Book excerpt: It is expected that by 2030, elderly minorities will comprise 20.2 million of the total 72.8 million population of Americans over 65 years (Pollard, 2013). For African American women over the age of 50, 5% have osteoporosis and an estimated 35% have low bone mass (NOF, n.d.). It is estimated that while over 40% of African American women over 80 years meet criteria for treatment, yet less than 12% are actually treated (Cauley, 2011). Striking disparities exist between African American and Caucasian women’s likelihood for dual-energy x-ray absorptiometry (DXA) referral, the measurement tool that is used to diagnosis osteoporosis. (Hamrick, Cao, Agbafe-Mosley & Cummings, 2012; Miller et al., 2005; Wanko, 2003). When African American females suffer a hip fracture, they show a higher morbidity and mortality rate when compared to their Caucasian counterparts (Geller & Derman, 2001; Kidambi, Partington, & Binkley, 2005; NOF, 2014). A disproportionate number of African Americans are limited in their physical and mental abilities after a fracture compared to Caucasian patients (Bohannon, 1999; Geller & Derman, 2005; Furstenberg & Mezey, 1987). African American women are twice more likely to die from a hip fracture than Caucasian women within the first year of the fall (Geller & Derman, 2001). Interventions are needed to identify health care provider biases regarding osteoporosis screening (Neuner, J., Zhang, X., Sparapani, R., Laud, P. & Nattinger, A., 2007). Many, including African Americans, perceive only Caucasians to be susceptible to the disease (Geller & Derman, 2001; NOF, 2013). Further, the National Osteoporosis Foundation (NOF) found the average age for discussion initiation between mothers and daughters about osteoporosis does not occur until after peak bone mass is reached (2011). Purpose: This research project aimed to investigate the relationship between osteoporosis knowledge, bone mineral density (BMD), risk factors of low BMD, and preventative behaviors among African American and Caucasian mothers and their biological adolescent daughters. Additionally, this study seeks to explore whether mothers and daughter discuss osteoporosis preventative behaviors. Methods: Thirty-seven pairs of African American and Caucasian mothers and their 13-18 year old biological daughters participated in this study. Femoral and spinal BMD using DXA were obtained. Participants also completed the Osteoporosis Risk Factor Assessment (ORFA), Osteoporosis Knowledge Test (OKT), Health Communication Survey and an online calcium quiz to measure preventative behaviors, knowledge, calcium intake, and osteoporosis discussion engagement. Results: Over a third (33%) of African American and 26% of Caucasian mothers in this study had osteopenic BMD at the hip and spine. African American mothers and daughters had lower overall spinal and hip bone density compared to their Caucasian counterparts. Body fat and BMI among African American and Caucasians were significantly different. There were no significant differences in mean spinal or femoral BMD based on the predictor variables. Book excerpt: Background: Osteoporosis is estimated to affect 200 million women in the world, affecting 10% of women aged 60, 20% of women aged 70, 40% of women aged 80 and 67% of women aged 90. Osteoporosis is characterized by low bone density and increases the risk for fractured bones; however, it may be prevented with modifiable factors such as supplements, diet, and physical activity. Vitamin D deficiency leads to bone mineral density loss, as Vitamin D3 is responsible for calcium absorption into the bones. Bone consolidation is believed to occur between 20 and 30 years old; thus, attaining peak bone mass is critical during pre-menopause. Methods: The relationship between vitamin D and bone mineral density has predominately been studied in postmenopausal populations. Therefore, we examined this association among 18-30 year old participants (n=271) in the cross-sectional UMass Vitamin D Status Study. The modified version of the Harvard Food Frequency Questionnaire was used to assess the average intake of vitamin D foods and supplements. Serum 25(OH)D3 concentrations were assayed from blood samples. Bone mineral content and bone area were measured by dual-energy X-ray absorptiometry scan. Bone mineral content (BMC), as measured in grams, provides a measure of bone mass. Bone area (BA), as measured in cm2, reflects a two-dimensional area, which is characterized by the periphery of a bone region. We used multivariable linear regression to model the relationship between bone mineral density and bone area with sources of vitamin D after adjusting for dietary and lifestyle factors. Results: In the present study, the mean and standard deviation of vitamin D is 372.7 IU and 285.8 IU, respectively. For vitamin D from supplements, the mean is 140.9 IU with a standard deviation of 232.3 IU. Finally, for vitamin D from food, the mean is 231.8 IU with a standard deviation of 182.0 IU. Compared to reference values of 600 IU, these data are below the recommended daily allowance. We did not observe an association between total vitamin D or vitamin D from foods sources with either BMC or BA. We also did not observe an association between serum 25-hydroxyvitamin D levels and BMC or BA. Conclusion: Future studies with larger sample sizes are warranted to validate this association among young premenopausal women.

