presented in this paper represent the percentage of students enrolled in public school districts nationwide. (Contains 4 figures and 4 tables.).

Book excerpt: Local school wellness policies (i.e., wellness policies) provide an opportunity to create and support a healthy school environment, promote student health, and reduce childhood obesity. Because they are required for all school districts participating in the federal Child Nutrition Programs including the National School Lunch Program and the School Breakfast Program, millions of children can be reached through implementation of these policies which focus on creating supportive school nutrition and physical activity environments. Research has documented that although almost all districts have adopted a wellness policy, they lack specificity related to competitive foods as well as requirements for implementation and compliance. The Child Nutrition and WIC Reauthorization Act of 2004, and more recently the Healthy, Hunger-Free Kids Act of 2010, required that school districts participating in the federal Child Nutrition Programs adopt, implement, and most recently, report on local school wellness policies. The following sections of this report highlight areas where policy opportunities exist, as well as areas where policies are well-established relative to the following wellness policy components: (1) nutrition education and promotion; (2) standards for competitive foods and beverages; (3) nutrition standards for school meals; (4) PA outside of physical education (PE); (5) PE; (6) stakeholder involvement; and (7) wellness policy monitoring, evaluation, and reporting. This brief summarizes the range of policy actions taken by public school districts from the 2011-2012 school year, from the Bridging the Gap (BTG) study. All policies were collected and coded by BTG researchers using a standardized method based on evidence based guidelines and recommendations from expert organizations and agencies. Schools are uniquely positioned to positively affect childhood obesity. School districts are required to have a school wellness policy. In 2016, the most recent federal mandate regarding school wellness policies strengthened the communication, implementation, transparency and evaluation component requirements. Previous research has conveyed the need to study implementation in addition to school wellness policy quality. Purpose The purpose of the study was to describe the quality (strength and comprehensiveness) of school wellness policies and implementation of those policies in San Diego county school districts and to determine if there is an association between them. Additionally, this study explored if a relationship exists between school wellness policies, implementation and district characteristics (total enrollment, percentage of students eligible for free and reduced priced meals and ethnicity). Methods This study utilized a cross sectional design. The WellSAT 2.0 was used to assess the quality of school wellness policies. A novel tool was created to analyze implementation of policies based on evidence from school district websites, and was modeled after the WellSAT 2.0. All school district characteristic data were collected from Ed-Data.org for the school year 2016-2017. Results School Wellness Policy total comprehensiveness score was 56.2%, while total strength was 30.4%. Implementation total comprehensiveness was 46.3%, while total strength was 25.5%. No significant relationship was found between the quality of school wellness policies and implementation or between policy quality and various district demographic characteristics. A significant relationship was found between implementation scores and total enrollment. School districts with a dedicated wellness coordinator had higher average policy and implementation scores than districts without. Conclusion Study results will help guide improvements to school wellness policies and implementation transparency in San Diego county by providing an assessment of all San Diego county school districts. Findings suggest that policy and implementation may not be effected by school district demographic characteristics, suggesting equal wellness efforts across diverse districts. Further efforts are needed to develop federal and state level funding and accountability for school district wellness policies and implementation.

In recognition of this opportunity, each school participating in federal nutrition programs is required by federal law to enact a local school wellness policy. This study assessed the strength and comprehensiveness of 143 school wellness policies in a stratified, random sample of Minnesota public school districts, examining also relationships between school policy quality and district-level demographic variables. Overall, policies scored low in strength and moderate in comprehensiveness. Scores related to policy evaluation were the strongest and scores related to USDA school meal program standards were the most comprehensive, while scores related to physical education/activity were the weakest and least comprehensive. Higher percentages of students eligible for free and reduced-price meals in school districts predicted higher school wellness strength and comprehensive scores. No significant relationships were found between policy total strength and comprehensiveness scores and geographic locale, minority enrollment, or total student enrollment.

Book excerpt: School district wellness policies (SDWP) may provide guidance to schools seeking to improve and maintain healthy learning environments throughout the United States. State and federal legislation and regulations influence the creation, maintenance, and revision of local SDWP. To examine the strength and comprehensiveness of local SDWP content categories, the changes in those characteristics over time, and the associations between changes in SDWP over time and concurrent changes to state and federal legislation and regulation. An observational sample of 19 SDWPs that were in effect over three periods between 2006 - 2014 within a single county of Washington State were scored using the Wellness School Assessment Tool (WellSAT). Content areas of strength and comprehensiveness were compared across time periods. Policy events were divided into three time periods: 2004-2006, 2006- 2010, and 2011-2014. All districts released a SDWP within the 2004-2006 time period when state and federal legislation required the adoption of the policies; in general, more districts updated policies between 2011 and 2014 when changes were required and policies were monitored by state education agencies than in the previous period when policy updates were suggested but not required. SDWP often did not meet recommendations for optimal school health. Because they influence the development and implementation of local school district wellness policies, changes to federal and state policies and practices offer opportunities to improve the daily environments of school children. This study demonstrates that clear and definitive legislative language is required to encourage positive change in SDWP.

