Book excerpt: As of December 2012, Operation Enduring Freedom (OEF) in Afghanistan and Operation Iraqi Freedom (OIF) in Iraq have resulted in the deployment of about 2.2 million troops; there have been 2,222 US fatalities in OEF and Operation New Dawn (OND)1 and 4,422 in OIF. The numbers of wounded US troops exceed 16,000 in Afghanistan and 32,000 in Iraq. In addition to deaths and morbidity, the operations have unforeseen consequences that are yet to be fully understood. In contrast with previous conflicts, the all-volunteer military has experienced numerous deployments of individual service members; has seen increased deployments of women, parents of young children, and reserve and National Guard troops; and in some cases has been subject to longer deployments and shorter times at home between deployments. Numerous reports in the popular press have made the public aware of issues that have pointed to the difficulty of military personnel in readjusting after returning from Iraq and Afghanistan. Many of those who have served in OEF and OIF readjust with few difficulties, but others have problems in readjusting to home, reconnecting with family members, finding employment, and returning to school. In response to the return of large numbers of veterans from Iraq and Afghanistan with physical-health and mental-health problems and to the growing readjustment needs of active duty service members, veterans, and their family members, Congress included Section 1661 of the National Defense Authorization Act for fiscal year 2008. That section required the secretary of defense, in consultation with the secretary of veterans affairs, to enter into an agreement with the National Academies for a study of the physical-health, mental-health, and other readjustment needs of members and former members of the armed forces who were deployed in OIF or OEF, their families, and their communities as a result of such deployment. The study consisted of two phases. The Phase 1 task was to conduct a preliminary assessment. The Phase 2 task was to provide a comprehensive assessment of the physical, psychologic, social, and economic effects of deployment on and identification of gaps in care for members and former members, their families, and their communities. The Phase 1 report was completed in March 2010 and delivered to the Department of Defense (DOD), the Department of Veterans Affairs (VA), and the relevant committees of the House of Representatives and the Senate. The secretaries of DOD and VA responded to the Phase 1 report in September 2010. Returning Home from Iraq and Afghanistan: Assessment of Readjustment Needs of Veterans, Service Members, and Their Families fulfills the requirement for Phase 2.

Book excerpt: "The impact of service-related trauma on those returning from deployment remains a considerable issue among United States service members. While some veterans experience few to no setbacks upon homecoming, many service members struggle to reintegrate into their stateside lives, families, and communities following deployment. Little is yet understood about how veterans utilize social support to process trauma, the impact of trauma on service members' abilities to access the support available to them, and whether social support may help soldiers with trauma to reintegrate into civilian communities with greater success. The purpose of this study is to add to the body of literature concerning service members' experiences of trauma, social support, and postdeployment reintegration. Over the past five years, the Veterans Administration has observed a 35% increase in the number of service members receiving mental health care. Moreover, completed suicides among service members are at an all-time high, with more service member lives lost to suicide than to combat in recent years (Blumenthal, Maliha, & Mathews, 2012; Donnelly, 2011; Pilkington, 2013). Much is now known about the risk and resiliency factors that contribute to a service member's ability to metabolize service-related trauma and reintegrate stateside. Yet, despite the resources devoted to studying these influences and the increased utilization of mental health services, outcomes for many returning veterans continue to be bleak. As the drawdown of OIF/OEF/OND troops continues and the potential deployment of ground troops to combat ISIS/ISIL looms, the demand for a comprehensive understanding of the unique needs of those service members experiencing reintegration difficulties can be expected to increase. Since stigma surrounding mental health treatment remains high among military service members (Johnson & Faller, 2011; Zoroya, 2010), many veterans will remain untreated for months, or even years, before life becomes uncomfortable or unmanageable enough to press them into treatment (Hoge et al., 2004). Civilian and military counselors, social workers, clergymen, public health educators, psychologists, psychiatrists, and service members themselves need to be better informed about the manifestations of combat-related trauma, the influence of social support on trauma symptomology, and the potential role of social support in ameliorating trauma response and facilitating service member reintegration. Mental health workers with a deeper knowledge of how trauma and social support interact to inform postdeployment experiences will be better suited to facilitate stateside transitions in service members. Accordingly, a Web-based assessment was deployed to examine the relationships among the variables of service-related trauma, postdeployment social support, and reintegration in a sample of veterans of the conflicts in Iraq and Afghanistan. Subsequent epidemiologic studies conducted on military and veteran populations that served in the operations in Afghanistan and Iraq provided scientific evidence that those who fought were in fact being diagnosed with mental illnesses and experiencing mental healthâ€"related outcomesâ€"in particular, suicideâ€"at a higher rate than the general population. This report provides a comprehensive assessment of the quality, capacity, and access to mental health care services for veterans who served in the Armed Forces in Operation Enduring Freedom/Operation Iraqi Freedom/Operation New Dawn. It includes an analysis of not only the quality and capacity of mental health care services within the Department of Veterans Affairs, but also barriers faced by patients in utilizing those services.

As women veterans increase in number and reintegrate to civilian society, their challenges are becoming more apparent in the civilian community. Despite the presence of Veterans Affairs and other non-profit organizations effort to moderate these challenges by improving efforts to address barriers that may impede women veterans from accessing services or resources, it seems that the availability of resources and services are not enough to minimize reintegration challenges for women veterans which emanate from intrapersonal problems such as loss of identity, impaired sense of belonging, and shame or unworthiness. These intrapersonal problems may be the reason women veterans experience disempowerment in their community or avoid seeking help in the reintegration process. Therefore, the purpose of this project is to examine the underlying problems in the reintegration processes that affect women veterans' struggles, then supplement the gaps in services through social support in a support group format.

Research from multiple scientific fields suggests that women veterans have unique issues from health (both mentally and physically) to basic hierarchy of needs in a society that continues to perceive veterans as men. This has hindered the lack of care, acknowledgement, services, and respect that women veterans receive or expect over time. ¶ Purpose: To further understand the challenges of women veterans as described by key veteran informants, social workers, and other providers that work with women veterans and also to gather recommendations from them on what can be done to further address those challenges. ¶ Methods: Qualitative interviews were conducted with social workers and other providers to obtain knowledge about the experiences women veterans encounter after they return home with unmet needs, and develop recommendations for improving services. ¶ Results: Based on the variety of participants, there is a gamut of recommendations of how women veterans can be helped, what needs to be done, and a lack of understanding of the underlying issues that female veterans have faced and will face. Themes included: 1). Common experiences of women veterans: trauma-PTSD, Military Sexual Trauma (MST), and social bias; 2). Coming Home - challenges in services and assistance provided: dealing with Veterans Affairs, non-gender specific programming, and lack of education by providers; 3). What works: stand-alone sites, gender specific and responsive services, follow-up services, and peer to peer; and, 4). More needs to be done. ¶ Discussion/Conclusion: Suggestions included peer to peer services, transforming the VA Hospitals to recognize the need for gender specific care, providers understanding the transitions women have made throughout the many years they have served with and alongside males, and perhaps the creation of a program prior to service discharge to support women veterans. Based on the information gathered, more research should be done on a larger scale.
