Elle est souvent atypique et pose des problèmes diagnostiques et thérapeutiques. METHODE. Etude épidémiologique chez des patients de plus de 70 ans vus en consultations au cabinet médical ou à leur domicile, quelque soit le motif de consultation. Les diagnostics de dépression caractérisée et de dépression mineure ont été posés en utilisant l'échelle d'évaluation du M.A.D.R.S. RESULTATS. 75 patients répartis chez 7 médecins généralistes du Val-de-Marne ont été rencontrés sur 6 mois. Chaque patient avait en moyenne 4 affections chroniques. 27.3% des patients présentaient un syndrome dépressif avéré et 16.9% une dépression dite mineure. 25 % des patients prenaient des psychotropes et seuls 20% des patients ayant une dépression mineure étaient traités. Dans 50% des cas il existait un évènement traumatique récent. CONCLUSION. La dépression chez le sujet âgé est une réalité. Le dépistage et la prise en charge sont malaisés du fait de la complexité des effets du vieillissement d'une part et de la poly médication et ses conséquences de ces patients d'autre part

Malgré des conséquences parfois dramatiques, son diagnostic est, et reste encore malaisé en médecine ambulatoire. Matériel et méthode : Etude descriptive au sein d'une population de personnes âgées d'au moins 70 ans, consultant, quel que soit le motif, au cabinet médical de trois médecins généralistes situés en zones rurales dans le Jura. Le diagnostic de dépression a été posé en utilisant l'échelle de dépression gériatrique, la GDS à 30 items. Résultats : Cent dix patients, âgés de 71 à 101 ans, ont été inclus entre février et avril 2012. La moyenne d'âge était de 80,5 ans et le sex-ratio homme/femme de 0,57. La prévalence de patients présentant possiblement un syndrome dépressif était de 41,9 %. Parmi ceux-ci, 60,9 % consultaient pour un renouvellement d'ordonnance. La tranche d'âge la plus touchée par la dépression était celle des 85-89 ans avec 63,2 % de patients dépressifs. Les femmes plus touchées par cette pathologie (45,7 % d'entre elles ; contre 35 % parmi les hommes). Les patients vivants seuls présentaient plus souvent un syndrome dépressif (47,4 %) que les patients mariés ou vivants en couple (38 %). Discussion : La dépression du sujet âgé est une réalité, encore plus marquée en milieu rural dans le Jura. Son dépistage est rendu difficile notamment par une symptomatologie parfois atypique mais surtout par l'absence de plainte de la part du patient.