Adipose tissue, especially visceral fat, is an important source of inflammatory markers, which contributes to the development of a pro-inflammatory state. Conversely, high levels of physical activity and exercise have an anti-inflammatory effect. One-hundred and two healthy premenopausal women participated in a 5-year longitudinal observational study (MONET: Montreal Ottawa New Emerging Team). The present secondary analyses were performed on 58 participants between the ages of 47 and 54 years with a full set of data.The aim of study was to investigate the impact of menopause transition and physical activity on inflammatory makers. The major finding of the first of 3 studies was that menopausal transition is accompanied by an increase in inflammatory markers, namely ferritin, IL-8, and sTNFR 1 and 2. The increase in IL-8 and sTNFR2 with menopause could be explained, in part, by changes in fat mass and peripheral fat, respectively. During and after menopause, significant bone loss occurs in women due to reduced estrogen production. Estrogen reduction favors bone resorption by regulating the production and activity of inflammatory markers. Therefore we further investigated the association between inflammatory markers and bone mineral density in premenopausal women transitioning to menopause (paper 2). Our results showed no significant association between change in inflammatory markers and change in bone mineral density in women transitioning to menopause. However, in premenopausal women hs-CRP was negatively associated with total, lumbar spine and femoral neck bone mineral density and along with weight and cardiorespiratory fitness may play a role in bone mineral density variation. Baseline level of hs-CRP, Hp, IL-6 and femoral neck bone mineral density along with percent change in physical activity energy expenditure and menopausal status partly explained the individual variation of bone mineral density losses in women transitioning to menopause. Finally, we investigated time spent in the postmenopausal years and the influence of the duration of the postmenopause status on body composition and cardiometabolic risk factors. We indicated that postmenopausal years and years since menopause is associated with decrease in blood glucose and increase in waist circumference, percent fat mass, total cholesterol, and high density lipoprotein. Inflammatory markers including ApoB, ferritin, adiponectin, sCD14 were higher during years after final menstrual period while sTNFR1 and sTNFR2 were higher during the menopause transition and early postmenopausal years.

Book excerpt: This study was designed to investigate the relationship between retinol intake, bone mineral density, and falls in 101 elderly women aged 72 to 90 years (78.6 yrs. ± 4.3 yrs.). Bone mineral density (BMD) (g/cm2) of the left hip, anterior-posterior lumbar spine (L3), and lateral spine (L3) was measured using dual-energy x-ray absorptiometry. Dietary intake and physical activity were assessed by validated questionnaires (the 100-item Block Food Frequency Questionnaire and the Physical Activity Scale for the Elderly, respectively). Isometric hip abduction strength of the right and left legs was assessed using a hand-held dynamometer. Fall surveillance was collected using a "postcard" system at three-month intervals over a two-year period. Multiple regression analyses were used to show the predictability of retinol, vitamin D, calcium, years past menopause, years on hormone replacement therapy, and physical activity on BMD variables. Together, these variables explained 14% of the variance in total hip BMD at follow-up (R2=0.14, SEE=0.12, p=0.020), 26% of the variance in the anterior-posterior spine BMD at follow-up BMD (R2=0.26, SEE=0.17, p=0.051), and 33% of the variance in lateral spine BMD at follow-up (R2=0.33, SEE=0.10, p=0.009). Two-year changes in hip BMD were poorly predicted using the model with only 5% of total hip BMD variance being explained by the six independent variables (R2=0.05, SEE=0.03, p=0.558). Logistic regression was used to determine whether the likelihood of being a faller vs. a non-faller could be predicted from a model using retinol, vitamin D, average hip strength, and physical activity. It was shown that 11.5% of the variability in fall status could be explained by the model (Cox & Snell's R2=0.115). Using an ROC curve analysis, the model correctly classified 69% of the individuals into the correct "fall category". We conclude that retinol, although not an independent predictor of BMD or fall status, is an important component in the prediction of both BMD and falls. Further interventional research is needed to determine the effects of retinol on BMD and falling.
