Book excerpt: Functionality is a measurement of the ability of an individual to perform simulated daily tasks providing insight into the physical fitness of older adults. The overall purpose of this study was to examine the relationship between body composition, including bone mineral density (BMD) and weight/body mass index (BMI) with functionality performance in 37 overweight (BMI of 31.3"4.8 kg/m2) postmenopausal Caucasian women, aged 54.7"3.8 y. Participants completed functionality tests including: 8 meter normal and brisk walks, 8 foot get up-and-go, handgrip strength assessment, one leg stance, and the chair-stand test. Dual X-ray absorptiometry technique with a Lunar iDXA instrument (GE Medical Systems, Madison, WI) was used to measure BMD of the total body, lumbar spine, dual femur (neck, Ward's triangle, trochanter, shaft, and total) and forearm (ulna and radius at ultradistal and 1/3 distance). BMI was less closely correlated with various functionality measures than age but was most correlated with ability to balance on one leg (r=-.465, P=.004) and normal step length (r=.354, P=.032). Book excerpt: A meta-analysis was done to determine what effect exercise has on the bone mineral density (BMD) of the lumbar spine, proximal femur, and distal radius in postmenopausal women, utilizing data from 18 studies. Treatment effects (TE) were calculated for each of the skeletal sites, aerobic and strength training, as well as methods of BMD measurement (dual energy x-ray absorptiometry and dual photon absorptiometry). The resulting TEs found exercise, in general, to significantly increase (p c 0.05) the BMD of the lumbar spine (0.73%), and the proximal femur (0.35%); however, the distal radius showed a decrease ( -0.91%) in BMD. When looking for the effect of aerobic versus strength training, significant (p c 0.05) increases in BMD were found in the spine and femur. Since positive changes were not seen in all sites, the benefits of exercise may be specific to the skeletal sites which the working muscles are attached. When the % change for exercise and control groups were analyzed separately, the exercise group increased 2.31% at the lumbar spine and 0.61% at the proximal femur. However, the exercise group decreased -2.21% at the radius. The control group decreased at the lumbar spine -1.09%, and at the femur -1.1 I%, however, increased at the radius 0.36%. The BMD % changes were significantly larger for the spine and femur, therefore, vigorous exercise along with resistance training may help slow the decrease in BMD as one ages.

Book excerpt: The overall purpose of this study is to determine the relationship between skeletal and oral bone density, identify factors influencing bone loss, and determine the relationship between osteoporosis and oral bone loss, periodontal disease and tooth loss. We hypothesize that reduction in bone density leading to osteoporosis, plays a significant role in increasing susceptibility to destructive periodontitis and tooth loss. Sensitive and accurate measures of skeletal and oral bone mineral density, periodontal disease and tooth loss will be used. A wide variety of other risk factors for both osteopenia and periodontal disease will be assessed. A total of 1463 subjects were recruited from an ongoing NIH funded study cohort, the Women's Health Initiative (WHI) and are participants of this study. In this year findings were presented at the Congress of Epidemiology (June 2001, Toronto Canada) and at the International Association for Dental Research Meeting (June 2001, Chiba Japan). Data were presented for the first 608 study participants enrolled from the larger study of over 1400 women. Data analysis is ongoing. Copies of published abstracts and descriptive analysis of the 1463 enrolled is presented in this report.
