Book excerpt: Excerpt from Report of the Commissioners of Lunacy to the Commonwealth of Massachusetts: January, 1875 Another thought connected with this review is, that almost one-half of all the insane in the State remain still outside of the hospitals. It is true, some of them may be well provided for; but as far as cure and treatment are concerned, they surely cannot, in the acute stages of the disease, have as good advantages. Besides, what aggravates the matter is, that we know so little about them as a whole. There are undoubtedly some acute cases of insanity in private houses and in alms houses that should at once he transferred to the hospital. The commission of 1854 found over three hundred such cases, and there is good reason to believe such still exist. This ignorance and these wrongs will continue until some steps are taken by legislation to cause this whole field to be more carefully surveyed, and proper measures taken to correct existing evils. Grob's Mental Institutions in America: Social Policy to 1875 has become a classic of American social history. Here the author continues his investigations by a study of the complex interrelationships of patients, psychiatrists, mental hospitals, and government between 1875 and World War II. Challenging the now prevalent notion that mental hospitals in this period functioned as jails, he finds that, despite their shortcomings, they provided care for people unable to survive by themselves. From a rich variety of previously unexploited sources, he shows how professional and political concerns, rather than patient needs, changed American attitudes toward mental hospitals from support to antipathy. Toward the end of the 1800s psychiatrists shifted their attention toward therapy and the mental hygiene movement and away from patient care. Concurrently, the patient population began to include more aged people and people with severe somatic disorders, whose condition recluded their caring for themselves. In probing these changes, this work clarifies a central issue of decent and humane health care. Gerald N. Grob is Professor of History at Rutgers University. In 1860, her husband, a strong-willed Calvinist minister, committed her to an Illinois insane asylum in an effort to protect their six children and his church from what he considered her heretical religious ideas. Upon her release three years later (as her husband sought to return her to an asylum), Packard obtained a jury trial and was declared sane. Before the trial ended, however, her husband sold their home and left for Massachusetts with their young children and her personal property. His actions were perfectly legal under Illinois and Massachusetts law; Packard had no legal recourse by which to recover her children and property. This experience in the legal system, along with her experience as an asylum patient, launched Packard into a career as an advocate for the civil rights of married women and the mentally ill. She wrote numerous books and lobbied legislatures literally from coast to coast advocating more stringent commitment laws, protections for the rights of asylum patients, and laws to give married women equal rights in matters of child custody, property, and earnings. Despite strong opposition from the psychiatric community, Packard's laws were passed in state after state, with lasting impact on commitment and care of the mentally ill in the United States. Packard's life demonstrates how dissonant streams of American social and intellectual history led to conflict between the freethinking Packard, her Calvinist husband, her asylum doctor, and America's fledgling psychiatric profession. It is this conflict--along with her personal battle to transcend the stigma of insanity and regain custody of her children--that makes Elizabeth Packard's story both forceful and compelling.
