Book excerpt: Concerted efforts by national programmes, supported by public–private partnerships, nongovernmental organizations, donors and academia under the auspices and coordination of the World Health Organization (WHO), have produced important achievements in the control of human African trypanosomiasis (HAT). As a consequence, the disease was targeted for elimination as a public health problem by 2020. The Sixty-sixth World Health Assembly endorsed this goal in resolution WHA66.12 on Neglected tropical diseases, adopted in 2013. National sleeping sickness control programmes (NSSCPs) are core to progressing in the control of the disease and in adapting to the different epidemiological situations. The involvement of different partners, as well as the support and trust of long-term donors, has been crucial for these achievements. More than 20 years of partnership among WHO, Sanofi and Bayer have enabled WHO to strengthen and sustain financial, technical and material support for the implementation of control activities in countries where HAT is endemic. The long-term support from the Government of Belgium, the Bill & Melinda Gates Foundation (BMGF) and other research institutions in the Democratic Republic of the Congo (DRC) has also been essential. WHO convened the fifth stakeholders meeting on the elimination of HAT due to infection with Trypanosoma brucei gambiense (g-HAT) and Trypanosoma brucei rhodesiense (r-HAT) in Geneva, Switzerland, on 7–9 June 2023. The meeting was held again in person after the coronavirus disease (COVID-19) pandemic and jointly for both forms of the disease. The previous meetings on g-HAT held in 2014, 2016 and 2018, as well as on r-HAT in 2015, 2017 and 2019, and jointly for g-HAT and r-HAT in 2021 (8) reinforced the partnership and commitment for HAT elimination and structured the mechanisms of collaboration within the WHO network for HAT elimination. The network includes NSSCPs, groups developing new tools, international and nongovernmental organizations involved in disease control, and donors. Fewer than 1000 cases of HAT annually have been reported over the past 5 years, which is a historic achievement. The area at risk has been substantially reduced. The elimination of HAT as a public health problem at the global level has been achieved. The new road map for neglected tropical diseases (NTDs) 2021−2030 (“the road map”) with the target to interrupt the transmission of g-HAT requires the strengthened and sustained efforts of all stakeholders, national authorities and partners, under WHO coordination. It will take disproportionally high efforts and innovative strategies to find the last cases of g-HAT and neutralize its transmission. Given the limited resources and other competing public health priorities, this is a challenge that requires our joint commitment.

HAT has a reputation for spectacular reappearances, and the latest peak of 40,000 reported and over 300,000 estimated cases only dates back to 1998. Efforts of the WHO and partners as well as the development of simpler and much better-tolerated treatments, improved diagnostics, and vector control tools made it possible to reduce this number by 95%. Case identification and confirmation remain complex and require specific skills, treatment remains error-prone and reports on long-term survivors have emerged, and the relevance of the animal reservoir for T. b. gambiense HAT needs clarification. In addition, to win the “end game” against this massively stigmatized disease, the human factor will play a key role. This Special Issue addresses many of the burning topics about disease elimination in its 12 research and 7 review articles and one case study. The papers critically reflect the approaches used, investigate the mentioned challenges, and propose novel approaches and interventions from various points of view.

Book excerpt: "The presence, or absence, of neglected tropical diseases (NTDs) can be seen as a proxy for poverty and for the success of interventions aimed at reducing poverty. Today, coverage of the public-health interventions recommended by the World Health Organization (WHO) against NTDs may be interpreted as a proxy for universal health coverage and shared prosperity - in short, a proxy for coverage against neglect. As the world's focus shifts from development to sustainable development, from poverty eradication to shared prosperity, and from disease-specific goals to universal health coverage, control of NTDs will assume an important role towards the target of achieving universal health coverage, including individual financial risk protection. Success in overcoming NTDs is a "litmus test" for universal health coverage against NTDs in endemic countries. The first WHO report on NTDs (2010) set the scene by presenting the evidence for how these interventions had produced results. The second report (2013) assessed the progress made in deploying them and detailed the obstacles to their implementation. This third report analyses for the first time the investments needed to achieve the scale up of implementation required to achieve the targets of the WHO Roadmap on NTDs and universal coverage against NTDs. INVESTING TO OVERCOME THE GLOBAL IMPACT OF NEGLECTED TROPICAL DISEASES presents an investment strategy for NTDs and analyses the specific investment case for prevention, control, elimination and eradication of 12 of the 17 NTDs. Such an analysis is justified following the adoption by the Sixty-sixth World Health Assembly in 2013 of resolution WHA6612 on neglected tropical diseases, which called for sufficient and predictable funding to achieve the Roadmap's targets and sustain control efforts. The report cautions, however, that it is wise investment and not investment alone that will yield success. The report registers progress and challenges and signals those that lie ahead. Climate change is expected to increase the spread of several vector-borne NTDs, notably dengue, transmission of which is directly influenced by temperature, rainfall, relative humidity and climate variability primarily through their effects on the vector. Investments in vector-borne diseases will avoid the potentially catastrophic expenditures associated with their control. The presence of NTDs will thereby signal an early warning system for climate-sensitive diseases. The ultimate goal is to deliver enhanced and equitable interventions to the most marginalized populations in the context of a changing public-health and investment landscape to ensure that all peoples affected by NTDs have an opportunity to lead healthier and wealthier lives."--Publisher's description.