Having coordinated the revision of a wellness policy at a local school district, the researcher observed the transition from a template-based to a locally developed policy. In this thesis, stakeholder participation, successes, and challenges of the revision process were analyzed; the quality of the policies were quantitatively compared using standardized tools WellSAT-96 and WellSAT; and an assessment plan was developed for the district. Challenges included disagreement among stakeholders and limiting policy focus, while successes included increased awareness and creation of an ongoing committee. Quantitative analysis of the policies revealed that the revised policy is of higher quality than the original in terms of language strength and comprehensiveness of goals. The assessment plan includes an annual survey of school principals, measurement of indicators, a list of wellness activities, and a report comparing the model policy to the revised policy. Recommendations were made regarding future policy revision. The intent of this work was to provide support to a local district implementing its wellness policy, as well as to provide a useful resource to other districts undergoing policy revision.

Book excerpt: The overarching objective of the research included in this dissertation was to identify program models that support healthy behaviors in school-age children. Special emphasis was given to models and methods that focus on the sustainability of community-based health and wellness programs. Chapter I of this dissertation provides a review of literature that explores the prevalence, definition, consequences, and potential causes of childhood overweight and obesity as well as potential methods, theories, and approaches designed to help ameliorate the problem. This review recognizes obesity as a complex issue and presents a variety of approaches that requires coordinated, multifaceted and collaborative approaches that target not only the individual, but also the whole community. In addition to providing a comprehensive review of behavior change theories and examples of their application in the school setting, this review concludes with a proposed solution to a secondary issue: the long-term sustainability of school-based health and nutrition programs and their outcomes. Chapters II provides a Statement of Purpose for the research discussed in Chapters III through Chapter V. Chapter III describes a study conducted in collaboration with the California Department of Education. The purpose of this study was to develop, implement, and assess the impact of a statewide professional development program designed to equip school nutrition directors with the knowledge and skills needed to cost-effectively implement federal health and wellness initiatives and policies at the local level. The professional development program was entitled, the "Stepping Up To the Challenge: Creating a Healthy School Environment" and was a unique partnership among state education agency staff, university researchers, and practitioners. Post-professional development questionnaire identified key "insights, inspirations, and strategies" as (a) marketing of school foods pro- gram, (b) expansion of salad bars, and (c) collaboration with community partners. A six-month follow-up questionnaire identified that 86% of participants made progress toward implementing at least one of their five insights, inspirations, and strategies in their school districts. Most common areas that were implemented were marketing and branding (32%), revamping salad bars (18%), and motivating staff (16%). School and Community Actions for Nutrition survey analysis showed a significant increase in the use of marketing methods in school nutrition programs from baseline to six-month post-program implementation (p = .024). The nutritional impact of several of the techniques presented in this program were tested in a farm to school (F2S) project funded by a specialty block grant program sponsored by the California Department of Food and Agriculture described in Chapter IV. Chapter IV describes a study designed to evaluate the impact of F2S programming on dietary behavior outcomes, specifically fruit and vegetable preferences and consumption patterns, among school aged children attending schools in three distinct California school districts. To ensure that the intervention methods were appropriate for each unique school community, the intervention plan was designed in conjunction with school nutrition directors. As an outcome of this study, the surrounding community benefited through the establishment of a sustainable relationship between local farmers and the school nutrition directors. Data collected from a series of behavioral questionnaires and plate waste observations were assessed pre- and post-F2S programming. Results in the small, rural school district showed an increase in the percent of students able to correctly identify asparagus (34.5% to 58.6%; p= 0.02), an increase in the amount of fruit consumed; (53% to 94%; p = 0.01), and an increase in total produce consumed; (59% to 94%; p = 0.01). Results in the medium rural/urban school district showed an increase in the percent of students able to correctly identify red bell pepper (79% to 92%; p = 0.0415). Students increased their consumption of fruit and total produce at follow-up as compared to baseline (25% to 50%; p = 0.04 and 33% to 58%, p = 0.04). Results in the large, urban school district showed an increase in the percent of students able to correctly identify cucumber (39% to 61%; p=0.05). No significant positive changes were observed in student's willingness to taste a feature produce item, with a decrease willingness to taste a radish at the small, rural school district (90% to 62%; p= 0.01). Overall, results showed that limited intervention strategies yielded positive nutrition behavior changes in school-aged children's eating behavior. Results from these findings informed the wellness component of a multi-component project entitled the Shaping Healthy Choices Program. The Shaping Healthy Choices Program is a school-based intervention that integrated five components: nutrition education; family and community partnerships; regional agriculture; foods available on school site; and school wellness policies. The research discussed in Chapter V used a mixed-methods approach to assess the effectiveness of site-specific school wellness committees (SSWCs) to supplement Shaping Healthy Choices Program activities, and create a best practices model for sustainability of SHCP components. Community-Based Participatory Research principles and approaches were utilized to develop SSWC objectives. Baseline results from the School and Community Actions for Nutrition (SCAN) Survey served as a platform to launch SSWC objectives. An ethnographic approach was to analyze the level of community involvement during each phase of the project. The wellness committee was also involved in a smaller research project through which they identified the strengths and weakness of their school environment and implemented activities based on this information. Results from the wellness committee's study were compared with community-based principals in order to draw any relationships with the wellness committee outcomes and the level of community based participation. Results suggested that the more the community was involved in the planning, implementation, and maintenance phases of an activity, the more likely the School Sites were to implement these activities into the second year. More research is needed to assess the sustainability of the program outcomes over a longitudinal course. This dissertation concludes with Chapter VI, a discussion of research results, implications, conclusions, and future directions.
