Le comportement des patients participe à cette inflation : en effet, on constate dans ces services une forte augmentation des demandes pour des soins non urgents, et pour des patients venus sur leur propre initiative, non adressés par des professionnels de santé. Pour répondre à cette demande de soins non programmés, il est nécessaire de pouvoir proposer aux patients une offre de soins adaptée. Pour cela, il est nécessaire de s'enquérir de l'opinion des patients, d'étudier et de comprendre leur comportement, et notamment de connaître les causes de ce recours spontané aux services des urgences. Méthode : Enquête transversale par questionnaire, réalisée auprès de patients de la maison de santé de Steenvoorde ayant été consultés de manière spontanée auprès d'un service d'urgences, en général celui de l'hôpital d'Hazebrouck, incluant 125 répondants. Résultats : La raison principale évoquée par le patient consultant spontanément dans un service d'urgences est la nécessité de bénéficier d'un plateau technique et/ou d'un geste technique. Le cause du recours concerne le plus souvent un problème d'ordre traumatique. Le consultant est le plus souvent un homme, actif, d'âge moyen (35-50 ans). Le recours au service des urgences est plus important en dehors des heures d'ouverture de la maison de santé, et ce durant le week-end mais également en fin d'après midi. Il est constaté que plus de la moitié des patients ont bénéficié d'un examen d'imagerie. La majorité des patients se dit satisfaite de la prise en charge. Celle-ci est relativement rapide puisque inférieure à 2 h pour les 2/3 des patients. Conclusion : Dans le contexte actuel où il existe une diminution préoccupante de la démographie médicale, notamment en zone rurale, la prise en charge de ces soins non programmés apparaît d'autant plus complexe. Une des solutions est le développement de structures telles que les maisons de santé, qui de part leur concept permettent d'offrir un meilleur accès aux soins. La maison de santé de Steenvoorde en est un exemple avec la création prochaine d'une « salle d'urgences et d'accueil des soins non programmés ». Un autre point essentiel à la prise en charge des soins non programmés serait d'optimiser le recours au système de régulation des soins primaires, apparaissant à ce jour sous-utilisé.