Membrane transport proteins are crucial to maintain homeostasis and assure cell survival upon intracellular or environmental stress. A failure of any of these transport systems may have dramatic consequences for cell function. There is increasing evidence that membrane transport proteins play important functions in healthy conditions and that their absence or dysfunction may cause diseases. In recent years much attention has been paid to diseases resulting from defective transporters (“carrier diseases”) and ion channels (“channelopathies”). Very interestingly, altered expression of transporters has been described in several human pathologies. On this basis, many transport proteins are well acknowledged targets for drugs. Many others are involved in drug delivery and disposition and/or are considered potential targets. Others are off-targets for drugs and then, are responsible for side effects. Thus, membrane protein drug discovery is now an emerging field where the search for physiological mechanisms of regulation and for chemical compounds as modulators of transport activity, present new opportunities for drug development and for new therapies. This Research Topic addresses the latest research advances in membrane transport proteins, stimulating future research on these important protein families.

IVM can be used when the aim is to control or eliminate vector-borne diseases and can also contribute to insecticide resistance management. This toolkit provides information on where vector-borne diseases are endemic and what interventions should be used presenting case studies on IVM as well as relevant guidance documents for reference. The diseases that are the focus of this toolkit are malaria lymphatic filariasis dengue leishmaniasis onchocerciasis human African trypanosomiasis and schistosomiasis. It also includes information on other viral diseases (Rift Valley fever West Nile fever Chikungunya yellow fever) and trachoma. If other vector-borne diseases appear in a country or area vector control with an IVM approach should be adopted as per national priorities. Malaria as one of the most important vector-borne diseases in sub-Saharan Africa is the main focus of this document. Programmes targeting other vector-borne diseases can learn from the experiences gained from malaria vector control and presented here.

The 2018 TZG updates and expands on the guidance in the one previous jointly-developed, zoonoses-specific guidance document: the 2008 Tripartite “Zoonotic Diseases: A Guide to Establishing Collaboration between Animal and Human Health Sectors at the Country Level”, developed in WHO South-East Asia Region and Western Pacific Region. The 2018 TZG supports building by countries of the resilience and capacity to address emerging and endemic zoonotic diseases such as avian influenza, rabies, Ebola, and Rift Valley fever, as well as food-borne diseases and antimicrobial resistance, and to minimize their impacts on health, livelihoods, and economies. It additionally supports country efforts to implement WHO International Health Regulations (2005) and OIE international standards, to address gaps identified through external and internal health system evaluations, and to achieve targets of the Sustainable Development Goals. The 2018 TZG provides relevant country ministries and agencies with lessons learned and good practices identified from country-level experiences in taking OH approaches for preparedness, prevention, detection and response to zoonotic disease threats, and provides guidance on multisectoral communication, coordination, and collaboration. It informs on regional and country-level OH activities and relevant unisectoral and multisectoral tools available for countries to use.

The aim of the IVM approach is to contribute to achievement of the global targets set for vector-borne disease control, by making vector control more efficient, cost-effective, ecologically sound and sustainable. Use of IVM helps vector control programs to find and use more local evidence, to integrate interventions where appropriate and to collaborate within the health sector and with other sectors, as well as with households and communities. By reorientating to IVM, vector control programs will be better able to meet the growing challenges in the control of malaria, dengue and other vector-borne diseases in the face of dwindling public sector human and financial resources. This handbook presents an operational framework to guide managers and those implementing vector-borne disease control programs in designing more efficient, cost-effective systems. As a national IVM policy and an intersectoral steering committee are essential for establishing IVM as a national strategy, the handbook begins with the policy and institutional framework for IVM. Policy analysis is a means for identifying options for policy reform and suggesting instruments for implementing policy. IVM transforms the conventional system of vector control by making it more evidence-based, integrated and participative. This may require changes in roles, responsibilities and organizational links. The transition to IVM involves both reorientation of vector-borne disease control programs and embedding IVM within local health systems. Intersectoral partnerships and collaboration at both national and local levels will result in cost savings and benefits to other health services. Other relevant sectors, such as agriculture, environment, mining, industry, public works, local government and housing, should incorporate IVM and vector control into their own activities to prevent vector proliferation and disease transmission. Planning and implementing IVM involve assessing the epidemiological and vector situation at country level, analyzing the local determinants of disease, identifying and selecting vector control methods, assessing requirements and resources and designing locally appropriate implementation strategies. Solid evidence on the cost-effectiveness of interventions and their underlying parameters and a comprehensive vector surveillance system are essential for locally appropriate decision-making. Capacity-building, in particular human resource development, is a major challenge, because the IVM strategy requires skilled staff and adequate infrastructure at central and local levels. The handbook outlines the core functions and essential competence required for IVM at central and local levels, complementing a separate set of documents containing the Core structure for training curricula on integrated vector management and associated training materials. Like any new approach, IVM must be actively advocated and communicated in order to become established. The handbook lays out the elements and processes of IVM to enable policy-makers, donors and implementing partners to use it for vector-borne disease control. During the period of transition and consolidation of an IVM strategy, regular feedback is required on performance and impact in order to ensure continued support. The general public must also be made aware of the strategy and participate in its implementation. The communication tools for reaching the public are the media and various types of educational interventions to increase their knowledge and skills, which should lead to behavioral change and empowerment. The final section presents a comprehensive framework for monitoring and evaluation of IVM, covering aspects discussed in the previous sections. Indicators and methods for measuring process, outcomes and impact are proposed.
